Evaluating shared genetic influences on nonsyndromic cleft lip/palate and oropharyngeal neoplasms.

It has been hypothesised that nonsyndromic cleft lip/palate (nsCL/P) and cancer may share aetiological risk factors. Population studies have found inconsistent evidence for increased incidence of cancer in nsCL/P cases, but several genes (e.g., CDH1, AXIN2) have been implicated in the aetiologies of both phenotypes. We aimed to evaluate shared genetic aetiology between nsCL/P and oral cavity/oropharyngeal cancers (OC/OPC), which affect similar anatomical regions. Using a primary sample of 5,048 OC/OPC cases and 5,450 controls of European ancestry and a replication sample of 750 cases and 336,3

Oropharyngeal Mixed Neuroendocrine-Nonneuroendocrine Neoplasm (MiNEN): A Case Report and Literature Review.

Mixed neuroendocrine-nonneuroendocrine (MiNEN) neoplasms in the head and neck are exceptionally rare biphasic tumors with unclear pathogenesis and an aggressive clinical behavior. This is the first reported case of an oropharyngeal MiNEN with the nonneuroendocrine component being an HPV-associated adenocarcinoma. The tumor arose in a 56 year-old male with history of long-term cigarette smoking and was composed of an adenocarcinoma intermixed with a small cell neuroendocrine carcinoma. P16 immunohistochemical stain and HPV16/18 in-situ hybridization were strongly and diffusely expressed in both

Localized Tonsillar Amyloidosis Associated With a Plasma Cell Neoplasm Presenting as an Oropharyngeal Mass: A Diagnostic Challenge.

Amyloidosis is a rare and heterogeneous group of disorders characterized by extracellular deposition of misfolded proteins, which may present as either localized or systemic disease. In the head and neck region, the presentation of amyloidosis is uncommon and may clinically mimic neoplastic processes. We report the case of a 58-year-old man with a five-year history of progressive oropharyngeal symptoms, initially perceived as a foreign body sensation and intermittent odynophagia, later evolving to dysphagia to solids. Physical examination revealed marked asymmetry of the palatine tonsils due t

Association of human papillomavirus (HPV) infection with the occurrence of second neoplasms and their prognostic impact in patients with oropharyngeal carcinoma.

INTRODUCTION: This study analyses the incidence of second neoplasms in patients with oropharyngeal carcinoma according to human papillomavirus (HPV) status and its impact on survival. MATERIAL AND METHODS: A retrospective analysis of 583 patients with oropharyngeal cancer treated between 1991 and 2023 was performed. A total of 112 patients (19.2%) had HPV-positive tumours. RESULTS: During the follow-up, 181 patients (31.0%) had a second neoplasm. The 5-year and 10-year second neoplasm-free survival for HPV-negative patients were 60.5% and 37.5%, significantly lower than in HPV-positive patie

Bazex syndrome (acrokeratosis paraneoplastica): persistence of cutaneous lesions after successful treatment of an associated oropharyngeal neoplasm.

Acrokeratosis paraneoplastica is a rare paraneoplastic syndrome commonly affecting males over 40 years of age. There exists a strong association with squamous cell carcinoma (SCC) of the upper aerodigestive tract or cervical metastatic disease originating from an unknown primary. We report a case associated with SCC of the right tonsil with persistent paraneoplastic cutaneous lesions 2 years after successful treatment of the underlying neoplasm.

Parotid squamous cell carcinoma metastases: Application of human papillomavirus-DNA test on liquid-based cytology to recognize oropharyngeal origin of the neoplasm.

Malignancies of the parotid gland are relatively uncommon and in most cases are primary neoplasms; intraparotid metastases are rare. Oral and oropharyngeal squamous cell carcinoma (O- and OP-SCC) can potentially metastasize to the parotid gland or intraparotid lymph nodes. Fine-needle aspiration cytology (FNAC) serves as the initial diagnostic approach for this purpose. HPV status in FNAC specimens is relevant and can guide the diagnostic workup, indicating a potential oropharyngeal origin of the primary tumor. A small series of occult SCC metastases is presented below, in which HPV-DNA testin

Tonsillar Actinomycosis Simulating an Oropharyngeal Neoplasm in a 67-Year-Old Man With Diabetes Mellitus, Hypertension, and Ischemic Heart Disease: A Case Report From King Faisal General Hospital, Hofuf, Saudi Arabia.

