As in most areas of clinical medicine, the history often is the key to determining the diagnosis in patients presenting with neurotologic symptoms. The most common cause of vertigo--benign positional vertigo--can be diagnosed at the bedside based on the characteristic history and the finding of fatigable positional nystagmus on the Dix-Hallpike positional test. Furthermore, it can be cured with a simple positioning maneuver. Quantitative auditory and vestibular function tests are important for documenting the site and severity of deficit in patients with chronic neurotologic symptoms. Only a f

Pediatric neurotology is a subspecialty encompassing the medical and surgical treatment of conditions involving the middle ear, inner ear and lateral skull base. Multiple otologic and neurotologic diseases exist. In the pediatric patient the most common neurotologic conditions are vertigo, complications of otitis media, cholesteatoma, hearing loss and facial paralysis. Vertigo is differentiated into peripheral and central vestibular abnormalities. Peripheral vestibular causes include Meniere's disease, benign paroxysmal positional vertigo, vestibular neuronitis, otitis media and labyrinthitis.

Vestibular diagnostics and therapy is the mirror of technological, scientific and socio-economics trends as are other fields of clinical medicine. These trends have led to a substantial diversification of the field of neurotology. The improvements in diagnostics have been characterized by the introduction of new receptor testing tools (e. g., VEMPs), progress in imaging (e. g., the endolymphatic hydrops) and in the description of central-vestibular neuroplasticity. The etiopathology of vestibular disorders has been updated by geneticists (e. g., the description of the COCH gene mutations), the

Neurotologic symptoms are common with migraine, yet relatively little is known about the pathophysiology of such symptoms. Motion sensitivity with bouts of motion sickness occurs in about two thirds of patients with migraine. Episodes of vertigo occur in about one fourth of patients and, in some, vertigo is the only symptom (so-called "migraine equivalent"). Phonophobia is the most common auditory symptom, but fluctuating hearing loss and acute permanent hearing loss occur in a small percentage. Migraine can mimic Meniere's disease and so-called "vestibular Meniere's disease" is usually associ

For many diseases the etiology of disordered function is unknown and therapy is either symptomatic or empirical. The empirical approach to therapy is acceptable to many physicians, particularly if a treatment can be based on some attractive, although unproven, concept of pathogenesis. A hypothetical explanation of disordered function can become widely popular in spite of a serious lack of scientific support. When evidence emerges that refutes the logic of a concept of pathogenesis, then that concept becomes a myth. Human temporal bone studies have identified several myths in neurotology that d

In their professional capacity, physicians interact with the legal profession in a variety of ways. The interaction occurs most often in the situation where an individual suffering from a bodily injury seeks the recovery of monetary benefits through the legal system. This article reviews the manner in which physicians are contacted by attorneys in connection with workers' compensation cases and civil tort cases. It will also set forth various definitions of disability and contrast those definitions with the concept of impairment as that term is used in the medical profession. Suggestions for p

INTRODUCTION: Neurotology is a rapidly expanding field of knowledge. The study of the vestibular system has advanced so much that even basic definitions, such as the meaning of vestibular symptoms, have only recently been standardized.
OBJECTIVE: To present a review of the main subjects of neurotology, including concepts, diagnosis and treatment of Neurotology, defining current scientific evidence to facilitate decision-making and to point out the most evidence-lacking areas to stimulate further new research.
METHODS: This text is the result of the I Brazilian Forum of Neurotology, which bro

INTRODUCTION: Recently, mechanical vibration on the neck has begun to be used to improve diagnostic sensitivity in patients who have vestibular disorders.
METHOD: We review results of studies of this stimulus in normal subjects and patients with vestibular lesions: the ocular responses, changes in the subjective straight ahead, and postural changes generated by the neck vibration.
CONCLUSION: Although the underlying mechanism for the responses to neck vibration remains uncertain, it is an excellent addition to the neurotologic exam since it allows us demonstrate vestibular deficit with great

