Endovascular-assisted microsurgical clipping of ophthalmic segment aneurysms.

BACKGROUND: Proximal arterial control is critical for safe and effective microsurgical clipping of ophthalmic segment aneurysms (OSAs). Traditionally, this is achieved via neck dissection and temporary clamping of the cervical internal carotid artery (ICA). Advances in endovascular technology have introduced temporary balloon occlusion (TBO) as a potentially less invasive alternative. This study aims to assess the utility of TBO during microsurgical clipping of OSAs. METHODS: A retrospective review was conducted of all patients at a single institution who underwent microsurgical OSA clipping

Preformulation-Assisted Design of Ketorolac Tromethamine for Effective Ophthalmic Delivery.

Background: The eye is a highly protected organ from ocularly administered drugs; drug- and formulation-related factors contribute significantly to ocular bioavailability. There has been a growing interest in using nonsteroidal anti-inflammatory drugs in ophthalmology for treating postoperative pain, inflammation, and seasonal allergic conjunctivitis. A preformulation-assisted design boosts efficacy and reduces dose requirements. Methods: This work aims to study the preformulation characteristics of ketorolac tromethamine to improve ocular performance and future formulation development through

Formalin-assisted training eyes for ophthalmic wet lab practice.

The foundation of an ophthalmologists' microsurgical career begins in the wet lab. Training on donor cadaveric, animal like goat or pig eyes provide the most realistic surgical environment, however, the availability of a donor's eyes for practice is limited. This scarcity is further escalated in this current coronavirus disease 2019 pandemic where eye donations have decreased. Even among those eyes which find their way into the wet lab, quite a few would have collapsed significantly making training difficult. Therefore, we looked at ways to salvage these collapsed globes. We describe a novel w

Assisted reproductive technology and long-term ophthalmic morbidity of the offspring.

In this study, we investigate if children born following assisted reproduction technologies (ARTs) are at an increased risk for long-term ophthalmic complications. For this purpose, a population-based cohort analysis was conducted which included all deliveries between 1991 and 2014 at a single tertiary medical center. Offspring were classified relative to conception method as ART or spontaneous pregnancies. Offspring hospitalizations up to the age of 18 years involving ophthalmic morbidities were evaluated according to a predefined set of ICD-9 codes. A Kaplan-Meier survival curve was used to

Stereo reconstruction from microscopic images for computer-assisted ophthalmic surgery.

PURPOSE: This work presents a novel platform for stereo reconstruction in anterior segment ophthalmic surgery to enable enhanced scene understanding, especially depth perception, for advanced computer-assisted eye surgery by effectively addressing the lack of texture and corneal distortions artifacts in the surgical scene. METHODS: The proposed platform for stereo reconstruction uses a two-step approach: generating a sparse 3D point cloud from microscopic images, deriving a dense 3D representation by fitting surfaces onto the point cloud, and considering geometrical priors of the eye anatomy.

Rectification of Cornea Induced Distortions in Microscopic Images for Assisted Ophthalmic Surgery.

OBJECTIVE: Image distortions induced by the high refractive power of the eye's optical components challenge the accuracy of geometric information derived from intra-operative sensor data in ophthalmic surgery. Correcting these distortions is vital for advancing surgical assistance systems that rely on geometric scene comprehension. In this work, we focus on cornea induced distortions (CIDs) in surgical microscope images of the anterior eye. METHODS: We employ a convolutional neural network (CNN) with stereo fusion layers to predict distortion distribution maps (DDMs) to correct CIDs in stereo

Ophthalmic artery aneurysm treated by surgical clipping and balloon-assisted temporary carotid occlusion.

A 29-year-old woman is diagnosed with a large broad-base right ophthalmic artery aneurysm. Despite an intact visual field, she complained of mild right visual blurriness. Between endovascular and surgical treatments, surgery was favored due to her young age. It was decided to perform the surgery with simultaneous endovascular temporary carotid balloon occlusion. A right pterional craniotomy and intradural anterior clinoidectomy were performed, the balloon was positioned in the petrous carotid, and the distal durai ring was opened exposing the proximal neck. Under temporary proximal carotid bal

AI-assisted diagnosis of neuro-ophthalmic disorders: a systematic review from optic neuritis to papilledema.

BACKGROUND: Neuro-ophthalmic disorders like optic neuritis and papilledema are diagnostically challenging, with non-specific symptoms and reliance on expert interpretation of optic nerve head imaging (Liu TYA et al. J Neuroophthalmol. 41(3):368–74, 2021). Misdiagnosis can lead to severe consequences, including irreversible vision loss or failure to detect life-threatening conditions. Artificial intelligence (AI), particularly deep learning (DL), shows promise but its application in this nuanced field is not well-defined. OBJECTIVE: This systematic review evaluates the current state of AI in di

Ophthalmic viscosurgical device-assisted sutureless-incision cataract surgery for a hard nucleus or mature cataract.

UNLABELLED: We describe a modified method of sutureless-incision cataract surgery using an ophthalmic viscosurgical device (OVD) to extract a hard lenticular nucleus or mature cataract. After an isosceles trapezoid-shaped sclerocorneal tunnel and a round 7.0 mm diameter capsulorhexis are made, the nucleus is displaced into the anterior chamber. As OVD is injected, the OVD cannula acts as a slide to guide the nucleus out of the eye. Ophthalmic viscosurgical device-assisted sutureless-incision cataract surgery was used in a consecutive series of 182 eyes with a hard nucleus (57 eyes), mature cat
