Long-term non-invasive ventilation in children: Transition from hospital to home.

Long-term non-invasive ventilation (NIV) is an accepted therapy for sleep-related respiratory disorders and respiratory insufficiency or failure. Increase in the use of long-term NIV may, in part, be driven by an increase in the number of children surviving critical illness with comorbidities. As a result, some children start on long-term NIV as part of transitioning from hospital to home. NIV may be used in acute illness to avoid intubation, facilitate extubation or support tracheostomy decannulation, and to avoid the need for a tracheostomy for long-term invasive ventilation. The decision ab

Discharge Readiness and Patient Experience in the Hospital-to-Home Transition After Digestive Surgery.

Background/Objectives: Complete postoperative recovery increasingly occurs after hospital discharge, particularly in the context of enhanced recovery after surgery (ERAS) programmes. However, patient-reported experience during the transition from hospital to home after digestive surgery remains poorly characterized. This study aimed to evaluate patient-reported experience and to explore its associations with perceived quality of recovery at discharge, surgical complexity, postoperative complications, and unplanned healthcare utilization. Methods: This study analyzed a prospective observational

Challenges of Care Transition From Hospital to Home for Older Colorectal Surgery Patients: Surgeons' Perspectives.

OBJECTIVE: Postdischarge transitions from the hospital to home in older (≥65 years) colorectal surgery patients have a high risk of medication errors, complications, and worsening of existing conditions. Up to 14% are readmitted within 30 days, costing ~$180 million annually. The anticipated 50% increase in colorectal cancer surgeries in older adults by 2040 necessitates an improvement in care transitions and outcomes. METHODS: We conducted semi-structured qualitative interviews with 10 surgeons from 8 US health systems to inform the design of a multicomponent care transition model. We select

Making the transition from hospital to home: caring for the newly diagnosed child with cancer.

Current trends in the management of health care show increasing pressure for earlier discharge of children, and their families may or may not be ready to continue the child's care at home. This article provides information that helps the home care provider bridge the transition from hospital care to home care. Care frequently is coordinated by a hospital-based clinical nurse specialist who provides psychosocial support, reinforces teaching, minimizes risks for complications, and performs home infusion therapy, such as antibiotics, chemotherapy, analgesics, total parenteral nutrition, and biolo

Living the transition: Experiences of patients receiving palliative care from hospital to home - A phenomenological study.

PURPOSE: Transitions from hospital to home in palliative care are emotionally and organisationally complex, influencing patients' dignity, safety, and continuity of care. Although previous studies have focused largely on professionals and informal caregivers, patients' own perspectives remain insufficiently understood. This study aimed to explore the lived experience of adults receiving palliative care during the transition from hospital to home. METHODS: A descriptive phenomenological design guided by Giorgi's method was used. Twenty adults (n = 20) recently discharged from hospital and rece

Transition from Hospital to Home-Based Care for Older Adults in Southwestern Uganda: Informal Caregiver's Experiences.

BACKGROUND: The transition from hospital-based to home-based care is a complex and challenging process. In most developing countries, patients transfer directly from hospital to home immediately after stabilizing from acute illness. After discharge, all the patient's care needs are taken over by informal caregivers whose experiences and practices may directly or indirectly impact the quality of patient care as well as the outcomes. This study aimed at exploring the experiences of informal caregivers during the hospital to home transition process of older adults. METHODS: We conducted a qualit
