New-Onset Mental Illness Among Gestational Carriers.

IMPORTANCE: Pregnancy is a period of increased vulnerability for psychological well-being. The mental health of gestational carriers is understudied. OBJECTIVE: To study the association between gestational carriage and new-onset mental illness. DESIGN, SETTING, AND PARTICIPANTS: This population-based, retrospective cohort study included all women from the entire province of Ontario, Canada, without known mental illness before pregnancy and who gave birth at greater than 20 weeks' gestation from April 1, 2012, to March 31, 2021. Data were analyzed from September 10, 2024, to May 23, 2025. EX

Using family members as gamete donors or gestational carriers.

The use of adult intrafamilial gamete donors and gestational surrogates is generally ethically acceptable when all participants are fully informed and counseled, but consanguineous arrangements or ones that simulate incestuous unions should be prohibited. Adult child-to-parent arrangements require caution in order to avoid coercion, and parent-to-adult child arrangements are acceptable in limited situations. Programs that choose to participate in intrafamilial arrangements should be prepared to spend additional time counseling participants and ensuring that they have made free, informed decisi

Navigating the gestational surrogacy seas: the legalities and complexities of gestational carrier services.

This paper offers a comprehensive review of the gestational surrogacy process in the US, as well as internationally, focusing on the legal and ethical challenges that gestational carriers, intended parents, fertility providers, and OB/GYNs may face. The objective of this review article is to serve as an overview and provide information on legal, cultural, and ethical aspects of the decisions to pursue gestational surrogacy both for intended parents and gestational carriers in the US and globally. By understanding the surrogacy landscape and the obstacles, the surrogacy agencies and other invol

Genetic counseling decisions in gestational carrier pregnancies.

OBJECTIVE: Pregnancies conceived as contracted gestational carriers are a relatively new phenomenon for reproductive medicine. Since the intended parents control genetic screening decisions, there may be differences in genetic decisions made for gestational carrier (GC) in vitro fertilization (IVF) pregnancies as compared to traditional non-gestational carrier IVF pregnancies. Our goal was to investigate the frequency and types of these genetic testing decisions. METHODS: We performed a retrospective study of GC pregnancies counseled at a private maternal-fetal medicine practice between Janua

Gestational trophoblastic neoplasia following assisted reproductive technology in a gestational carrier: A case report.

BACKGROUND: This case report presents the treatment of a gestational trophoblastic neoplasia pregnancy after in vitro fertilization (IVF). The incidence of moles in pregnancies resulting from IVF is rare, although studies show that the risk of their occurrence is not eliminated. CASE PRESENTATION: This report describes a 30-yr-old multiparous woman with secondary infertility and molar pregnancy after IVF. The patient had conceived with IVF treatment after 10 yr of secondary infertility. Suction and evacuation were performed under ultrasound guidance, and the patient was followed up with seria

Gestational Carrier Pregnancies: Legal and Ethical Considerations for Pediatricians.

This case explores the legal and ethical considerations for pediatricians surrounding gestational carrier pregnancies in the United States. Because of high success rates for assisted reproduction, state laws supporting same-sex adoption and surrogacy, and established legal precedents, gestational carrier pregnancies are increasingly common. The case presented involves a gestational carrier in preterm labor at 30 weeks' gestation with malpositioned twins who declines a cesarean delivery. Three commentaries are presented. The first highlights the importance of understanding the ethical implicati

Gaucher disease carrier with gestational thrombocytopenia and anemia: a case report.

BACKGROUND: Gaucher disease is an autosomal recessive inborn error of metabolism that causes disorders of blood, bone, and central nervous system as well as hepatosplenomegaly. We present the case of a carrier of Gaucher disease with gestational thrombocytopenia and anemia that required blood transfusion therapy. CASE PRESENTATION: A 24-year-old Nepalese primipara was diagnosed with idiopathic thrombocytopenia at 12 weeks of gestation. Her platelet count had reduced to 30,000/µL at 21 weeks of gestation, and the hemoglobin content reduced to 7.6 g/dL at 27 weeks of gestation. As she did not r

Reproductive Travel of Intended Parents for Delivery of Gestational Carrier Pregnancies.

OBJECTIVE: To explore the role of reproductive travel (travel to another state or country for reproductive services) for intended parents at the time of delivery of gestational carrier pregnancies and to analyze the sociodemographic characteristics of those who build families through gestational surrogacy. METHODS: We conducted a cross-sectional study of births involving gestational surrogacy in Utah from 2009 to 2018. Data were obtained from birth certificates. State and country of residence were collected for intended parents, and the legal climates of these locations were assessed by revie

Normal infant by a gestational carrier for a phenylketonuria mother: alternative therapy.

The consequences of pregnancies in untreated phenylketonuria (PKU) mothers are a high incidence of spontaneous abortion, intrauterine growth retardation with microcephaly, congenital malformations, and abnormal intellectual development. PKU fathers, on the other hand, produce normal children. Obviously children of PKU women and men are at least heterozygous, proving that the abnormalities produced by the PKU mothers are not genetic but "intrauterinely environmental." Exposure to the mother's metabolic abnormalities affects the fetus during the entire pregnancy. A PKU mother can produce a healt

Mild obesity does not affect perinatal outcome in gestational carrier cycles.

STUDY QUESTION: Does BMI of gestational carriers (GCs) affect perinatal outcomes after embryo transfer? SUMMARY ANSWER: Overweight and class I obesity in GCs does not affect the rate of good perinatal outcomes. WHAT IS KNOWN ALREADY: The use of GCs is increasing, but uniform guidance regarding optimal BMI for GCs is lacking. Women with obesity who conceive without fertility treatment or through autologous or donor in vitro fertilization are at higher risk of adverse maternal and fetal outcomes, but data on obesity in GCs are very limited. STUDY DESIGN, SIZE, DURATION: We performed a retrosp

Ovarian Ectopic Pregnancy as IVF Complication: First Report in a Gestational Carrier.

Ovarian pregnancy is a rare subtype of ectopic pregnancy with an increased incidence after assisted conception. We present a 31-year-old gestational carrier who presented with suprapubic and pelvic pain at 6 weeks and 2 days' gestation. An ultrasound scan demonstrated an empty uterus and a complex mass in the left adnexa. Operative laparoscopy was performed and an ovarian pregnancy was found and treated. We believe this to be the first report of ovarian pregnancy after IVF in a gestational carrier. Appropriate counselling of surrogate mothers is of utmost importance as the risk of ectopic preg

In vitro fertilization with cryopreserved oocytes or embryos after cancer: The role of gestational carriers.

BACKGROUND: Fertility preservation counseling is recommended for reproductive-age cancer patients. Gestational carriers are individuals who carry a pregnancy for someone else. This service creates a path to biological children when cancer treatment necessitates the removal of the uterus. The authors examined the involvement of gestational carriers among women diagnosed with cancer. METHODS: Using data from eight statewide cancer registries linked with the Society for Assisted Reproductive Technology Clinic Outcomes Reporting System, the authors identified women with a cancer diagnosis who sub

Examining pre-term birth and cesarean section rates in gestational carrier pregnancies.

PURPOSE: This study sought to report on the route and gestational age at delivery of gestational carrier (GC) pregnancies with respect to the GCs' prior obstetric history. METHODS: A retrospective analysis of all GC pregnancies from one of the largest surrogacy agencies in California between 2008 and 2018 was performed. Available demographic data and obstetric history, including a history of prior cesarean section (CS) and preterm birth (PTB), were collected for each GC and correlated to outcomes of the index GC pregnancy. Primary outcomes for the index GC pregnancies included delivery route
