Metered-dose inhalers with spacers vs nebulizers for pediatric asthma.

OBJECTIVE: To determine whether the administration of beta-agonists by metered-dose inhaler (MDI) with a spacer device is as effective as the administration of beta-agonists by nebulizer for the treatment of acute asthma exacerbations in children. DESIGN: Randomized trial with two arms. SETTING: Urban pediatric emergency department (ED) in Bronx, NY. PATIENTS: Convenience sample of 152 children 2 years and older with a history of at least two episodes of wheezing presenting to the ED with an acute asthma exacerbation. INTERVENTIONS: Patients were randomly assigned to receive standard doses

Spacer devices used with metered-dose inhalers. Breakthrough or gimmick?

In an effort to improve the delivery of a drug to the lungs, to correct problems of hand-lung discoordination, and to reduce local side effects such as oral candidiasis, a number of spacer devices have been developed to attach to metered-dose inhalers. Administration of bronchodilator drugs to patients with faulty techniques of inhalation has been improved with the addition of spacers. In adults and older children with a correct technique of inhaling bronchodilators, the spacer devices do not seem to have any advantage over the simple metered-dose inhalers. Young children (two to five years) c

[A new spacer, Babyhaler, for BDP inhalation therapy in severe infantile asthma].

Recently, it has been recognized that airway inflammation is the most important pathogenesis of bronchial asthma, and inhaled corticosteroids therapy is effective for childhood asthma. However, using metered dose inhalers (MDI) of beclomethasone dipropionate (BDP) is difficult for infants. In this study, we administered BDP inhalation therapy with a new spacer, Babyhaler, for five cases of early childhood with severe infantile asthma that we could not control even by combination of theophylline round the clock (RTC) therapy and disodium cromoglycate (DSCG) + beta 2 stimulant (beta 2) regular u

Clinical use of spacer systems for corticosteroid inhalation therapy: a preliminary analysis.

This is a preliminary report on the first half of a study designed to assess the ability of two different spacer devices to improve the results of aerosol steroid treatment in chronic asthmatic patients given budesonide in low and high dosages. The spacers significantly reduced oropharyngeal candidiasis but did not affect dysphonia - possibly because the incidence of the latter was so low throughout the study. The spacers augmented the airway response to budesonide in these patients without materially increasing systemic absorption of the drug (assessed indirectly in terms of the change in blo

Metering performance of several metered-dose inhalers with different spacers/holding chambers.

Metered-dose inhalers (MDI) are routinely used to administer inhaled antiasthma drugs. Actuation-inhalation coordination problems are overcome and systemic side effects are reduced by using spacers/holding chambers (SP/HCHs). Many of these devices do not allow the use of the manufacturer's actuator. The objectives of this study were (a) to investigate the effect of the interaction of eight MDI products with four different SP/HCHs on their metering performance (MP); and (b) to test the hypothesis whether the MP obtained with a SP/HCH and a given drug (MDI) can be extrapolated to other MDIs, eve

A large volume spacer significantly reduces the effect of inhaled steroids on bone formation.

Inhaled steroids are increasingly advocated as first line treatment for mild asthma. Some studies suggest that inhaled steroids suppress bone formation as reflected by a fall in plasma osteocalcin. Spacers have been shown to increase the proportion of inhaled aerosol that is deposited in the lungs and to reduce the amount swallowed. We measured plasma osteocalcin levels to determine the effect on bone formation of inhaled beclomethasone dipropionate (BDP) with and without a 750 ml spacer in a double-blind, randomised, placebo-controlled, cross-over study. Twenty-six healthy male volunteers too

Inhaled salbutamol delivery in small children with disposable and reusable spacers: an in vitro study.

BACKGROUND: Research on spacers for paediatric breathing patterns is limited, especially for disposable types, which may be a practical alternative to reusable valved holding chambers (VHC) in certain clinical settings. METHODS: In vitro, the fine particle dose (FPD) of salbutamol from a pressurised metered-dose inhaler (pMDI) was tested using two paperboard spacers-DispozABLE (Diz) and LiteAire (LA)-and three reusable VHCs: AeroChamber Plus Flow-Vu (AC), EasyChamber (EC) and OptiChamber Diamond (OC). The pMDI+VHC setup was connected to a child throat model without a facemask. Salbutamol avai

Metered-dose inhaler therapy with spacers: Are newborns capable of using this system correctly?

INTRODUCTION: Aerosol therapy using a metered-dose inhaler (MDI) coupled to a spacer chamber is the most widely used long-term treatment modality for chronic lung disease of prematurity. However, its use in neonates is based on data obtained from other age groups. Proper use of maintenance treatment is essential for the long-term stability of these patients. OBJECTIVE: To ascertain whether newborns are capable of generating negative pressure during the use of a spacer with face mask for aerosol therapy. PATIENTS AND METHODS: Total of 117 low-risk newborns (age 12-48 hours), with no congenita

Metered dose inhaler with spacer versus dry powder inhaler for delivery of salbutamol in acute exacerbations of asthma: a randomized controlled trial.

BACKGROUND: Delivery of various drugs by aerosol inhalation is the mainstay of treatment of asthma. Many delivery systems have been developed for children, each having its own advantages and disadvantages. Studies comparing the clinical efficacy of metered dose inhalers (MDI) and dry powder inhalers (DPI) in the treatment of acute exacerbations of asthma in children are limited. We conducted a study to compare the response to salbutamol inhalation delivered by metered dose inhaler with a spacer versus rotahaler (DPI) in children presenting with mild or moderate acute exacerbations of asthma.

