IRB perspectives on the return of individual results from genomic research.

PURPOSE: Return of individual research results from genomic studies is a hotly debated ethical issue in genomic research. However, the perspective of key stakeholders-institutional review board (IRB) professionals-has been missing from this dialogue. This study explores the positions and experiences of IRB members and staff regarding this issue. METHODS: In-depth interviews with 31 IRB professionals at six sites across the United States. RESULTS: IRB professionals agreed that research results should be returned to research participants when results are medically actionable but only if the pa

Preferences of research participants with experience receiving individual results for future return of results.

INTRODUCTION: Research participants express strong desires to receive their individual research results (IRR), consistent with key ethical principles of research. Participants' preferences, especially when based on past experiences with IRR, can help in formulating approaches to the process. METHODS: Participants (n = 149) were recruited from a site of the eMERGE-IV study, which returned risk estimates for 8 common diseases based on polygenic scores and other factors. Most were White, highly educated, and English-speaking. Participants completed an online survey regarding their eMERGE experie

Canadian Research Ethics Board Leadership Attitudes to the Return of Genetic Research Results to Individuals and Their Families.

Genomic research may uncover results that have direct actionable benefit to the individual. An emerging debate is the degree to which researchers may have responsibility to offer results to the biological relatives of the research participant. In a companion study to one carried out in the United States, we describe the attitudes of Canadian Research Ethics Board (REB) chairs to this issue and their opinions as to the role of the REB in developing related policy.

Whole Exome Sequencing in South Africa: Stakeholder Views on Return of Individual Research Results and Incidental Findings.

The use of whole exome sequencing (WES) in medical research is increasing in South Africa (SA), raising important questions about whether and which individual genetic research results, particularly incidental findings, should be returned to patients. Whilst some commentaries and opinions related to the topic have been published in SA, there is no qualitative data on the views of professional stakeholders on this topic. Seventeen participants including clinicians, genomics researchers, and genetic counsellors (GCs) were recruited from the Western Cape in SA. Semi-structured interviews were cond

Return of individual research results from genome-wide association studies: experience of the Electronic Medical Records and Genomics (eMERGE) Network.

PURPOSE: Return of individual genetic results to research participants, including participants in archives and biorepositories, is receiving increased attention. However, few groups have deliberated on specific results or weighed deliberations against relevant local contextual factors. METHODS: The Electronic Medical Records and Genomics (eMERGE) Network, which includes five biorepositories conducting genome-wide association studies, convened a return of results oversight committee to identify potentially returnable results. Network-wide deliberations were then brought to local constituencies

Returning Individual-Level Urgent or Emergent Research Results to Participants: The Project Baseline Health Study Experience.

BACKGROUND: Returning results to research participants is increasingly recognized as an ethical mandate, yet little is known about best practices to optimally communicate urgent or emergent results. METHODS: We describe the development of and experience with a process to return results to participants in the Project Baseline Health Study (PBHS), which was a prospective observational cohort study of 2502 participants enrolled from 2017 to 2019 and followed through 2023. Urgent or emergent results were returned during or after the baseline visit from vital signs; clinical laboratory testing; an

Strategies to facilitate the report-back of research results: a systematic review of methods and evaluations.

BACKGROUND: Reporting back of research results (RBRR) is becoming a recognized component of ethical community-engaged and human subjects research. Scholars emphasize the importance of involving participants in developing reports and methods of report-back, arguing that RBRR enhances comprehension, trust, and engagement with findings. Yet, despite growing recognition, standardized guidelines for ethical RBRR remain limited. To address this gap, we conducted a systematic literature review of peer-reviewed studies- primarily in genomics, environmental health, and biomonitoring- to identify RBRR d

Leading by example: How the Webster team's systematic review quietly rebuked the field's preoccupation with biomarker "disclosure," and why it matters.

In this issue, Webster and colleagues report findings and provide direction following a systematic review and evidence synthesis designed to understand the decisional needs of research participants considering whether to receive biomarker or genetic risk information. The report serves as a foundation for a future of participant-centered approaches to the return of individual results in Alzheimer's disease research. This Ethics Response comments on the authors' terminology choices highlighting the notable absence of references to disclosure. The Response concludes that alternative terms like re

[Clinical pharmacogenetics and the use of biobanks].

The Danish strategy for personalised medicine 2025-2027 includes initiatives for improving the use of pharmacogenetic (PGx) information to guide drug therapy. While PGx is already in use, its potential to prevent side effects and improve treatment success is not fully realised. 20% of Danish adults are already genotyped, which is a powerful resource, and this review emphasises the need for improved infrastructure and data access for physicians. The experience with return of genetic results is good but must be balanced with considerations on both cost-benefit and clinical effect.

