Impact of diabetes mellitus on adenolymphangitis episodes in patients with filarial lymphedema-Towards an integrated care approach.

BACKGROUND: Filarial lymphedema, caused by lymphatic filariasis, is characterized by chronic swelling and recurrent skin infections. Acute adenolymphangitis (ADL) episodes significantly exacerbate morbidity. Diabetes mellitus (DM) increases susceptibility to infections; however, the relationship between diabetes and ADL frequency and severity in filarial lymphedema patients remains unclear. This study investigated the influence of diabetes on ADL attacks and identified predictors influencing ADL episodes in patients with filarial lymphedema. METHODOLOGY/PRINCIPAL FINDINGS: This case-control s

Elephantiasis mimicry in recurrent lower limb skin infections in a diabetic patient: a case report.

BACKGROUND: Chronic edema as a complication of systemic diseases or infections can mimic filarial lymphedema (also known as elephantiasis) and considered so. We describe a case of chronic lymphedema that mimicked elephantiasis in a diabetic man. CASE PRESENTATION: The patient was a 70-year-old black man, bed-bound at the time of admission following a diagnosis of stroke and hypertension in the previous 5 years. He had been diabetic for 20 years with poorly controlled diabetes mellitus. He suffered recurrent bilateral lower limb skin infections for 5 years prior to admission that culminated in

Lymphatic abnormalities in human filariasis as depicted by lymphangioscintigraphy.

BACKGROUND: Investigation into filarial lymphedema has been hampered by the lack of a simple, safe, and easily repeated test to image the peripheral lymphatic system. Recent refinements in radionuclide lymphangioscintigraphy have established this noninvasive technique as the initial procedure of choice for visualizing lymphatics. Accordingly, we applied lymphangioscintigraphy to patients with filariasis and, for purposes of interpretation, compared the findings with those in patients with non-filarial lymphedema. METHODS: Thirty-three patients with classic symptoms or signs consistent with ac

Podoconiosis: A Possible Cause of Lymphedema in Micronesia.

Podoconiosis is a type of tropical lymphedema sharing some clinical characteristics with lymphatic filariasis. Also referred to as endemic non-filarial elephantiasis, podoconiosis is a non-infectious disease from barefoot exposure to irritant red clay soil of volcanic origins. Podoconiosis is most common in Ethiopia and has also been reported in many other countries, but not in the Pacific Islands. Lymphatic filariasis is endemic in the Pacific Islands and was historically reported as elephantiasis in Micronesia. It was considered to have been eradicated in Guam and the Northern Mariana Island

Diagnosis of elephantiasis nostras verrucosa as a clinical mani-festation of Kaposi's sarcoma.

Elephantiasis nostras verrucosa, a rare manifestation of Kaposis sarcoma, is a progressive cutaneous hypertrophy caused by chronic non-filarial lymphedema secondary to obstruction of the lymphatic system that can lead to severe disfigurement of parts of the body that have gravity-dependent blood flow, due to edema, fibrosis, and hyperkeratosis, especially lower extremities. Among the various conditions that can induce chronic lymphedema are tumors, trauma, radiotherapy, obesity, hypothyroidism, chronic venous stasis, and AIDS-related Kaposis sarcoma. Kaposis sarcoma is a vascular tumor associa

Tropical leg lymphedema caused by podoconiosis is associated with increased colonisation by anaerobic bacteria.

The non-filarial and non-communicable disease podoconiosis affects around 4 million people and is characterized by severe leg lymphedema accompanied with painful intermittent acute inflammatory episodes, called acute dermatolymphangioadenitis (ADLA) attacks. Risk factors have been associated with the disease but the mechanisms of pathophysiology remain uncertain. Lymphedema can lead to skin lesions, which can serve as entry points for bacteria that may cause ADLA attacks leading to progression of the lymphedema. However, the microbiome of the skin of affected legs from podoconiosis individuals

Bacterial diversity significantly reduces toward the late stages among filarial lymphedema patients in the Ahanta West District of Ghana: A cross-sectional study.

BACKGROUND: Lymphatic Filariasis (LF), a neglected tropical disease, has been speculated to be complicated by secondary bacteria, yet a systematic documentation of these bacterial populations is lacking. Thus, the primary focus of this study was to profile bacteria diversity in the progression of filarial lymphedema among LF individuals with or without wounds. METHODS: A cross-sectional study design recruited 132 LF individuals presenting with lymphedema with or without wounds from eight communities in the Ahanta West District in the Western Region, Ghana. Swabs from the lymphedematous limbs,

Rare case of extralymphatic dirofilariasis presenting as a facial subcutaneous nodule in non-endemic region of Spain.

BACKGROUND: Filarial infestation is highly prevalent in tropical countries, lymphangitis and chronic lymphedema being the most common complaints in these regions. Yet, extralymphatic filariasis outside endemic areas is an extremely rare scenario. CASE PRESENTATION: We report a case of filariasis in the south of Spain. A previously healthy 49-year-old male patient reported a subcutaneous facial swelling. Surgery showed an encysted solitary filarial worm. CONCLUSION: To our knowledge, this is a rare clinical manifestation of filariasis in non-endemical regions in Spain. Still, it should be kep

Elephantiasis and verrucous lesions in an Ethiopian man living in Italy.