Neurotologic and skull base surgery involves working around important neurovascular and neurotologic structures and can incur unwarranted complications. Knowledge of surgical anatomy, good preoperative planning, intraoperative monitoring, and excellent microsurgical technique contribute to minimizing and avoiding complications. In the event of a complication, however, the neurotologic surgeon should be prepared to manage it. In this article, the authors focus on the management of complications encountered in neurotologic skull base surgery, including hemorrhage, stroke, cerebrospinal fluid lea

During the last few years many advances occurred in the field of Otolaryngology. In this article we will present significant advances in the fields of otology and neurotology. The adaptation of laser energy to surgery is a significant event in the evolution of ear microsurgery. Lasers in otology reduce trauma and increase accuracy. Implantable hearing devices are used to rehabilitate patients with mild to moderately severe hearing loss. Cochlear implants have secured a permanent role in the management of selected profoundly deaf patients. Intraoperative cranial nerve monitoring is a major adva

Since the early 1980s rigid endoscopes have been used by otorhinolaryngologists in the United States primarily for sinus surgery. Recently rigid endoscopes have been used as an adjunct to standard otologic and neurotologic procedures. Diagnostic inspection of the middle ear can be performed through a myringotomy incision to rule out perilymphatic fistula, for identification of cholesteatoma, or for evaluation of the status of the ossicular chain. During chronic ear surgery endoscopes can be used to locate hidden cholesteatoma in difficult to visualize areas such as the eustachian tube, attic,

Otology & neurotology : official publication of the American Otological Society, American Neurotology Society [and] European Academy of Otology and Neurotology2018eng
OBJECTIVES: 1) Describe publishing trends for otologic/neurotologic disorders over a 35-year span. 2) Compare trends in publishing with disease prevalence. 3) Evaluate changes in topic and journal specific ranking scores over time.
METHODS: PubMed searches were performed on 35 otologic/neurotologic disorders using medical subject headings (MeSH) terms from 1980 to 2015. Searches were limited in scope to the English language. A Mann-Kendall trend analysis evaluated changes in publication frequency as a discrete variable while correcting for total number of articles published per year. Scopus w

The severe acute respiratory syndrome corona virus 2, responsible for the worldwide COVID-19 pandemic, has caused unprecedented changes to society as we know it. The effects have been particularly palpable in the practice of medicine. The field of otolaryngology has not been spared. We have had to significantly alter the way we provide care to patients, changes that are likely to become a new norm for the foreseeable future. This article highlights some of the changes as they apply to otology/neurotology. Although this is written from the perspective of an academic physician, it is also applic

The Choosing Wisely Canada Campaign aims to raise awareness amongst physicians and patients regarding unnecessary tests and treatment. The otology/neurotology subspecialty group within the Canadian Society of Otolaryngology - Head & Neck Society developed a list of five common otologic presentations to help physicians deliver high quality effective care: (1) Don't order specialized audiometric and vestibular testing to screen for peripheral vestibular disease, (2) Don't perform computed tomography or blood work in the evaluation of sudden sensorineural hearing loss, (3) Don't perform auditory

Several cases illustrating potential pitfalls in neurotologic diagnosis from the viewpoint of the otologist are presented. The role of auditory brain stem response testing is specifically emphasized in cases with discordant audiologic and radiographic findings. Included are one case of a "false negative" ABR in a patient with an intracanalicular acoustic neuroma, a case of a "false positive" CT scan in a patient with Meniere's disease, and a case of a patient with normal hearing in whom an acoustic neuroma was discovered serendipitously.

In the evaluation of patients with complaints of dizziness, hearing loss, and/or tinnitus, the primary objective is to determine the site of the lesion. An accurate localization of the lesion is important for selecting the most appropriate radiologic study. The information obtained from the vestibular and audiometeric evaluations identifies the site of the lesion accurately. For the vestibular evaluation we use photoelectric nystagmography (PENG) and the Torok monothermal differential caloric test. The initial audiometeric evaluation consists of a pure-tone audiogram and speech discrimination
