Lower gastrointestinal tract bleeding.

Lower GI bleeding can be slow and chronic or massive and fatal. ED evaluation of these patients begins with history directed at determining the severity and amount of bleeding, and eliciting symptoms of volume depletion. Physical examination determines orthostasis, exclusion of an upper GI source for bleeding, and rectal examination. Laboratory evaluation is directed at determining baseline status of hemoglobin/hematocrit and platelet adequacy, as well as assessing concomitant medical problems. Although many of these patients are elderly, resuscitation is vigorous and should not be deterred by

Physiology of lower gastrointestinal tract.

BACKGROUND: The lower gastrointestinal (GI) tract, formed from the midgut and hindgut, encompasses the colon, rectum and anal canal. AIM: The aim of this review is to provide an overview of the anatomy and physiology of the lower GI tract. METHODS: Literature review on anatomy and physiology of the lower GI tract, including normal motility and phases of defecation. It derives its blood supply from the superior and inferior mesenteric arteries while it is innervated by the extrinsic autonomic (the thoracolumbar and sacral nerves) and the intrinsic enteric nervous system. The colon has four la

Lower gastrointestinal tract. 1. Constipation.

An accurate diagnosis is of primary importance in lower gastrointestinal problems in the elderly. Reversible causes should be treated specifically whenever possible, but symptomatic treatment can be a useful adjunct or a mainstay if no cause can be found. For constipation symptomatic treatment should proceed stepwise, commencing with increase in dietary fibre content. Only if this approach fails should laxatives (osmotic) be used. Stimulant laxatives and paraffin are best used intermittently, if at all, because of their side effects.

Anastomoses of the lower gastrointestinal tract.

Patients with gastrointestinal anastomoses are treated by physicians of multiple specialties, including gastroenterologists, radiologists and surgeons. This Review provides an overview of the surgical principles and techniques involved in the creation of lower intestinal anastomoses, including some of the mechanisms of healing. Anatomical configurations of small and large bowel anastomoses are illustrated. Stapled, hand-sewn, and sutureless anstomotic techniques are also discussed. Laparoscopy has revolutionized our approach to surgery of the gastrointestinal tract and we describe some of the

Pneumoperitoneum of the lower gastrointestinal tract.

Pneumoperitoneum due to lower gastrointestinal perforation is an acute clinical situation requiring rapid dia-gnosis and treatment. This condition is often the result of dis-eases such as diverticulitis, malignant tumors, Crohn's dis-ease, or complications of endoscopic examination methods and surgical procedures. If this condition is not treated promptly, it can lead to life-threatening complications. Dia-gnosis relies mainly on imaging techniques such as X-ray, ultrasound, and CT, which allow the detection of free gas in the peritoneal cavity and the localization of the perforation. Treatmen

[Endoscopic ultrasound in the lower gastrointestinal tract].

Endoscopic ultrasound is a minimally invasive modality that combines endoscopy with ultrasound, providing a possibility to visualize the wall of the gastrointestinal tract and adjacent tissues and organs. Since the development of the modality in the 1980s, advancements in endoscopic ultrasound technology have led to increasingly broadening indications: besides diagnostic indications, therapeutic indications have also expanded greatly. According to recent guidelines regarding rectal cancer staging, rectal ultrasonography is mainly considered to be a secondary imaging modality compared to magnet

Radionuclide evaluation of the lower gastrointestinal tract.

This review outlines the technical aspects and diagnostic performance parameters of nuclear medicine procedures used on patients with disorders of the lower gastrointestinal tract, with the exclusion of techniques using tumor-seeking radiopharmaceuticals. Chronic disorders of the lower gastrointestinal tract often reduce the quality of life because of discomfort from constipation or diarrhea. Five classes of radionuclide procedures are used to characterize these disorders: transit scintigraphy, searches for ectopic gastric mucosa in Meckel's diverticulum, scintigraphy of active inflammatory bo

Serotonin and Melatonin in Human Lower Gastrointestinal Tract.

BACKGROUND AND AIMS: Melatonin is a ubiquitous hormone produced not only by the pineal gland but also by other organs and tissues. It is involved in the regulation of several gastrointestinal functions. The main cells responsible for the production and release of extrapineal melatonin are the enterochromaffin (EC) cells that produce serotonin. They are involved in the pathogenesis of neuromotor disorders that characterize functional gastrointestinal disorders and in the pathophysiology of inflammatory intestinal diseases. Our aim was the immunohistochemical highlighting on biopsy samples of no

Massive arterial haemorrhage from the lower gastrointestinal tract.

We have studied the clinical, radiological and pathological features of three patients with recurrent massive lower gastrointestinal arterial haemorrhage. Case 1 was an example of Dieulafoy's vascular malformation within the proximal ascending colon in a 46-year-old woman. Cases 2 and 3 were men aged 81 and 83 years with arterial erosions contained within small mucosal diverticula in the hepatic flexure and descending colon, respectively. All three patients presented with recurrent acute episodes of massive lower gastrointestinal haemorrhage. Selective mesenteric angiography was performed in c

Contemporary radiological examination of the lower gastrointestinal tract.

