[Image-guided radiotherapy for gynaecological cancer].

Intensity-modulated radiation therapy reduces the dose to organs at risk in patients treated for gynecological malignancies. Given the steep dose gradients around the planning target volume, internal movement of organs during treatment may cause a planning target volume underdosage and organs at risk overdosage in case of systematic error. It is therefore important to take into account the movement of the pelvic organs and to investigate potential solutions provided by image-guided radiotherapy. A review of the literature was carried out using the following databases: Medline, Embase, Cochrane

Digital reconstructed radiography with multiple color image overlay for image-guided radiotherapy.

Registration of patient anatomical structures to the reference position is a basic part of the patient set-up procedure. Registration of anatomical structures between the site of beam entrance on the patient surface and the distal target position is particularly important. Here, to improve patient positional accuracy during set-up for particle beam treatment, we propose a new visualization methodology using digitally reconstructed radiographs (DRRs), overlaid DRRs, and evaluation of overlaid DRR images in clinical cases. The overlaid method overlays two DRR images in different colors by dividi

Dose optimization of 2D X-ray image acquisition protocols in image-guided radiotherapy.

PURPOSE: In contemporary radiotherapy, patient positioning accuracy relies on kV imaging. This study aims at optimizing planar kV image acquisition protocols regarding patient dose without degrading image quality. MATERIALS AND METHODS: An image quality test-object was placed in-between PMMA plates, suitably arranged to model head or pelvis. Constructed phantoms were imaged using default protocols, the resultant image quality was assessed and the corresponding radiation dose was measured. The process was repeated using numerous kV/mAs combinations to identify those acquisition settings provid

ESTRO technical guideline: intensity modulated radiotherapy and image guided radiotherapy for rectal cancer.

INTRODUCTION: Intensity Modulated Radiotherapy (IMRT) and Image Guided Radiotherapy (IGRT) have become an integral part of standard care for rectal cancer, but evidence-based detailed guidance is lacking to support its clinical implementation and use. This European SocieTy for Radiotherapy and Oncology (ESTRO) technical guideline aimed to assess the available evidence and provide recommendations for their use in rectal cancer treatment. MATERIALS AND METHODS: The ESTRO Lower Gastrointestinal (GI) Cancer guidelines subcommittee formed a writing panel to address key questions (KQs) on the appli

Synergistic action of image-guided radiotherapy and androgen deprivation therapy.

The combined use of androgen deprivation therapy (ADT) and image-guided radiotherapy (IGRT) can improve overall survival in aggressive, localized prostate cancer. However, owing to the adverse effects of prolonged ADT, it is imperative to identify the patients who would benefit from this combined-modality therapy relative to the use of IGRT alone. Opportunities exist for more personalized approaches in treating aggressive, locally advanced prostate cancer. Biomarkers--such as disseminated tumour cells, circulating tumour cells, genomic signatures and molecular imaging techniques--could identif

Is rectal preparation necessary in contemporary image-guided prostate radiotherapy?

INTRODUCTION: Rectal preparation during prostate radiotherapy (RT) was established in the era of two-dimensional bone-based IGRT, but its necessity with contemporary three-dimensional IGRT is uncertain. We examined the impact of omitting rectal preparation on prostate inter- and intrafraction motion, rectal volume, and their relationship. METHODS: Two datasets were analysed. Dataset one included patients receiving RT for de novo prostate cancer on a C-arm linear accelerator, stratified by use of micro-enema or no rectal preparation. Interfraction prostate motion was calculated from daily CBCT

Organ-contour-driven auto-matching algorithm in image-guided radiotherapy.

PURPOSE: This study aimed to demonstrate the potential clinical applicability of an organ-contour-driven auto-matching algorithm in image-guided radiotherapy. METHODS: This study included eleven consecutive patients with cervical cancer who underwent radiotherapy in 23 or 25 fractions. Daily and reference magnetic resonance images were converted into mesh models. A weight-based algorithm was implemented to optimize the distance between the mesh model vertices and surface of the reference model during the positioning process. Within the cost function, weight parameters were employed to priorit

Development of an integral system test for image-guided radiotherapy.

An integral system test was developed to determine the precision and accuracy of an image-guided radiotherapy system involving an x-ray volumetric imaging device mounted onto the gantry of a medical linear accelerator. The test was designed to interrogate the system components as a whole without deconstructing the individual sources of error. The integral system test was based on the imaging of an unambiguous stationary object in the treatment position and so took no account of patient related errors. An array of micromosfets interspersed within slices of a tissue equivalent phantom was develo

Image-guided adaptive interventional radiotherapy (modern brachytherapy) boost for vaginal recurrences.

BACKGROUND: This study aimed to evaluate clinical outcomes in patients with vaginal recurrences treated with radiotherapy with or without chemotherapy, followed by image-guided interventional radiotherapy (IG-IRT). METHODS: We retrospectively analyzed patients with vaginal recurrences treated with external beam radiotherapy (EBRT) ± chemotherapy followed by endovaginal high dose rate IG-IRT. The EBRT total dose was 45 Gy. The interventional radiotherapy boost delivered 28 Gy in four high-dose-rate fractions, achieving 85-95 Gy EQD2 (α/β 10) to the high-risk clinical target volume and 60 Gy EQ

Investigation the Efficacy of Fuzzy Logic Implementation at Image-Guided Radiotherapy.

