[The "patellofemoral joint" problem].

In the differential diagnosis of femoropatellar joint disease, three conditions must be considered: (1) peripatellar irritation; (2) chondromalacia of the patella; (3) femoropatellar arthrosis. In approximately 90% of the cases, peripatellar irritation and chondromalacia of the patella can be kept under control by active extension and strengthening exercises for the femoral flexor and extensor musculature. In therapy-resistant cases, arthroscopic surgery can provide the opportunity to influence the pathological gliding process positively. The operative procedure is planned on the basis of clin

Imaging of the patellofemoral joint.

The patellofemoral (PF) joint is a complex articulation, with interplay between the osseous and soft tissue structures to maintain the balance between knee mobility and stability. Disorders of the PF joint can be a source of anterior knee pain (AKP). In this article, radiographic and magnetic resonance imaging of the PF joint are reviewed, including normal anatomy, imaging techniques, and imaging-based measurements. Common imaging findings associated with AKP are reviewed, including symptomatic normal variants, tendinopathy, apophysitis, osteoarthritis, chondromalacia patella, trochlear dyspla

MRI of the patellofemoral joint.

In routine MRI of the knee, less attention frequently is paid to the patellofemoral joint and its surrounding structures than to the menisci and ligaments. This article reviews MRI of common abnormalities of the anterior aspect of the knee. Chondromalacia patellae, pathological softening of the patellar cartilage, can be diagnosed with reasonable accuracy with MRI using standard sequences. Abnormalities of alignment can be assessed grossly on static images, but more sophisticated evaluation of patellofemoral tracking requires the kinematic technique. Other traumatic or overuse injuries involvi

Biomechanics of the patellofemoral joint.

The patellofemoral joint contains 2 complex mechanisms for ameliorating forces transmitted across it, namely increasing extensor lever arm-r in the important range of flexion 30 degrees-70 degrees and increasing contact area with increasing flexion. In this range the patella bears sole responsibility for transmitting resultant flexion of quadriceps force to the femur. Here lies the thickest cartilage in the body and perhaps not surprisingly, the most frequent site of cartilage degeneration. The Q-angle and valgus vector explain the predominance of the pathologic lesions on the lateral side of

Bracing for the Patellofemoral Joint.

Patellofemoral disorders are common causes of knee pain that result in frequent visitations to musculoskeletal care clinics. Patellar tendinopathy, patellar instability and patellar maltracking, and pain are some of the most common pathologies resulting in patellofemoral dysfunction. For each of these diagnoses, there are unique orthoses and braces available, some of which are uniquely designed to address the pathology involved. While the spectrum of patellofemoral disorders is wide ranging and can often be challenging to treat, bracing frequently plays a large role in the overall treatment al

Rehabilitation of patellofemoral joint disorders.

Patellofemoral dysfunction may be one of the most common and troublesome maladies to affect a patient. The correct identification of the underlying causes is paramount. Once identified, an effective rehabilitation program should be implemented. The program should include stretching and strengthening as well as several other therapeutic modalities. The most successful rehabilitation program is designed to address the specific needs of the patient.

Osteochondritis dissecans of the patellofemoral joint.

Osteochondritis dissecans of the patellofemoral joint is an uncommon condition that may be the cause of anterior knee pain or crepitus. We present the clinical features of 37 patients with osteochondritis dissecans lesions of the patellofemoral joint (24 on the patella, 13 on the trochlear groove), including two patients with medial trochlear groove lesions, which have not, to our knowledge, been previously reported. The osteochondral lesions involved the convex articular surfaces. The median age of patients when first examined was 15 years, and 54% of patients had open epiphyses. These lesion

Imaging Evaluation of the Patellofemoral Joint.

BACKGROUND: Patellofemoral anatomic abnormalities can result in a variety of issues and require thorough clinical evaluation as well as imaging analysis to determine the cause and help guide treatment. Multiple measurements across multiple imaging modalities are used to aid in proper diagnosis. INDICATIONS: Diagnostic findings on imaging evaluation require assessment of the imaging modality, as well as the use of appropriate measurements to ensure results are reliable and accurate. TECHNIQUE DESCRIPTION: Rotational malalignment of the knee can be evaluated with plain-film radiographs of the

Cartilage Restoration in the Patellofemoral Joint.

