[Metatarsal osteotomies for hallux valgus].

In the surgical treatment of hallux valgus, proximal, diaphyseal, and distal osteotomies of the first metatarsal bone are commonly used. In an overview article different procedures are described as well as our own stage-adapted concept. Deformities with congruent articulation of the metatarsophalangeal joint are successfully treated with a distal chevron osteotomy. In an incongruent joint a distal soft tissue procedure is required for reduction of the joint. The metatarsus varus displacement is corrected with a proximal osteotomy of the first metatarsal bone. The indications and details of the

Proximal metatarsal osteotomy in hallux valgus.

In order to obtain an optimal correction of hallux valgus and to prevent its recurrence, the authors have applied a surgical technique which combines a proximal valgus osteotomy of the first metatarsal bone with an excision of the pseudoexostosis and a distal soft tissue plasty at the first metatarsophalangeal joint. The procedure is based on an etiological theory regarding metatarsus primus varus as the primary cause of the deformity, which is in accordance with the opinion of many other authors. The osteotomy corrects the malposition of the first metatarsal bone thereby reducing the deformit

Distal metatarsal osteotomy for adolescent hallux valgus.

This is a review of 55 metatarsal osteotomies for hallux valgus and metatarsus primus varus in patients under 15 years of age. The results were analysed after the patients were reviewed clinically and radiologically. Complications of the osteotomies and the factors that caused them are discussed. Poor results, irrespective of the type of osteotomy, were due to excessive shortening of the first metatarsal and/or dorsal tilting of the metatarsal head. In both instances there was persistent metatarsalgia.

First Metatarsal Rotation in Hallux Valgus Deformity.

Rotation of the first metatarsal (M1) as a potential etiological factor of hallux valgus (HV) deformity was described relatively early in the description of HV pathoanatomy. However, because biplanar radiographs have been the standard method for imaging HV, clinicians primarily developed measurement methods and corrective operations confined to 2 dimensions, medial-lateral and inferior-superior. Recently, as our understanding of HV pathoanatomy has further developed, aided in part by advanced imaging technology, M1 rotation about its axis ("axial rotation") and its implications for HV deformit

Hallux valgus due to cuneiform-metatarsal instability.

We present a biomechanical rationale for the treatment of severe hallux valgus deformity by realignment and arthrodesis of the first cuneiform metatarsal (CM) joint. We think that this severe hallux valgus deformity represents instability at the CM-1 articulation, since normal motion at that joint is very small. A forefoot compression test was used to assess the foot preoperatively. This was found not only to realign the first metatarsal, but also to align the metatarsophalangeal joint. A surgical procedure has evolved to include a transverse screw between the first and second metatarsal bases

Influence of metatarsal head displacement on metatarsal pressure distribution after hallux valgus surgery.

We reviewed 63 feet after plain subcapital osteotomy for simultaneous correction of hallux valgus and metatarsus primus varus. Results were evaluated clinically and by radiographs and force plate measurements. Eighty-three percent of the patients were satisfied with their surgery. The hallux valgus angle was improved by 9.3 degrees and the intermetatarsal angle by 6 degrees. The osteotomy resulted in a displacement of the metatarsal head in the lateral direction (10 mm) and in the plantar direction (11.3 mm). Normalization of the load distribution on the metatarsal head was achieved in the gro

Second Metatarsal Transfer Lesions Due to First Metatarsal Shortening After Distal Chevron Metatarsal Osteotomy for Hallux Valgus.

BACKGROUND: The first metatarsal bone can shorten after a distal chevron metatarsal osteotomy (DCMO). This shortening can result in a postoperative second metatarsal transfer lesion. The aim of the present study was to investigate the occurrence of second metatarsal transfer lesions after DCMO. METHODS: This study involved 185 feet (138 patients), with hallux valgus (HV) deformity, treated with DCMO with Akin osteotomy. The mean patient age was 51.7 years (range, 21 to 74). Patients were followed for an average of 28 months, between June 2004 and June 2010. We measured the length of first met

Metatarsal Pronation in Hallux Valgus Deformity: A Review.