At image-guided radiotherapy, technique, different imaging, and monitoring systems are utilized for (i) organs border detection and tumor delineation during the treatment planning process and (ii) patient setup and tumor localization at pretreatment step and (iii) real-time tumor motion tracking for dynamic thorax tumors during the treatment. In this study, the effect of fuzzy logic is quantitatively investigated at different steps of image-guided radiotherapy. Fuzzy logic-based models and algorithms have been implemented at three steps, and the obtained results are compared with commonly avai

Definitive radiotherapy with image-guided adaptive brachytherapy for primary vaginal cancer.

Primary vaginal cancer is a rare cancer and clinical evidence to support recommendations on its optimal management is insufficient. Because primary vaginal cancer resembles cervical cancer in many aspects, treatment strategies are mainly adopted from evidence in locally advanced cervical cancer. To date, the organ-sparing treatment of choice is definitive radiotherapy, consisting of external beam radiotherapy and brachytherapy, combined with concurrent chemotherapy. Brachytherapy is an important component of the treatment and its steep dose gradient enables the delivery of high doses of radiat

Constrained non-rigid registration for use in image-guided adaptive radiotherapy.

A constrained non-rigid registration (CNRR) algorithm for use in prostate image-guided adaptive radiotherapy is presented in a coherent mathematical framework. The registration algorithm is based on a global rigid transformation combined with a series of local injective non-rigid multi-resolution cubic B-spline Free Form Deformation (FFD) transformations. The control points of the FFD are used to non-rigidly constrain the transformation to the prostate, rectum, and bladder. As well, the control points are used to rigidly constrain the transformation to the estimated position of the pelvis, lef

Quantitative MRI on MR-Linacs: Towards Biological Image-Guided Adaptive Radiotherapy.

Recognizing the potential of quantitative imaging biomarkers (QIBs) in radiotherapy, many studies have investigated the prognostic value of quantitative MRI (qMRI). With the introduction of MRI-guided radiotherapy systems, the practical challenges of repeated imaging have been substantially reduced. Since patients are treated inside an MRI scanner, acquisition of qMRI can be done during each fraction with limited or no prolongation of the fraction duration. In this review paper, we identify the steps that need been taken to move from MR as an imaging technique to a useful biomarker for MRI-gui

Establishment of image-guided radiotherapy of orthotopic hepatocellular carcinoma mouse model.

BACKGROUND: Hepatocellular carcinoma (HCC) is the most common type of liver cancer. Recently, developments in radiotherapy technology have led to radiotherapy becoming one of the main therapeutics of HCC. Therefore, a suitable animal model for radiotherapy of the orthotopic HCC mouse model is urgently needed. METHODS: In the present study, Hepa1-6 cells were injected into the liver of C57BL/6 mice in situ to mimic the pathological characteristics of the original HCC. Tumor formation was monitored by applying magnetic resonance imaging techniques and verified by H&E histopathological staining,

Decision-making processes in image guided radiotherapy: A think aloud study.

INTRODUCTION: 3D Image Guided Radiotherapy (IGRT) using Cone Beam Computer Tomography (CBCT) has been implemented for a range of treatment sites across the UK in the last decade. A paucity of evidence exists to understand how radiation therapists (RTTs) make clinical decisions during image interpretation as part of the IGRT process. The aim of this study was to investigate the decision-making processes used by RTTs during image interpretation of IGRT. METHOD: Case study methodology was adopted utilising a think aloud observational method with follow-up interviews. 12 RTTs were observed and in

[Image-guided radiotherapy contribution and patient setup for anorectal cancer treatment].

Intensity-modulated radiation therapy is recommended in anal squamous cell carcinoma treatment and is increasingly used in rectal cancer. It adapts the dose to target volumes, with a high doses gradient. Intensity-modulated radiation therapy allows to reduce toxicity to critical normal structures and to consider dose-escalation studies or systemic treatment intensification. Image-guided radiation therapy is a warrant of quality for intensity-modulated radiation therapy, especially for successful delivery of the dose as planned. There is no recommended international or national anorectal cancer

Comment on "Is rectal preparation necessary in contemporary image-guided prostate radiotherapy?".

This correspondence comments on the study by Alexander et al. evaluating whether routine rectal preparation is necessary during contemporary image-guided prostate radiotherapy. Although the reported motion differences were clinically small and support reducing burdensome preparation, interpretation is limited by structural differences between treatment schedules, non-equivalent definitions of patient motion, possible clustering of repeated fraction-level measurements, and differing planning rescan thresholds. Enema use was associated with radiotherapy prescription, making direct comparison bet

Adaptive Target Volume and Dosimetry in Image-Guided Radiotherapy for Cervical Cancer.

Background: Cervical cancer treatment with advanced radiotherapy techniques benefits from image guidance, particularly when anatomical changes occur during therapy. This case emphasizes the need for adaptive radiotherapy when target volume shifts significantly. Methods: A 70-year-old woman with International Federation of Gynecology and Obstetrics (FIGO) IIIC2 9th edition cervical squamous cell carcinoma presented with a distended uterine cavity due to fluid accumulation. She underwent definitive chemoradiotherapy using Volumetric Modulated Arc Therapy (VMAT) and weekly cisplatin. Results: Dai

Advances in Image-Guided Radiotherapy in the Treatment of Oral Cavity Cancer.

Image-guided radiotherapy (IGRT) is an advanced auxiliary radiotherapy technique. During cancer treatment, patients with oral cavity cancer (OCC) experience not only disease but also adverse effects due to RT. IGRT provides the relevant advantages of RT by precisely delivering tumoricidal doses via real-time knowledge of the target volume location and achieves maximal tumor control with minimal complications as recommended for cancer treatment. Additionally, studies have shown that IGRT can improve clinical outcomes in terms of not only treatment side effects but also survival benefits for can