Actinomyces species constitute part of the normal microbial flora of the oral cavity. When mucosal integrity is disrupted, these organisms can produce invasive cervicofacial infections. Tonsillar actinomycosis can closely mimic an aggressive oropharyngeal malignancy in elderly patients with significant comorbidities. A 67-year-old man with type 2 diabetes mellitus, hypertension, and ischemic heart disease presented with a chronic progressively enlarging right tonsillar mass accompanied by throat discomfort. Clinical assessment and computerized tomography imaging scan revealed a 42 mm × 24 mm ×

Dementia with oropharyngeal dysphagia and myopathy.

A 72-year-old man was referred for geriatric evaluation with a view toward placement in institutional care. He presented originally to an internal medicine team with a six-month history of weight loss, constipation, generalized weakness, and apathy; investigations to rule out an underlying neoplasm were negative. Interdisciplinary assessment revealed coexisting dementia, myopathy, and oropharyngeal dysphagia. These findings prompted further diagnostic evaluation and a diagnosis of inflammatory myopathy with associated oropharyngeal dysphagia and dementia was made. The dementia, myopathy, and o

Carcinoma of the soft palate and uvula.

Carcinoma of the soft palate and uvula is a rare form of oropharyngeal neoplasm with incidence ranging from 5% to 12% of all oropharyngeal carcinomas. The lesions are more commonly found in men in the sixth and seventh decade of life. Because of the high incidence of potential velopharyngeal incompetence, surgical resections are usually reserved for the lesions confined to the uvula, where larger lesions are treated with radiation therapy.

p53 mutations in oropharyngeal carcinomas: a comparison of solitary and multiple primary tumours and lymph node metastases.

Despite the steadily increasing number of patients suffering from squamous cell carcinomas of the oropharyngeal region, little is known about the molecular steps involved in the induction of these neoplasms. The aim of this study was to determine the incidence and type of p53 mutations in oropharyngeal squamous cell carcinomas. Five of 38 patients had lymph node metastases, three patients had multiple primary carcinomas and two patients presented with multiple primary tumours and lymph node metastases. Exons 5 through 8 of the p53 gene were screened by single-strand conformation polymorphism a

BCOR-Rearranged Sarcoma of the Tonsil.

A tonsillar mass in a young patient with no medical issues routinely presents as an infectious process. Practitioners must maintain a broad differential if diagnostic testing does not support an infection. Neoplasm must be excluded. Otolaryngologists must consider malignancies other than squamous cell carcinoma, the most common oropharyngeal malignancy, and lymphoma. Rare tumors, such as sarcomas, must also be considered. Otolaryngologists must be familiar with the proper management of rare oropharyngeal malignancies.

[Suprahyoid pharyngotomy for surgical therapy of malignant and benign oral and hypopharyngeal tumors].

Many cases of oropharyngeal and hypopharyngeal neoplasms without diffuse infiltration of the larynx or mandible cannot be treated effectively by a transoral approach. In such cases a lateral and/or median translingual pharyngotomy can permit effective surgical therapy. However, these surgical techniques require greater effort and violate uninvolved tissues, such as the lip, mandible and floor of the mouth. In contrast to this, a suprahyoid pharyngotomy can be a simple and precise approach to the oropharynx and hypopharynx, and provide the shortest distance to the pathological process. The exce

Metastatic breast carcinoma in the parapharyngeal space.

The parapharyngeal space is a complex anatomical area, which can give rise to a variety of both primary and metastatic neoplasms. We present an unusual case of metastatic adenocarcinoma of the breast, masquerading as a deep lobe parotid neoplasm, 15 years after the primary presentation. Metastatic breast carcinoma has been previously reported at other sites in the head and neck but this is the first report of metastases in the parapharyngeal space with an oropharyngeal presentation.

Congenital oropharyngeal teratoma in a neonatal goat and brief review of extragonadal teratomas in animals.

Teratomas originate from pluripotent germ cells and differentiate into the 3 germ cell layers: endoderm, mesoderm, and ectoderm. Hence, these tumors arise most often in the gonads. Extragonadal teratomas are rare in veterinary medicine. Congenital oropharyngeal teratoma, also known as epignathus, is a neoplasm that has been reported in humans and a few veterinary species. We describe the clinical, gross, cytologic, and histopathologic features of an oropharyngeal teratoma in a neonatal Boer × Nigerian Dwarf goat that died within 4 h of birth, and briefly review extragonadal teratomas in veteri