Hallux valgus deformity is a multiplanar deformity, where the rotational component has been recognized over the past 5 to 10 years and given considerable importance. Years ago, a rounded shape of the lateral edge of the first metatarsal head was identified as an important factor to detect after surgery because a less rounded metatarsal head was associated to less recurrence. More recently, pronation of the metatarsal bone was identified as the cause for the rounded appearance of the metatarsal head, and therefore, supination stress was found to be useful to achieve a better correction of the d

[Valgus osteotomy of the first metatarsal by a bone graft.].

The authors describe their experience of valgus osteotomy of the first metatarsal in hallux valgus - metatarsus primus varus (HV-MTPV). It is based on the Stamm technique consisting in the resection of one third of the base of the proximal phalanx of the great toe. From the resected part they make a wedge-shaped bone graft. Subsequently, they extend the section and perform a two-third transversal osteotomy of the first metatarsal (MT) 1 cm distal to the cuneometatarsal joint. The rest of the bone is broken into valgus position by pressure. Afterwards, the bone graft is inserted in the open ost

The distal metatarsal osteotomy for the treatment of hallux valgus.

This article discusses various forms of distal metatarsal osteotomy for the treatment of hallux valgus. The techniques for the various osteotomies have evolved over the years to allow the surgeon to match a procedure and its modifications to the individual patient's deformity, thus optimizing outcomes. Fixation techniques continue to evolve, and meticulous surgical technique to prevent complications remains a must. Regardless of the osteotomy used, the authors believe that adherence to the techniques laid out in current literature will provide gratifying results for the surgeon and the patient

Fixation of metatarsal osteotomies in the treatment of hallux valgus.

Osteotomies as a treatment for hallux valgus require careful preoperative planning and meticulous attention to surgical technique. The procedure selected should be tailored to fit the deformity. For a patient with mild hallux valgus, the chevron osteotomy is the most intrinsically stable of the distal osteotomies and has the least potential for complications. Interest in the Ludloff osteotomy has had a resurgence because of its improved stability compared with more traditional osteotomies for correction of metatarsus primus varus. This osteotomy, however, is less forgiving and more dependent o

Imaging Findings and First Metatarsal Rotation in Hallux Valgus.

BACKGROUND: Failure to identify and correct malrotation of the first metatarsal may lead to recurrent hallux valgus deformity. We aimed to identify the proportion of hallux valgus patients with increased first metatarsal pronation using weightbearing computed tomography (WBCT) and to identify the relationship with conventional radiographic measurements. METHODS: WBCT scans were analyzed for 102 feet with a hallux valgus angle (HVA) and intermetatarsal angle (IMA) greater than or equal to 16 and 9 degrees, respectively. Metatarsal pronation angle (MPA), alpha angle, sesamoid rotation angle (SR

Distal Metatarsal Osteotomy for Moderate to Severe Hallux Valgus.

UNLABELLED: Hallux valgus is the most common disorder of the hallux and often results in pain, functional disability, and impaired gait patterns. The goals of surgical management are to correct the deformity while improving patients' pain and function. Traditional treatment of moderate to severe hallux valgus deformities consist of proximal osteotomy and/or arthrodesis given their powerful corrective ability. Despite their corrective power, proximal osteotomies are more technically demanding, have a higher rate of complications, and require a more restricted post-operative recovery. We present

Percutaneous distal metatarsal osteotomy for correction of hallux valgus.

BACKGROUND: Distal osteotomy of the first metatarsal is indicated for the surgical treatment of mild-to-moderate hallux valgus deformity. The aim of this study was to evaluate the results of a subcapital distal osteotomy of the first metatarsal with use of a percutaneous technique. METHODS: From 1996 to 2001, 118 consecutive percutaneous distal osteotomies of the first metatarsal were performed for the treatment of painful mild-to-moderate hallux valgus in eighty-two patients. The patients were assessed with a clinical and radiographic protocol at a mean of 35.9 months postoperatively. The Am

First metatarsal base osteotomies for hallux abducto valgus deformities.

Proximal first metatarsal osteotomies have historically been associated with and limited to treatment of severe hallux valgus deformities. These procedures are powerful in deformity correction and overall realignment of first ray function because of their ability to correct closer to the deformity's origin. When considering all factors in bunion correction, they are good options for correction of a wide range of hallux abducto valgus deformities. This article discusses the use of proximal metatarsal osteotomies for correction of hallux valgus deformity, techniques to facilitate optimal outcome