Development of a question prompt list to support Consent for Genomic Testing (CoGenT) and research.

While genomic testing is increasingly utilised to personalise cancer treatment, its complexity creates challenges in achieving informed consent. Partnering with patients, carers, researchers and health professionals, we developed a Question Prompt List (QPL) aiming to support communication and informed consent for tumour profiling and genomic research in oncology. Participants (n = 38) included cancer patients participating (or not) in genomic research, carers, researchers and clinicians. Two rounds of interviews elicited information needs and suggested questions, followed by feedback on the d

The Psychosocial Impact of Receiving Whole Genome and Whole Exome Sequencing Results in Adults: A Systematic Review.

The expanding use of whole genome sequencing (WGS) and whole exome sequencing (WES) underscores the need to better understand the psychosocial impact of receiving a broader range of potential results than those generated by other types of genetic testing. This systematic review aimed to provide a comprehensive assessment of the psychosocial impact of WGS and WES testing in adult populations. An electronic literature search was conducted to identify studies published up to July 2025. Quantitative and qualitative studies involving adult population, published in English, and examining the psychos

Participant engagement and feedback in microbiome projects: a case of AWI-Gen 2.

Returning individualized microbiome results in ways that are ethical, comprehensible, and useful remains under-explored in African settings. We nested a multi-site, mixed-methods study within the AWI-Gen Wave 2 gut microbiome sub-study of 1,801 women aged 42-86 years to engage participants and provide feedback. All (1,001) participants from Agincourt and Soweto (South Africa) and Nairobi (Kenya) were invited to feedback meetings: 496 from Agincourt, 87 from Soweto, and 195 from Nairobi responded. Engagement strategies were tailored by site (small-group and home-based sessions, visual metaphors

The MyCancerGene study: a hybrid type 1 effectiveness-implementation randomized study comparing a patient-centered digital genetic health portal to usual care after receipt of cancer genetic testing.

BACKGROUND: Genetic testing has become more complex with the transition from single gene testing to multi-gene panels and whole-exome sequencing. The adoption of more complex genetic testing and the promise of precision medicine has created a critical and urgent need for longitudinal clinical follow-up with patients who undergo cancer genetic testing given the residual uncertainty. There is an increasing need for multidisciplinary translational research that focuses on how to advance precision medicine discoveries into clinical practice and to capitalize on connectivity (e.g. digital health) i

Predictors of extreme future time perspective change in persons who learn an Alzheimer's disease biomarker test result.

OBJECTIVES: Future Time Perspective (FTP) refers to how individuals perceive their future. FTP can be impacted when people learn their Alzheimer's disease (AD) biomarker test result. This study aimed to understand what factors explain change in FTP after people learn their AD biomarker result and whether large FTP changes, after learning an AD biomarker test result, can be predicted by clinical and demographic characteristics. METHOD: Cognitively unimpaired adults (N = 4,340) screening for an AD prevention trial, completed the 10-item FTP scale pre- and post- disclosure of their amyloid test

Return of Research Results to Participants in Longitudinal Cohort Studies in Health Research: Findings From 24 Cohorts of the Amsterdam Cohort Hub.

BACKGROUND: Returning research results to participants is increasingly recognised as part of participant-centred health research. How this is implemented in longitudinal cohort studies is not yet well characterised. This study aims to provide a descriptive overview of current result return practices across diverse cohort studies in the Netherlands. METHODS: Drawing on the Amsterdam Cohort Hub, an infrastructure of population-based and patient-based cohorts in the Netherlands, representatives of 24 affiliated cohort studies completed a structured questionnaire administered via individual inter

Misaligned Expectations: Motivations of Medically Underserved People to Enroll in the All of Us Research Program.

It is important to know what motivates people, especially from groups underrepresented in biomedical research, to accept or decline participation in research studies. With this information, engagement strategies, incentives to participate, and benefits of participation can be aligned with what potential research participants value and expect from participating in research. Our project sought to identify what motivated people recruited at a Federally Qualified Health Center (FQHC) to enroll, or not, in the All of Us Research Program (AoU). Qualitative interviews revealed that the most common mo

Long-term impact of disclosing amyloid PET results to individuals with subjective cognitive decline.

BACKGROUND: Biomarker assessments increasingly inform the diagnostic evaluation and treatment decisions in Alzheimer disease (AD). However, evidence on the impact of amyloid positron emission tomography (PET) disclosure is primarily derived from studies of cognitively unimpaired trial participants with follow-up limited to 18 months. In contrast, long-term implications for individuals with cognitive concerns who seek medical evaluation in memory clinics remain unknown. We aimed to examine the psychosocial and behavioral impact three years after amyloid PET disclosure in individuals presenting
