[Intra-articular injections].

It is not unusual for a specialist or general practitioner to be presented with a pathology which necessitates the use of an intra-articular injection of corticosteroids, hyaluronic acid or a local anaesthetic. It would seem to be interesting to update and to precise the techniques and methods of intraarticular injections which have appeared in recent international publications, when we know that 30 % of the injections given into the knee and so called "dry" are incorrect and, therefore, inefficient. The indication of an articular injection depends, firstly, on the diagnosis which should be do

Complications Associated with Intra-Articular and Extra-Articular Corticosteroid Injections.

Side effects of corticosteroids can occur in association with both intra-articular and extra-articular injections. Less-severe reactions include skin reactions and flare responses. Although rare, worrisome complications such as infections, tendon ruptures, and osteonecrosis can occur. It is important to note the effect of corticosteroids on blood glucose levels, particularly for patients with diabetes. Corticosteroid injections have visual side effects with a known relation to central serous chorioretinopathy.

Intra-articular corticosteroid injections in juvenile idiopathic arthritis.

Therapeutic intervention with intra-articular steroid injections in juvenile idiopathic arthritis (JIA) has evolved from experience with adults with inflammatory joint disease, with the earliest report being published in 1951. The technique has subsequently been introduced into paediatric rheumatology practice, although much of the evidence supporting its use remains anecdotal or based on open, non-controlled studies. This review examines the body of evidence relating to many aspects of treating children with JIA with intra-articular steroids, and is approached from both a medical and a physio

Fatal complications of intramuscular and intra-articular injections.

Four fatalities related to intramuscular and intra-articular injections are reported. In two of these cases a Staphylococcus aureus sepsis developed, as a consequence of injections into the left hip joint in one and in the lateral upper quadrant of the gluteal region in the other. The intra-articular injection of triamcinolone produced severe pain, but no marked signs of purulent arthritis were seen at autopsy, probably because of the anti-inflammatory effect of the corticosteroid. A cutaneous infection was seen in the gluteal region of the other patient, but no apparent abscess formation. In

Intra-articular platelet-rich plasma injections versus intra-articular corticosteroid injections for symptomatic management of knee osteoarthritis: systematic review and meta-analysis.

BACKGROUND: Intra-articular (IA) corticosteroid (CS) injections are the mainstay of treatment for symptomatic management in knee osteoarthritis (OA), particularly in the UK. IA platelet-rich plasma (PRP) injections are a promising alternative, but no systematic reviews to date have compared them to the current standard of care, IA CS injections. We aim to investigate the effect of IA PRP injections versus IA corticosteroid injections for the symptomatic management of knee OA. METHODS: All published trials comparing IA PRP and CS injections for knee OA were included. MEDLINE, EMBASE, Scopus an

Molecular Advances in Intra-Articular Injections for Knee Osteoarthritis.

Knee osteoarthritis (KOA) is a common musculoskeletal condition characterized by progressive cartilage loss, subchondral bone alterations, and persistent, low-grade synovial inflammation. There is growing evidence indicates that KOA should be viewed not only as a degenerative disorder but also as a multifaceted inflammatory process driven by complex interactions among immune cells, cytokines, and cartilage-related molecular pathways. This review outlines recent molecular advances related to intra-articular injection therapies and their proposed mechanisms of action. Corticosteroid (CS) injecti

Secondary Adrenal Insufficiency due to Intra-articular Glucocorticoid Injections.

BACKGROUND: The most common cause of hypothalamic-pituitary-adrenal axis suppression is systemic glucocorticoids administration. CASE CHARACTERISTICS: A 14-year-old boy with juvenile idiopathic arthritis receiving repeated intra-articular steroids for last 3 years developed fever, fatigue, nausea and abdominal pain. Stimulation with low-dose Synathen revealed low adrenal reserve, suggesting secondary adrenal insufficiency. OUTCOME: Temporary hydrocortisone substitution therapy improved condition. MESSAGE: Intra-articular steroids may cause potentially life-threatening suppression of the hyp

[Complication of an intra-articular injection with corticosteroids].

Osteoarthritis is a growing pathology due to aging population and sedentary lifestyles. Pharmacological and non-pharmacological treatments are available, but their effectiveness is debatable. We describe the case of a 73-year-old woman who benefited from intra-articular corticosteroids injection for refractory knee arthritis. This procedure was complicated by a septic arthritis requiring hospitalization, joint lavage, and intravenous antibiotic treatment. To date, there is no clear consensus regarding the use of intra-articular corticosteroid injection. Thus, the choice of treatment must be ma

Intra-articular injection of the acromioclavicular joint.