The morphological spectrum of colonic disease is wide. Various treatment modalities may influence the macroscopic aspect of colonic lesions and render a pathological differential diagnosis occasionally difficult or impossible. Before starting therapy in patients suspected of having colonic disease, a physician should undertake a thorough radiological and endoscopic evaluation of the extent and severity of disease activity in the large bowel (Ruderman and Farmer, 1987). DCBE and colonoscopy are complementary imaging modalities, each test has its own intrinsic advantages and merits (Lichtenstein

[Leakages after surgery of the lower gastrointestinal tract].

The incidence of anastomotic leakage in colorectal surgery is 1% to 12%. Every deviation from the normal postoperative course must raise suspicion of a leak. Diagnosis is made radiologically by rectal enema or CT. Limited leakages without clinical signs can be treated conservatively by wait-and-see. Larger anastomotic failure with intra-abdominal abscesses or peritonitis requires reanastomosis in combination with a diverting loop ileostomy or colostomy. A Hartmann procedure with open abdominal management may be indicated in severely ill patients with feculent peritonitis. In the pelvis, even l

Langerhans cell histiocytosis in the adult lower gastrointestinal tract.

A female in her early 80s presented with chronic anal fissures which were unresponsive to conservative therapies. Examination was concerning for possible perianal Crohn's disease, but subsequent colonoscopy and biopsies revealed Langerhans cell histiocytosis (LCH) involving the colon and anus, diagnosed per 2022 WHO Classification of Haematolymphoid Tumours criteria and confirmed by BRAF V600E mutation. Systemic staging identified two unrelated malignancies of the breast and kidney. The patient was treated with cladribine as systemic chemotherapy, resulting in complete remission of LCH symptom

Predictors of urinary tract infection after lower gastrointestinal surgery.

BACKGROUND: Urinary tract infections (UTIs) cause postoperative morbidity in patients undergoing lower gastrointestinal (GI) surgery that can prolong postoperative hospital stays. In patients with a fever of unknown origin (FUO), clinicians ignore what to do while waiting for the results of the urine culture test. This study aimed to develop a nomogram predicting UTI in the case of postoperative FUO. METHODS: This observational, retrospective study included all consecutive patients from 1 November 2020 to 1 November 2021 undergoing lower-GI surgery at the Chirurgia Generale 3, University Hosp

Analysis of urgent colonoscopy for lower gastrointestinal tract bleeding.

BACKGROUND: Three hundred and forty-five patients who underwent urgent colonoscopy for acute hematochezia during the past 20 years (from 1976 to 1995) were retrospectively analyzed. METHODS: Urgent colonoscopy was defined as endoscopy performed within 24 h after a bleeding episode. Preparation was initially minimal with a water or glycerine enema. Recently, however, polyethylene glycol is used. RESULTS: The overall diagnostic accuracy for bleeding site detection was 89.1% (307 cases). Successful insertion was made to the ileocecal region in 193 cases (55.9%). The disease most frequently disc

Lower gastrointestinal tract tuberculosis: an important but neglected disease.

PURPOSE: Difficulties in early and accurate diagnosis of intestinal tuberculosis lead to frequent misdiagnosis even in endemic areas. This study aimed to investigate clinical and laboratory characteristics of patients with lower gastrointestinal tract tuberculosis (LGITB). MATERIALS AND METHODS: Patients who met the criteria for LGITB in a medical center from 1997 to 2006 were identified and their medical records reviewed. RESULTS: A number of 4,567 patients with culture or histology-proven tuberculosis were identified, and 30 (0.66%) were diagnosed with LGITB. Principal co-morbidities were

Surgical Options in the Treatment of Lower Gastrointestinal Tract Cancers.

Colorectal cancer is the fourth most common cancer in the USA with the second highest mortality. Early detection with screening endoscopy, in addition to improvement in treatment modalities lead to higher overall survival rates. Treatment of localized colon cancer comprises of surgical resection with en bloc lymphadenectomy. Adjuvant therapy is determined by final pathologic stage. However, treatment of rectal cancer is more complex and is determined by the clinical pathologic stage. Neoadjuvant chemoradiation therapy is an important component to treatment to decrease the risk of local recurre

Clinical significance of lymphoid hyperplasia of the lower gastrointestinal tract.

Lymphoid hyperplasia of the intestine has been associated with multiple diseases and symptoms. This study was undertaken to analyze the number and topographical distribution of the lymphoid follicles. A total of 302 adult consecutive patients were enrolled when they underwent elective colonoscopy. Standardized pictures from terminal ileum and colon were taken using video colonoscopes. In each picture, the number, size, and mucosal elevation of lymphoid follicles were analyzed in relation to histological and immunological findings and medical history. Lymphoid hyperplasia was found to be most e

[Infectious diseases of the lower gastrointestinal tract and their differential diagnoses].

Infectious colitis comprises histologically a spectrum from normal mucosa to severe inflammation with ulceration. In many cases, a diagnosis is made microbiologically or virologically. Nevertheless, histology still plays an important role, given its potential time and cost-advantages as well as its possibility to provide a statement on differential diagnoses, leading to different therapeutic treatment strategies (anti-infective drugs, surgery, immunosuppressive agents). The pathologists' role is especially important in three scenarios: 1) The infectious agent is directly visible by light micro