Although patellofemoral (PF) chondral lesions are common, the presence of a cartilage lesion does not implicate a chondral lesion as the sole source of pain. As attributing PF pain to a chondral lesion is "diagnosis by exclusion," thorough assessment of all potential structural and nonstructural sources of pain is the key to proper management. Commonly, multiple factors contribute to a patient's symptoms. Each comorbidity must be identified and addressed, and the cartilage lesion treatment determined. Comprehensive preoperative assessment is essential and should include a thorough "core-to-flo

Physical examination of the patellofemoral joint.

Examination of the patellofemoral joint can prove to be challenging. Although certain acute injuries such as patella fracture or tendon rupture can be diagnosed quickly, more chronic injuries such as patellar subluxation and patellofemoral pain syndrome are more difficult to diagnose because of the subtlety of the examination findings. The source of the problem can also vary, and must be identified to direct treatment. Adding to the complexity is that other structures around the knee may present with anterior knee pain and can be mistaken for patellofemoral disorder, which is why the patellofe

[Imaging examinations of the patellofemoral joint].

Lesions in the patellofemoral region can be caused by trauma, chronic overloading, and especially regarding cartilage alterations by normal aging or pathologic processes. Very commonly these lesions lead to early arthrosis. An accurate clinical evaluation in all these patients is recommended. The combination of clinical information and radiological examinations should end up with an exact diagnosis. As part of the radiological evaluation of complaints of the patellofemoral region MR imaging is of special value since this method allows direct visualization of all intra- and extra-articular struc

Prosthetic replacement of the patellofemoral joint.

We are just beginning to fully appreciate the significant role of the patellofemoral joint in total knee replacement surgery. Most of the new generation prostheses have a flanged femoral component. The tendency is to make the flange more anatomic in shape as more attention is paid to patellar tracking. Most systems have an optional polyethylene button to resurface the patellar side of the patellofemoral joint, and indications for its use are increasing. Although pain relief with patellar buttons seems uniform, the potential complications of stress fracture, loosening, and wear must temper our

Anatomy and malalignment of the patellofemoral joint: its relation to patellofemoral arthrosis.

Patellofemoral joint function involves a complex orchestration of a number of dynamic, static, and neuromuscular factors. I will discuss the components of patellofemoral anatomy and whether the presence of certain anatomic variables may increase the risk of patellofemoral arthrosis. The patellofemoral surgeon is faced with several clinical challenges: the semantics of the language used to discuss these problems, the correlation of clinical symptoms and radiographic imaging to help diagnose a problem, and surgical challenges in optimizing extensor mechanism function.

Reconstructive surgery for patellofemoral joint incongruency.

PURPOSE: A retrospective analysis of a heterogeneous patient cohort was performed to determine the outcome and eligibility of a combined trochleaplasty and soft tissue-balancing technique for repair of patellofemoral joint disorders. METHODS: A strict surgical treatment algorithm including trochleaplasty and reconstruction of the medial patellofemoral ligament and vastus medialis oblique muscle was implemented to restore the patellofemoral joint. A heterogeneous patient cohort including 46 consecutively treated symptomatic knees was reviewed. The median follow-up period was 4.7 years (range 2

Patellofemoral joint: from instability to arthritis.

Disorders of the patellofemoral joint are commonly seen in musculoskeletal clinics. In recent years, the expansion of imaging techniques, improvements in correlative injury anatomy, and more focused physical examinations have resulted in new knowledge about patellofemoral disorders. To achieve optimal patient outcomes, it is helpful for orthopaedic surgeons who treat knee problems to review the management of patellar dislocations and isolated patellofemoral arthritis, including treatment algorithms.

Gender differences in patellofemoral joint biomechanics.

Patellofemoral pain is associated with patellar malalignment and quadriceps weakness which are seen more commonly in women. The objective of the current study was to determine the effects of gender, vastus medialis strength, and tibial rotation on patellofemoral joint biomechanics. Twelve fresh-frozen knees from cadavers were tested using a custom knee jig. Anatomic multiplane loading of the extensor mechanism was used with varying vastus medialis loads. Patellofemoral contact area and pressure were measured using pressure sensitive film at knee flexion angles of 0 degrees, 30 degrees, 60 degr

The patellofemoral joint: from dysplasia to dislocation.

Patellofemoral dysplasia is a major predisposing factor for instability of the patellofemoral joint. However, there is no consensus as to whether patellofemoral dysplasia is genetic in origin, caused by imbalanced forces producing maltracking and remodelling of the trochlea during infancy and growth, or due to other unknown and unexplored factors. The biomechanical effects of patellofemoral dysplasia on patellar stability and on surgical procedures have not been fully investigated. Also, different anatomical and demographic risk factors have been suggested, in an attempt to identify the recurre