Intra-articular punctures and injections are performed routinely on patients with injuries to and chronic diseases of joints, to release an effusion or haemarthrosis, or to inject drugs. The purpose of this study was to investigate the accuracy of placement of the needle during this procedure. A total of 76 cadaver acromioclavicular joints were injected with a solution containing methyl blue and subsequently dissected to distinguish intra- from peri-articular injection. In order to assess the importance of experience in achieving accurate placement, half of the injections were performed by an

Changes in articular cartilage after intra-articular injections of methylprednisolone acetate in horses.

Eight mature horses with no prior signs of joint disease or history of intra-articular therapy were treated with 8 weekly intra-articular injections of methylprednisolone acetate. Treatments were given at a dose of 120 mg/joint into the right radiocarpal and intercarpal joints, with the left joints as untreated controls. Articular cartilage samples were obtained at necropsy 1, 4, and 8 weeks after the last injection. Compared with controls, cartilage from injected joints had a loss of hematoxylin basophilia and decreased intensity of staining in safranin O fast green dye. Chondrocyte necrosis

How safe are intra-articular corticosteroid injections to the hip?

BACKGROUND: Intra-articular corticosteroid injections (ICSI) are an effective symptomatic treatment for osteoarthritis of the hip. However, the safety of ICSI has been questioned and a relatively high risk for septic arthritis, rapidly progressive osteoarthritis (RPIO) and periprosthetic joint infections (PJI) in patients undergoing subsequent total hip arthroplasty (THA) have been suggested. METHODS: This is a retrospective evaluation of 682 hips that underwent ICSI with 40 mg of Triamcinolone for primary osteoarthritis of the hip. All ICSI were performed using sterile techniques, the number

Septic arthritis following intra-articular corticosteroid injections: a retrospective analysis.

INTRODUCTION: Septic arthritis is a rare but devastating complication of intra-articular corticosteroid injection (CSI), associated with significant medical morbidity and poor clinical outcomes. Although previous studies have examined risks associated with CSI, few have tracked patients long-term. This study evaluates the incidence, timing, and patient characteristics related to iatrogenic septic arthritis within 6 months of receiving a large joint CSI, offering new insights to inform clinical practice. MATERIALS AND METHODS: A retrospective, descriptive cohort study was conducted using Slicer

Safety of intra-articular glucocorticoid injections - state of the art.

Intra-articular glucocorticoid injection (IAGCI) is frequently used to treat joint pain and inflammation. While its efficacy has been extensively studied, there are not as many detailed descriptions regarding safety. This review aimed to describe the immediate-, short- and long-term complications of IAGCI and their predictors. Most studies mainly report mild and self-limited adverse events with an incidence similar to placebo. However, the reported incidences vary significantly and are mostly inferred from retrospective data. Septic arthritis is the most feared adverse event due to its associa

Cartilage turnover and intra-articular corticosteroid injections in knee osteoarthritis.

Intra-articular corticosteroid injections (IACI) are commonly used interventions for pain relief in patients with knee osteoarthritis (OA). Biomarkers may be helpful in further elucidating how IACI exert their effect. The aim of this study is to look at the response of biomarkers of cartilage and bone metabolism after IACI in knee OA. Eighty subjects with symptomatic knee OA [45% male, mean age (SD) 64 (11) years] underwent routine knee joint injection with 40 mg triamcinolone acetonide and 4 ml 1% lignocaine. Knee pain (as pain subscale of WOMAC VAS) and biomarkers [C-telopeptides of type-II

Intra-articular corticosteroid injections in haemophilic arthropathy: are they recommended?

INTRODUCTION: Intra-articular (IA) corticosteroid (CS) injections are commonly used in the treatment of osteoarthritis. However, they are rarely utilized in haemophilic arthropathy. In fact, the efficacy of this method in haemophilic arthropathy is frequently discussed and debated in clinical practice. AIM: To investigate the effectiveness of IA CS injections in patients with painful haemophilic arthropathy. METHODS: A review of the literature on the topic was performed. RESULTS: In osteoarthritis, reports with a high level of evidence state that the efficacy of IA injections of CS is contr
