Rapid expansion of the palate with a new surgical technique.

After briefly reviewing the literature concerning palatal expansion, a new technique combining surgical and medical orthodontic treatment is described. The technique, which entails detaching the nasal septum, is particularly appropriate in oral-breathing patients. Sixty-one patients (aged 10-32 years) were included in the study. The technique, which has low morbidity, gave good short- and long-term results.

Surgical and orthodontic rapid palatal expansion in adults using Glassman's technique: retrospective study.

In 1984, Glassman et al. described a conservative surgical method of separation of the midpalatal suture in which an osteotomy is done only at the lateral and anterior wall of the maxilla. Between 1991 and 1997, we have operated on 21 patients with maxillary transverse discrepancies using the method that they described. This gave good results in 20 patients. The other, who was operated on at the age of 38 years, developed a fracture of the alveolar process of the maxilla on one side because of ossification of the midpalatal suture. The surgically assisted rapid palatal expansion described by G

A Digitally Guided, Single-Visit Workflow for Miniscrew-Assisted Rapid Palatal Expansion: The YOM Technique.

Miniscrew-assisted rapid palatal expansion (MARPE) extends non-surgical, predominantly skeletal expansion to skeletally mature patients. Its success depends on the palatal miniscrews achieving sound engagement in a safe site and on the expander and the screws fitting together accurately. Two sequences are described. In a bone-first sequence, the miniscrews are inserted first, commonly through a CAD/CAM (computer-aided design/computer-aided manufacturing) guide that controls trajectory and depth, after which the appliance must be fitted to the screws in the positions they actually occupy. In an

A comparison of two different rapid palatal expansion techniques from the point of root resorption.

The aim of this investigation is to compare Haas and Cast Cap Splint devices from the point of root resorption. The material comprised thirty-eight upper and twelve lower premolar teeth derived from nineteen patients who required RME and subsequent removal of the first premolars as part of their full banded orthodontic treatment. Root resorption and repair areas were observed on the buccal surfaces of premolars. Repair tissue was cellular cementum in both groups. There was no significant difference between these two techniques from the point of root resorption amount.

Minimally invasive surgically assisted miniscrew anchored rapid palatal expansion (sa-MARPE) technique: avoiding conventional horizontal maxillary osteotomies - a technical note.

In skeletally mature patients, transverse maxillary deficiency poses a significant challenge due to the rigidity of craniofacial sutures. This technical note describes a minimally invasive, surgically assisted MARPE (sa-MARPE) protocol designed to overcome these limitations while avoiding complete Le Fort I osteotomies. The technique involves three small mucosal incisions and limited piezosurgical osteotomies targeting the midpalatal suture, canine pillars, and pterygomaxillary junctions. By preserving mucosal integrity and vascular supply, this approach reduces postoperative morbidity and enh

Three-Dimensional Effects of Two Minimally Invasive Surgically Assisted Rapid Palatal Expansion Techniques.

OBJECTIVE: This study evaluated, in three dimensions, the skeletal effects of two minimally invasive bone-anchored techniques for surgically assisted palatal expansion, with and without pterygomaxillary disjunction (PD) and nasal septum disinsertion (NSD). MATERIALS AND METHODS: Thirty-nine patients were retrospectively selected from two centres and assigned to groups: G1 (without PD and NSD) and G2 (with PD and NSD). Cone-beam computed tomography scans were analysed before and after expansion, using landmarks to assess skeletal changes along the transverse (X), vertical (Y) and anteroposteri

Is significant mandibular advancement possible after the peak of puberty? Dento-osseous palatal expansion and the STM4 technique (Skeletal Therapy Manni Telescopic Herbst 4 miniscrews): A case report.

INTRODUCTION: Treatment of skeletal class II growing patients often requires the use of functional appliances, aimed at promoting mandibular advancement. Among these, Herbst appliance is recommended for its effectiveness, efficiency, and reduced need for compliance. Despite its skeletal favourable effects, well-known dental compensations can occur, especially when the appliance is not used close to the pubertal peak: upper incisors retroclination, lower incisors proclination, upper molars distalization and lower molars mesialization could reduce the overjet needed for a proper mandibular advan

Effect of surgical exposure technique, age, and grade of impaction on ankylosis of an impacted canine, and the effect of rapid palatal expansion on eruption: A prospective clinical study.

INTRODUCTION: This study had 2 aims: (1) to assess whether the surgical exposure technique, the patient's age, and the grade of impaction are associated with ankylosis of the impacted canine; and (2) to investigate the effect of rapid palatal expansion on an impacted canine's automatic eruption. METHODS: The sample for this prospective longitudinal study consisted of 118 orthodontic patients (72 female, 46 male) who were treated surgically and orthodontically by the first author (S.I.K.) over 18 years. The patients' ages at the beginning of therapy ranged from 11.2 to 46.1 years. They had 157

Minimally Invasive Separation of Mid-Palatal Suture Using Piezosurgery: A Novel Type of SARME Assisting Maxillary Expansion in Adults.

Maxillary transverse deficiency (MTD), a common malocclusion, is typically treated with maxillary expansion. However, adult MTD patients often present challenges for skeletal expansion due to matured mid-palatal sutures. To address this, the authors developed a minimally invasive surgical technique utilizing piezosurgery to separate the mid-palatal suture and buccal/lingual bone sutures in the anterior maxilla. Then, the Hyrax expander was used for rapid maxillary expansion (RME). This approach effectively achieves maxillary skeletal expansion with successful mid-palatal suture opening, offeri

Three-dimensional morphometric analysis of the effects of subperiosteal palatal soft-tissue expansion in growing cats.

A prospective longitudinal study in 75 growing cats was conducted to evaluate palatal soft-tissue expansion in cleft lip and palate surgery. In 31 cats, palatal scars were induced by simulated Langenbeck surgery at the age of 8 weeks. At the age of 14 weeks, custom-made tissue expanders were inserted in 61 animals. Tissue expansion was performed by weekly inflation in 33 cats (16 without and 17 with scars) for an 8-week period. The remaining 28 cats (14 without and 14 with scars) served as sham groups. A control group was formed by 14 animals (without scars and without tissue expander). The ef

Two-dimensional cephalometric analysis of the effects of subperiosteal palatal soft-tissue expansion in growing cats.

The feasibility and possible effects of palatal soft-tissue expansion in palatal repair were studied. A prospective longitudinal animal experiment was performed in 75 growing cats assigned to 5 groups. In 31 cats, a midline defect was made, and bipediced flaps were raised at the age of 8 weeks (stimulated Langenbeck operation) in order to create palatal scars. At the age of 14 weeks, custom-made tissue expanders were inserted palatally in 61 animals. Tissue expansion was performed by weekly inflation in 33 cats (16 without and 17 with scars) for an 8-week period. The remaining 28 cats (14 with