Emergency treatment of obstructive bronchitis: change from nebulizers to metered dose inhalers with spacers.

AIM: To evaluate how the change from drug nebulizers to metered dose inhalers (MDI) with spacers succeeded and influenced the hospitalization rate and the length of the emergency room (ER) and hospital stay in wheezing preschool-aged children. METHODS: After educating and training the nurses and physicians, the use of MDIs with spacers was started on 1 November 2006 in the ER and acute wards of our hospital. All of the 1-to-5-year-old patients admitted for bronchial obstruction from 1 November 2004 to 28 February 2005 and from 1 November 2006 to 28 February 2007 were included in this retrospe

Procaterol via nebulizer versus metered-dose inhaler with a spacer for acute asthma exacerbation in children.

BACKGROUND: To date, no studies have assessed the outcomes of different inhalation methods for procaterol, a short-acting β2-agonist frequently used in Japan to treat acute asthma exacerbations in children. The present study compared the efficacy and safety of procaterol using a nebulizer (NEB) and a metered-dose inhaler (MDI) with a spacer. METHODS: The present before-and-after study targeted patients aged 4-16 years with moderate to severe acute asthma exacerbations who visited the pediatric emergency department (PED). The primary outcome was a reduction in the Modified Pulmonary Index Scor

Factors affecting the efficiency of aerosol therapy with pressurised metered-dose inhalers through plastic spacers.

AIM: The main objective of this study was to compare the in vitro delivery of salbutamol from a chlorofluorocarbon(CFC)-propelled pressurised metered-dose inhaler (pMDI) versus a newly developed hydrofluoroalkane(HFA)-propelled pMDI through various spacers. In addition, we aimed to study the effect on bronchodilator response when using an optimal pMDI/spacer combination for aerosol delivery compared to a suboptimal combination. METHODS: Particle size distribution and output from salbutamol pMDIs containing either CFC propellants (Ventolin) or HFA propellants (Airomir) were measured using a mu

Aerosol delivery to non-ventilated infants by metered dose inhaler: should a valved spacer be used?

In a randomized double-blind cross-over study on 20 spontaneously breathing, oxygen-dependent preterm infants who had received positive pressure ventilation for respiratory distress syndrome, we tested the hypothesis that the one-way non-rebreathing valves of aerosol spacer devices might impair rather than enhance the delivery of aerosols to small infants by metered dose inhalers (MDI). Ten infants were given 2 doses (200 micrograms/dose) of MDI albuterol through a neonatal Aerochamber 4 h apart. At random sequence, one dose was delivered with the non-rebreathing valve of the Aerochamber in pl

Delivery of salbutamol by metered dose inhaler and valved spacer to wheezy infants: effect on bronchial responsiveness.

The efficacy of a new valved spacer device, the Babyhaler inhaler (Glaxo) for administering metered dose inhaler treatment via a facemask to infants was assessed. In a double blind, single dose study, salbutamol (800 micrograms) or placebo were given on separate days to 12 sedated, sleeping, wheezy infants during a symptom free interval. Lung function was measured before and after administration and the bronchial response to aerosol challenge with methacholine was then assessed using the squeeze technique. A small increase in heart rate and a drop in arterial oxygen tension followed salbutamol

Peak Inspiratory Flow in COPD Patients Treated with Pressurized Metered-Dose Inhalers and Spacers: Implications for Inhaler Selection. A Multicenter Cross-Sectional Study.

BACKGROUND: Spacer devices are frequently prescribed with pressurized metered-dose inhalers (pMDIs) in patients with COPD for a variety of clinical and practical reasons, including concerns about inspiratory capacity, inhaler technique, and hand-breath coordination. However, inspiratory flow is rarely measured objectively. PURPOSE: To determine whether COPD patients treated with pMDIs and spacers can generate adequate peak inspiratory flow (PIF) for dry powder inhaler (DPI) use, to evaluate the impact of incomplete exhalation, and to examine patient perceptions of spacer use. Usefulness of Nonvalved Spacers for Administration of Inhaled Steroids in Young Children with Recurrent Wheezing and Risk Factors for Asthma.

BACKGROUND: In vitro and scintigraphic studies have suggested that effectiveness of metered-dose inhalers (MDI) with nonvalved spacers (NVS) is similar to that of MDI with valved holding chambers (VHC). Nevertheless, there are no clinical studies that compare these techniques in long-term treatment with inhaled steroids in young children with recurrent wheezing and risk factors for asthma. OBJECTIVE: To compare the efficacy of a long-term treatment with Fluticasone Propionate administered by an MDI through both type of spacers, with and without valves, in young children with recurrent wheezin

[Influence of beclomethasone dipropionate inhalation therapy with a large spacer on emergency room visits by patients with bronchial asthma].

From April 1992 we introduced beclomethasone dipropionate (BDP) inhalation therapy with a large spacer for patients with bronchial asthma who were admitted to the internal medicine department of our hospital because of an exacerbation of asthma. To investigate the influence of this BDP inhalation therapy on emergency room visits by patients with bronchial asthma, we studied the patients who were admitted to the emergency room at our hospital with asthmatic attacks from April 1991 to March 1994.
