Nipple centralization for the correction of breast deformity from segmental mastectomy.

A breast deformed by lateral tissue deficiency and severe lateral displacement of the nipple, caused by the treatment of an early breast cancer with segmental mastectomy and radiotherapy, was corrected by a modification of techniques used commonly for mastopexy. The nipple and areola were moved medially on a central pedicle to create the illusion that the missing lateral tissues had been restored. Simultaneously, the opposite breast was reduced to improve symmetry. The result was a normalization of breast appearance without the need for distant tissue. It is possible that modifications of this

Use of the Ultrasound-Guided Erector Spinae Plane Block in Segmental Mastectomy.

Erector spinae plane (ESP) block is a recently described regional anaesthesia/analgesia technique. Although there is stil no consensus on its mechanism, the published case series seem to promise a new block for both chronic and acute pain. In this case report, the use of ESP block in 47-year-old female patient scheduled for bilateral segmental mastectomy surgery is presented. ESP was formerly reported for rib fracture, bariatric surgery, open abdominal surgery and breast implant surgery. To the best of our knowledge, no article has been published for ESP block in breast cancer surgery includin

Repeat Breast Cancer Surgeries After Segmental Mastectomy Meaningfully Increase Long Term Healthcare Costs.

BACKGROUND AND OBJECTIVES: Among patient with early-stage breast cancer, breast conservation surgery (BCS) is the most performed surgery, utilized in approximately two-thirds of patients. While 5-year overall-survival rates are strong-typically above 90%-additional re-excisions can occur in up to 25% of patients who undergo index BCS and some patients will require subsequent surgery for new breast cancer or recurrences. As such, these additional procedures can complicate patients' pathway to survivorship, negatively impact quality of life, and lead to excess healthcare costs. METHODS: This re

A High Index of Awareness About the Inherent Complications of Thoracic Segmental Spinal Anesthesia: A Case of Mastectomy With Bronchiectasis Under Thoracic Segmental Spinal Anesthesia.

General anesthesia is the gold standard for breast cancer surgeries. Considering the nature of the surgery and its associated pain, various regional techniques are used as an adjunct to general anesthesia. Regional anesthesia as a sole anesthetic technique for breast cancer surgery is an upcoming technique - especially in high-risk patients considering the risk-benefit ratio, various regional blocks like pectoralis major block, pectoralis minor block, and erector spinae block - in which thoracic segmental spinal anesthesia is the recent one. Here we present a 68-year-old patient with bronchiec

Anaesthetic Management for Mastectomy in a Male With Unilateral Gynecomastia: The Utilization of Thoracic Segmental Spinal Anaesthesia and Erector Spinae Plane Block.

Males are frequently affected by gynecomastia, a benign proliferative glandular tissue condition of the breast. Gynecomastia is usually treated with surgery to remove breast tissue. Using erector spinae plane block and thoracic segmental spinal anaesthesia in place of typical general anaesthesia during breast procedures has become more common in recent years. This case report presents the management of a 24-year-old male with long-standing left breast gynecomastia. Using a combination of erector spinae plane block and thoracic segmental spinal anaesthesia, the patient had the breast tissue exc

Single dose systemic acetaminophen to improve patient reported quality of recovery after ambulatory segmental mastectomy: A prospective, randomized, double-blinded, placebo controlled, clinical trial.

Few systemic drug interventions are efficacious to improve patient reported quality of recovery after ambulatory surgery. We aimed to evaluate whether a single dose systemic acetaminophen improve quality of recovery in female patients undergoing ambulatory breast surgery. We hypothesized that patients receiving a single dose systemic acetaminophen at the end of the surgical procedure would have a better global quality of postsurgical recovery compared to the ones receiving saline. The study was a prospective randomized double blinded, placebo controlled, clinical trial. Healthy female subjects

Comparison of mammography and ultrasound findings in the follow-up of patients with breast cancer treated with segmental mastectomy followed by intraoperative electron radiotherapy versus external whole breast radiotherapy.

PURPOSE: This study aims to describe imaging findings in patients treated with intraoperative electron radiotherapy and compare them with those detected in patients treated with external whole breast radiotherapy (WBRT). METHODS: The study population consisted of 25 patients who received intraoperative radiotherapy [IORT (21 Gy)] as single-dose radiotherapy and a control group of 25 patients who received WBRT at the same institution. Mammography and ultrasound (US) findings were divided into three groups: minor, intermediate, and advanced. On mammography, mass lesions were considered advanced

Clinical application of vacuum sealing drainage in patients with idiopathic granulomatous mastitis.

BACKGROUND: Idiopathic granulomatous mastitis (IGM) is a chronic inflammatory breast disease with an unclear etiology. Patients with IGM remain at risk of recurrence even after clinical cure. We assessed the long-term clinical outcomes of segmental mastectomy paired with vacuum sealing drainage (VSD)-based integrated wound management for IGM patients eligible for surgical intervention. METHODS: Patients were divided into two groups according to the clinical treatment protocol: 87 patients underwent segmental mastectomy combined with VSD (VSD comprehensive group), and 63 underwent segmental ma

Surgical Burden of Breast Cancer Treatment: Implications of Mastectomy, Breast Conservation, and Reconstruction Choices.

