Periprosthetic tibial fractures.

Periprosthetic tibial fractures following total knee arthroplasty are rare; they are generally related either to an unrecognized intraoperative weakening of the tibial cortex or to an actual traumatic event occurring in an osteoporotic context. Their management requires careful assessment of the tibial component's stability, the location of the fracture line, and the patient's overall condition. This literature review addresses the following questions: (1) What are the epidemiology, mechanisms, and classification of these fractures? (2) Is there still a role for conservative (non-operative) ma

Periprosthetic femoral fractures.

The incidence of periprosthetic femoral fractures is rising in the UK, because of an ageing population and an increasing number of hip arthroplasty operations being performed. They can occur intra- or postoperatively, and usually follow low energy trauma. They present with pain, swelling over the thigh, and an inability to weight bear. Periprosthetic femoral fractures are usually classified as per the unified classification system. Their management usually is dependent on their classification, with type A (fracture at level of greater or lesser trochanter) managed non-operatively with protecte

Periprosthetic femoral fractures.

Periprosthetic fractures of the femur are increasing both in the absolute number of fractures per year and in frequency (proportion of fractures among patients with hip arthroplasty). As a result, orthopaedic surgeons will face these challenging injuries more often. The optimal treatment must be individualized, taking into consideration the fracture location relative to the arthroplasty component, the implant stability, the underlying quality of the bone, and the medical and functional status of the patient. Based on these factors, successful treatment usually involves some combination of open

Periprosthetic knee fractures.

Fractures of the distal femur, proximal tibia, and patella that occur adjacent to a total knee replacement may be very difficult to treat. Fixation options are compromised because of the short articular segment, frequent comminution, pre-existing osteoporosis, previous surgical incisions, and the presence of the knee implant itself. This review article summarizes current concepts regarding the incidence, classification, treatment options, and outcomes for periprosthetic fractures of the knee.

Periprosthetic Tibial Fractures.

Periprosthetic fractures around total knee arthroplasty have become an increasingly common and challenging orthopaedic problem. Appropriate management of these fractures depends on careful scrutiny of radiographs and a thorough clinical history to exclude the diagnosis of a periprosthetic infection. In a periprosthetic tibial fracture with a stable, well-aligned tibial component and well-aligned mechanical tibial axis, the fracture can be successfully managed with closed reduction and cast immobilization; meticulous follow-up is essential to ensure that the alignment is maintained. Major fract

[Periprosthetic Acetabulum Fractures].

In contrast to periprosthetic fractures of the femur, periprosthetic fractures of the acetabulum are rare complications - both primary fractures and fractures in revision surgery. This topic is largely under-reported in the literature; there are a few case reports and no long term results. Due to an increase in life expectancy, the level of patients' activity and the number of primary joint replacements, one has to expect a rise in periprosthetic complications in general and periprosthetic acetabular fractures in particular. This kind of fracture can be intra-, peri- or postoperative. Intraope

Periprosthetic acetabular fractures.

PURPOSE: The aim of this article is to propose a diagnostic and therapeutic algorithm for the acetabular periprosthetic fractures. METHODS: This article explores the current literature on the epidemiology, causes and classification of periprosthetic acetabular fractures. Integrating data with the experience of the authors, it offers a guide to diagnosis and possible therapeutic strategies. RESULTS: Intra-operative fractures can occur during rasping, reaming or implant impaction, and they must be treated immediately if the component(s) is (are) unstable. Post-operative fractures can be due to

Periprosthetic patellar fractures.

UNLABELLED: Patellar fracture after total knee arthroplasty is a rare yet challenging complication. Patellar fracture can occur as a result of trauma or it may be atraumatic. A multitude of factors can lead to periprosthetic patellar fracture including patient related factors, surgical technique related factors, and implant specific factors. Understanding the etiologic factors leading to atraumatic patellar fractures could result in minimizing complications. We present the results of peri-prosthetic patellar fractures in 12 patients. All type I non-displaced fractures (7 cases) were treated no

Infection in periprosthetic hip fractures.

Fracture around the acetabulum and femur in total hip arthroplasty is a possible complication, sometimes with difficult surgical solution, namely when a sepsis is present. Periprosthetic hip fractures were classified according to a modified Vancouver classification. We treated 112 patients (67 men and 45 women) with periprosthetic hip fractures: 105 femoral fractures (94%) and seven acetabular fractures (6%). Mean follow-up was 10.6 years. For Type A (seven cases - 7%), B1 (10 cases - 10%) or C (13 cases - 12%) fractures around well fixed femoral stems we only fixed the fractures. For Type B2

Periprosthetic fractures of the femur.