Elephantiasis refers to chronic, severe lymphedema caused by the abnormal accumulation of interstitial fluid due to reduced lymph transport and lymph stasis. This condition is observed secondary to injury, infection, cancer or cancer-related treatment, or congenital abnormalities of the lymphatic system. One of the several forms of elephantiasis is Elephantiasis Nostras Verrucosas (ENV), where "nostras" refers to a non-filarial form of lymphedema that occurs primarily in non-tropical areas. We describe a case of ENV in an Ethiopian man living in Italy, presenting with fever, hypotension, woody

The effectiveness of long-acting penicillin (penidur) in preventing recurrences of dermatolymphangioadenitis(DLA) and controlling skin, deep tissues, and lymph bacterial flora in patients with "filarial" lymphedema.

Dermatolymphangioadenitis (DLA) is a common and serious complication of so-called "filarial" and bacterial non-filarial lymphedema of the limb, affecting skin, lymphatics and lymph nodes. In our previous studies, we demonstrated that more than 60% of patients revealed presence of bacterial isolates in deep tissues, tissue fluid and lymph from the lymphedematous limbs. The question remained open whether elimination or suppression of bacteria dwelling in lymphedematous tissues by administration of low doses of penicillin for long time periods would prevent recurrence of DLA attacks. In this stud

Tissue tonometry: a useful tool for assessing filarial lymphedema.

A tissue tonometer was used to assess peripheral lymphedema in patients with filariasis in a Bancroftian endemic community. Matched populations of 34 patients with Grade II and 29 patients with Grade III unilateral lower limb edema and 26 healthy subjects were assessed for leg tissue compressibility and circumference. Tonometry was performed at three fixed points on the leg using three weight levels (70, 140, and 210 gms). The mean value of compressibility for each weight level and points measured in the edematous leg were significantly less compared with the contralateral non-edematous leg in

A Case of Elephantiasis Nostras Verrucosa Secondary to Lymphedema Praecox Complicated by Congestive Cardiac Failure.

BACKGROUND In lymphedema, an imbalance in the formation and absorption of lymph causes accumulation of protein-rich fluid in the interstitium of the most gravity-dependent parts of the body. Diagnosis is usually made based on patient medical history and a physical examination showing a typical appearance of the affected body part. Differential diagnosis is confirmed by imaging. CASE REPORT Primary lymphedema is inherited in through an autosomal dominant pattern. Congestive cardiac failure and non-filarial infections predispose patients to the secondary form of lymphedema, elephantiasis nostras

Unveiling the gendered abyss: exploring the case of women with lymphatic filariasis and the path to inclusive care in India.

INTRODUCTION: Lymphatic filariasis (LF), a neglected tropical disease (NTD), affects tropical regions marked by poor socioeconomic conditions and often results in non-curable filarial lymphedema. The impact of LF is often disproportionate, adversely affecting women due to a multitude of intersecting disadvantages. There has been little effort to understand the unique illness experience of women with LF. METHODS: To explore the interplay between gender and the illness experience of women afflicted with filarial lymphoedema, this qualitative study mapped the experience of 18 women and the perce

Podoconiosis - From known to unknown: Obstacles to tackle.

Podoconiosis is a non-filarial and non-communicable disease leading to lymphedema of the lower limbs. Worldwide, 4 million individuals live with podoconiosis, which is accompanied by disability and painful intermittent acute inflammatory episodes that attribute to significant disability adjusted life years (DALYs). Different risk factors like contact with volcanic red clay soil, high altitude (above 1000 m), high seasonal rainfall (above 1000 mm/year) and occupation (e.g., subsistence farmer) are associated with the risk of podoconiosis. Although podoconiosis was described to be endemic in 32

Podoconiosis: Review of this Unusual Cause of Lower Extremity Lymphedema and its Global Health Burden.

Podoconiosis, or non-filarial elephantiasis, affects nearly 4 million people globally, primarily in regions with volcanic red clay soil. Characterized by lower extremity lymphedema from prolonged barefoot exposure to irritant soil particles, it causes significant economic losses, including $200 million annually in Ethiopia. While its pathogenesis is unclear, genetic predisposition (HLA expression), inflammatory responses, and altered skin microbiota are implicated. Prevention focuses on footwear and hygiene, though these are costly. The stigma surrounding visible symptoms highlights the need f

Lymphatic filariasis: a method to identify subclinical lower limb change in PNG adolescents.

Lymphedema related to lymphatic filariasis (LF) is a disabling condition that commonly manifests in adolescence. Fifty-three adolescents, 25 LF infected and 28 LF non-infected, in age and sex-matched groups, using the Binax ICT rapid card test for filarial antigen were recruited to the study. None of the participants had overt signs of lymphedema. Lymphedema assessment measures were used to assess lower limb tissue compressibility (tonometry), limb circumference (tape measure), intra- and extra-cellular fluid distribution (bioimpedance) and joint range of motion (goniometry). The mean tonometr

The Concept of Chronic Edema-A Neglected Public Health Issue and an International Response: The LIMPRINT Study.

Lymphedema has always been a neglected global health care problem. A central requirement for the development of any chronic disease is the clear use of public health definitions that can be used internationally to define populations. The term "lymphedema" has historically been defined as either primary, resulting from failure of lymphatic development, or secondary, following damage to the lymphatics (e.g., cancer treatment, injury, or filariasis). Attempts to integrate causes of edema arising from damage to the venous system or the effects of gravity, immobility, and systemic disease have rare