Breast cancer surgical management encompasses a spectrum of options that extend beyond oncologic control and carry substantially different cumulative surgical burdens. Although breast-conserving therapy (BCT) and mastectomy offer equivalent survival outcomes in many clinical scenarios, the downstream implications of these choices, including the number of operations, complication profiles, recovery timelines, and need for revision, are often underrecognized during initial treatment planning. This review aims to provide non-plastic surgeons with a practical framework for understanding the surgic

Defining a place for nipple sparing mastectomy in modern breast care: an evidence based review.

Breast cancer is a diverse disease that requires a fully integrated multidisciplinary approach. Breast surgery has undergone a revolutionary change leading us from the conventional radical mastectomy of the Halstedian era to the current motion of nipple sparing mastectomy (NSM). Despite the lack of randomized controlled trials, the technique of NSM continues to gain popularity as a prophylactic procedure in high risk patients. The current indications for NSM, if any, in the treatment of early invasive breast cancer remains uncertain and requires rigorous scientific scrutiny. This article aims

Predictors of surgery choices in women with early-stage breast cancer in China: a retrospective study.

BACKGROUND: The breast-conserving surgery and reconstruction rate in China is relatively low when compared with those in Western countries. Moreover, predictors of surgical choices for women with breast cancer in China have not yet been explored. This study aims to explore differences in the surgical choices of women with different demographic and clinical characteristics and the predictors that influence surgical choices of women with early-stage breast cancer. METHODS: This retrospective study included women with early-stage (0-II) breast cancer who underwent surgeries at one of two Xiamen

Secondary angiosarcoma following catheter-based brachytherapy.

Secondary angiosarcoma of the breast following catheter-based brachytherapy after lumpectomy is rare. We describe a case of a patient with breast cancer treated with partial mastectomy and sentinel node biopsy followed by accelerated partial breast irradiation (APBI), who developed skin changes 6 years after completion of therapy. Punch biopsy confirmed the diagnosis of secondary angiosarcoma. This case is even more unique in that the location of the skin changes was remote to the lumpectomy site. There is a critical need to recognize secondary angiosarcoma presentation after APBI and determin

Postoperative Complications from Breast and Axillary Surgery.

Although surgery of the breast and axilla is generally well-tolerated by patients, the breast surgeon recognizes that complications can occur even when operating with experience on the lowest risk patients. The operative repertoire ranges from breast conserving surgery, mastectomy (including skin-sparing and nipple-sparing types), to modified radical mastectomy, with each procedure carrying a different expected surgical morbidity. Patients and families who are fully informed of potential complications before their operation describe greater trust in their surgeon and are better able to co-mana

[Breast reconstruction by autologous fat transfer].

INTRODUCTION: Autologous fat tissue transfer has recently become extremely popular in the field of plastic and reconstructive surgery. Fat represents a filling material with ideal properties - it naturally incorporates into tissues and it is autologous, and therefore 100% biocompatible. An increasing number of publications describe the regenerative properties of autologous fat tissue, especially when used in soft tissue with post-radiation damage. The aim of this article is to describe our experience with fat grafting in breasts. MATERIALS AND METHODS: An autologous fat grafting operation con

Breast surgery: a narrative review.

Breast cancer is the commonest human cancer globally and one in seven Australian women will develop it in their lifetime. Surgery is the mainstay of management both for women who are at high risk of breast cancer and for those who have been diagnosed. Increased understanding of how to predict who is most at risk of breast cancer is leading to the possibility of risk-based screening, allowing better and more targeted early detection for women at high risk, and contrast imaging techniques are proving more accurate in diagnosing and staging cancer. The evolution of surgical practice includes the

Surgical excision of BioZorb device eroding through the nipple-areolar complex one year postoperatively: A case report.

The recent use of placing a BioZorb device during breast conservation surgery has been shown to improve targeting of adjuvant radiation therapy by significantly reducing target volume to the breast. However, the risks of surgical and/or infectious complications related to a BioZorb placement are largely unknown. In this case report, we describe a patient who underwent BioZorb placement after breast lumpectomy for ductal carcinoma in situ (DCIS), who presented with repeated infections and eventual erosion of the BioZorb through her nipple-areolar complex (NAC), requiring surgical debridement an

Advances in Breast Cancer Radiotherapy: Implications for Current and Future Practice.

Radiation therapy (RT) is an essential component in the management of breast cancer. Following breast-conserving surgery (BCS), adjuvant RT is the standard of care for most patients. Traditionally, RT was delivered with standard whole breast irradiation (WBI) over 5-7 weeks following BCS. However, WBI regimens have evolved; hypofractionated WBI now represents the standard approach, reducing the duration of treatment to 3-4 weeks. Over the past year, five-fraction WBI regimens have also emerged as standard of care for some patients based on data from the FAST and FAST-Forward trials. An alterna