Treatment of periprosthetic femur fracture is based on fracture timing, implant stability, fracture pattern, and bone quality. Intraoperative fractures are usually stable; to prevent intraoperative fracture, careful preoperative planning and gentle operative techniques are essential. In managing unstable intraoperative and late postoperative periprosthetic fractures, the surgeon should be know the exact pattern of fracture, prosthesis stability, and bone quality. Loose prostheses should be revised and displaced fractures should be reduced and adequately fixed. These fractures typically occur i

Periprosthetic fractures of the knee.

Periprosthetic total knee arthroplasty fractures of the distal femur and proximal tibia can be among the most difficult complications to effectively manage within the realm of joint replacement. These fractures can occur intraoperatively or postoperatively. Intraoperative fractures can be avoided by early removal of hardware, use of stems for stress risers, and use of intraoperative radiographs whenever further visualization is required. Intraoperative fractures should be fixed and then protected by a stem and avoidance of weight bearing until healed. Postoperative fractures can occur with sig

Management of periprosthetic femoral fractures.

We followed up 37 periprosthetic fractures to assess the factors related to the outcomes and the loosening of prostheses. The average union time is 3.1 months (range, 2-6.2 months). The most common types were fractures around the stem tip and displaced (89.1%). In our study, the loosening rate (32.4%) was low compared with other reports. These might be due to the cementless stem, but the difference was not significant (P>.05). After management of the fracture with a plate, we noted 5 complications: 2 nonunion and 3 refractures. The causes of these complications were related to screw in the lev

Periprosthetic fractures of the knee.

Periprosthetic fractures around total knee arthroplasty are an increasingly common and complex problem driven by rising arthroplasty volumes, aging populations, and longer implant survivorship. Successful management requires integration of trauma principles with arthroplasty-specific considerations, particularly implant stability, bone stock, and patient frailty. Implant stability and bone quality are the primary determinants of treatment strategy, outweighing fracture morphology alone. Classification systems are useful frameworks but must be interpreted in the context of component fixation an

[Classification of periprosthetic shoulder fractures].

The key targets in the treatment of periprosthetic humeral fractures (PHF) are the preservation of bone, successful bony consolidation and provision of a stable anchoring of the prosthesis with the major goal of restoring the shoulder-arm function. A substantial problem of periprosthetic shoulder fractures is the fact that treatment is determined not only by the fracture itself but also by the implanted prosthesis and its function. Consequently, the exact preoperative shoulder function and, in the case of an implanted anatomical prosthesis, the status and function of the rotator cuff need to b

Periprosthetic fractures of the femur.

Periprosthetic fractures of the femur represent a heterogeneous and challenging problem for the orthopedic surgeon. The incidence of these fractures is dramatically increasing, as there are more and more patients with aging total joint replacements. The fractures may occur as the result of a traumatic event, but more often are the result of minor trauma spontaneous fracture, and they are frequently associated with preexisting, sometimes neglected, problems with the associated joint replacement.

Managing periprosthetic tibia fractures: International perspectives.

Knee arthroplasty, both total knee and unicompartmental, has had a significant impact on millions of patients globally. Although satisfaction is usually high, complications such as periprosthetic fracture are increasingly common. Distal femur periprosthetic fractures are relatively well researched and understood in comparison with periprosthetic proximal tibia fractures (PTFs). The management of PTFs is essentially an evidence-free area. This review explores the literature (or lack thereof) and integrates cases from Australia and Japan. As it stands, there is scant literature relating to all f

[Principles of management of periprosthetic fractures].

BACKGROUND: The increasing numbers of primary total hip and knee replacements have subsequently led to growing rates of periprosthetic fractures. In many cases geriatric patients with osteopenia or osteoporotic bone quality are affected. The goal of treatment is the retention or reconstruction of joint function using open reduction and internal fixation or a revision prosthesis. OBJECTIVE: The aim of this article is a description of the basic principles of treatment of periprosthetic fractures of the lower extremities. MATERIAL AND METHODS: An exact description of the fracture using current

[Periprosthetic Femoral Fractures: Classification and Therapy].

Periprosthetic femoral fractures can be categorized into proximal fractures around a hip stem or distally around a knee arthroplasty. This paper focuses on the proximal periprosthetic fractures. It is important to classify the fracture at diagnosis and to perform accurate planning of the surgery. Basically it is necessary to decide if the implant is fixed or loose, in the latter case a revision surgery including the change of the endoprosthesis is neccessary. If the implant is fixed an open reduction and internal fixation can be done. All implants have to be available on site because in the co
