Immunohistochemical expression of androgen receptors in urothelial carcinoma of urinary bladder. Is it significant? Experience from coastal India.

BACKGROUND: Bladder carcinoma (BC) ranks second among the genitourinary cancers worldwide. Influence of androgens and expression of androgen receptors in neoplasms are recent findings which were implicated in the development of BC. We aimed to study androgen receptor (AR) expression in bladder urothelial neoplasms and correlate its expression with grade and stage of the tumor. METHODS: Immunohistochemistry (IHC) was done on samples collected in a tertiary care hospital over one year consisting of 71 urothelial BC and 20 non-neoplastic urothelial conditions. Two pathologists graded the IHC and

[Recent Progress in Novel Therapies for Non-Muscle Invasive Bladder Cancer].

Non-muscle invasive bladder cancer(NMIBC)accounts for about 70% of bladder cancers, with most patients treated by TURBT followed by intravesical therapy according to recurrence risk.BCG is effective but BCG-unresponsive cases often require cystectomy.Recently, novel therapies including immune checkpoint inhibitors, drug-delivery devices, and gene therapy have emerged. This chapter reviews recent and ongoing phase Ⅲ trials in BCG-naïve NMIBC.

[En bloc transurethral resection for non-muscle invasive bladder cancer].

Bladder cancer (BC) is a severe, and in some cases disabling disease for which no active detection strategy has been developed. It requires careful differential diagnosis, and is associated with a high risk for recurrence and progression. The choice of optimal treatment of non-muscle-invasive bladder Cancer (NMIBC) can reduce the rate of recurrence and improve oncologic outcomes. The development of the Vesical Imaging Reporting and Data System (VI-RADS) protocol, which has high sensitivity and specificity for assessing the degree of tumor invasion into the detrusor, has been changing the parad

French AFU Cancer Committee Guidelines - Update 2024-2026: Non-muscle invasive bladder cancer (NMIBC).

OBJECTIVE: To update the CCAFU recommendations for the management of non-muscle invasive bladder cancer (NMIBC). METHODS: A systematic review (Medline) of the literature from 20222024 was performed, taking into account the elements of diagnosis, treatment options and monitoring of NMIBC and evaluating references with their level of evidence. RESULTS: The diagnosis of NMIBC (Ta, T1, or CIS) is made after complete and deep tumour resection. The use of bladder photodynamic diagnosis and the indication for a second TURBT (4 to 6weeks later) help to improve the initial diagnosis. The risk of recu

Low-grade non-muscle-invasive bladder cancer: molecular landscape, treatment strategies and emerging therapies.

Low-grade non-muscle invasive bladder cancer is a specific category of bladder cancer with a favourable prognosis; however, its management presents several challenges. The risk of stage progression is very low, but approximately half of patients will experience recurrence within the first 5 years after diagnosis. This high propensity for recurrence, coupled with the threat of progression, mandates ongoing surveillance. However, the optimal frequency and duration of follow-up monitoring remain undefined. Current management strategies for low-grade non-muscle invasive bladder cancer rely heavily

Metastasis development in non-muscle-invasive bladder cancer.

Non-muscle-invasive bladder cancer (NMIBC) is the most common type of bladder cancer presentation and is characterized by a varying probability of recurrence and progression. Sporadically, patients with NMIBC might also develop tumour metastases without any pathological evidence of muscle-invasive disease within the bladder, a condition known as metastatic NMIBC. In the published literature, this phenomenon is limited to several case reports and small reviews, with few data regarding the possible aetiologies. Several possible factors can be potentially associated with metastatic NMIBC, includi

New Treatment Options for Non-Muscle-Invasive Bladder Cancer.

Non-muscle-invasive bladder cancer (NMIBC) comprises 75% of newly diagnosed bladder cancer and poses significant clinical challenges because of high recurrence and progression rates. Despite the effectiveness of Bacillus Calmette-Guérin (BCG) therapy after transurethral resection of bladder tumor (TURBT), BCG fails nearly 40% of patients, requiring alternative treatments. Traditionally, radical cystectomy has been the standard for BCG-unresponsive disease, although it significantly affects quality of life. Recent advances have focused on bladder-preserving therapies that leverage immune checkp

Novel Therapies for High-Risk Non-Muscle Invasive Bladder Cancer.

PURPOSE OF REVIEW: The treatment options for high-risk non-muscle invasive bladder cancer (NMIBC), particularly following BCG, remain limited. We highlight recent, promising therapies for high-risk NMIBC. RECENT FINDINGS: Several therapies utilizing different mechanisms of action have demonstrated favorable results in the BCG-naïve and BCG-unresponsive settings. These treatments include intravenous and intravesical immunotherapy, viral- and bacterial-based intravesical therapies, combination intravesical chemotherapy regimens, and novel intravesical chemotherapy administration. Overall, the e

Radiotherapy for High-Risk Non-Muscle-Invasive Bladder Cancer: Current Evidence and Future Directions.

Introduction: Non-muscle-invasive bladder cancer (NMIBC) represents approximately 78% of newly diagnosed bladder cancers and is characterized by high recurrence rates and variable progression risk. While transurethral resection of bladder tumor (TURBT) followed by intravesical therapy remains standard management, optimal treatment of high-risk and Bacillus Calmette-Guerin (BCG)-unresponsive disease remains controversial. Radiotherapy (RT), particularly in combination with chemotherapy, has been explored as a bladder-preserving alternative. Material and Methods: We conducted a narrative review

Impact of intravesical Bacillus Calmette-Guérin and chemotherapy on the bladder microbiome in patients with non-muscle invasive bladder cancer.

INTRODUCTION: Intravesical therapy (IVT), including Bacillus Calmette-Guérin (BCG), is the standard of care for high grade (HG) non-muscle invasive bladder cancer (NMIBC). Despite the use of IVT, many patients recur after treatment. The bladder microbiome and its role in disease processes has recently risen to prominence. We aim to characterize changes that occur in the bladder microbiome over the course of intravesical therapy and assess whether these changes correlate with outcomes in patients with NMIBC. METHODS: Patients with NMIBC undergoing induction BCG or intravesical therapy were pro

Advances in Diagnosis of Non-Muscle-Invasive Bladder Cancer: A Review.

Urothelial carcinoma is a rapidly evolving field. An important area of recent advancement is in the diagnosis and surveillance of nonmuscle invasive bladder cancer (NMIBC). Approximately 75% of newly diagnosed bladder cancers are nonmuscle invasive. At least half of the NMIBC diagnoses are categorized as low-risk disease, for which surveillance and bladder-sparing treatments are recommended.
