Expressive and defensive behavior during discourse on unresolved topics: a single case study of pathological grief.

Both psychodynamic and social-cognitive theoretical domains have control process models of behavior but with different ideas about the purpose and loci of control. This study examines expressive and defensive behaviors associated with different topics of discourse in the time-limited psychotherapy of a woman treated for pathological grief. Conceptually the study is based on a model of defensive control processes that integrates states of mind and person schemas. Theoretically derived measures of discourse topics, verbal and nonverbal defensive behaviors, emotional disclosure, and states of min

Examining the impact of design-comparable effect size on the analysis of single-case design in special education.

Initially excluded from many evaluations of education research, single-case designs have recently received wider acceptance within and beyond special education. The growing approval of single-case design has coincided with an increasing departure from convention, such as the visual analysis of results, and the emphasis on effect sizes comparable with those associated with group designs. The use of design-comparable effect sizes by the What Works Clearinghouse has potential implications for the experimental literature in special education, which is largely composed of single-case designs that m

A proposed regulatory and ethical framework for the application of single-case experimental design methodology in rehabilitation research and clinical practice.

The use of single-case experimental design (SCED) methodology is growing across all domains of rehabilitation. SCED methodology may be used for several different purposes, which creates confusion about its ethical, legal and regulatory status. We will argue that aspects of SCED methodology can be helpful in clinical practice to determine the optimal treatment for a given person. Given the use of SCED methods in research, clinicians may assume that ethics committee approval is required, though this is not necessarily the case given that SCED methods may, under certain circumstances, be consider

Intervention research in anorexia nervosa - a plea for single case experimental designs.

Anorexia nervosa (AN) is a severe eating disorder and among the mental disorders with the highest mortality rate. To date, not much is known about its development and maintenance, and treatment outcomes are improvable. At the same time, research on psychological interventions for AN is a complicated endeavor, often characterized by small sample sizes and methodological heterogeneity, which impedes the comparison and evaluation of treatment outcomes. In this paper, several population-related, therapist-related and methodological challenges that may be encountered during research on psychologica

Does K-wire fixation improve outcomes in children with a Seymour fracture?

INTRODUCTION: The Seymour fracture is a juxta-epiphyseal fracture of the terminal phalanx of the finger. Sources vary on the recommended management, with some advocating treatment without K-wires to avoid metalwork-associated infection, and others suggesting that K-wire fixation is necessary due to the risks of fracture re-displacement. METHODS: A best evidence topic in paediatric hand surgery was written according to a structured protocol. Searches were performed on December 28, 2021 in Cochrane library and PubMed. RESULTS: 69 papers were found using the reported search strategy, and eight

The Integrative Single-Case Design as a Biosemiotic-Systemic Research Tool in Psychoneuroimmunology.

Psychoneuroimmunology (PNI) is a field of research that deals with the interactions between psyche, nervous, endocrine, and immune systems. Investigating these complex PNI relationships under as ecologically valid as possible conditions ("life as it is lived") necessitates a paradigm change in research. This shift places factors such as "time" and "subjective meaning" of personal experiences at the center of the research methodology. For this purpose, the biosemiotic-systemic research design "Integrative Single-Case Study" was developed. Initial results from healthy individuals as well as pati

A Neuropsychological Profile for Cerebrotendinous Xanthomatosis: A Single-Case Study.

OBJECTIVE: Cerebrotendinous xanthomatosis (CTX) belongs to a heterogeneous group of neurological disorders known as autosomal recessive cerebellar ataxias. Low awareness of CTX can result in misdiagnoses in the differential diagnostic process and may limit one's ability to offer suitable recommendations. While neurodegeneration is a recognized manifestation of CTX, there is scant literature to characterize the nature of cortical symptoms and even less detailing of its associated neurocognitive and neuropsychiatric manifestations. METHOD: Based on the lack of representation of CTX in neuropsyc

Personalized (N-of-1) Clinical Trials for Inflammatory Bowel Disease: Opportunities and Challenges.

BACKGROUND & AIMS: Heterogeneity of treatment effects in inflammatory bowel disease (IBD) means that many individuals or patient subgroups depart from the average for whom the outcomes from traditional randomized trials may not be applicable. The N-of-1 trial is a design in which a single patient is followed over time with the treatments being randomized from period to period with the intention of finding the most effective treatment for that patient. The aim was to investigate the utility of N-of-1 trials in IBD. METHODS: To identify relevant articles for this scoping review, a MEDLINE liter

A preliminary investigation of a novel method to manipulate penis length to measure female sexual satisfaction: a single-case experimental design.

OBJECTIVES: To investigate a novel methodology and explore whether artificially reducing the depth of penetration during intercourse matters to women. STUDY DESIGN AND METHODS: A study with a single-case experimental design ('n of 1'), in which a heterosexual couple act as their own control and the study is then replicated in subsequent couples, was conducted. Thirty-five couples were assessed for eligibility to participate. Twenty-nine couples without any sexual problems were randomized and 12 submitted sufficient data to analyse. As a proxy for reducing penis length, we artificially reduced

Meta-analysis of single-case design research: Application of multilevel modeling.

This study describes the benefits and challenges of meta-analyses of single-case design research using multilevel modeling. The researchers illustrate procedures for conducting meta-analyses using four-level multilevel modeling through open-source R code. The demonstration uses data from multiple-baseline or multiple-probe across-participant single-case design studies (n = 21) on word problem instruction for students with learning disabilities published between 1975 and 2023. Researchers explore changes in levels and trends between adjacent phases (baseline vs. intervention and intervention vs

DELTA: Strengthening human biological resilience with an N=1 digital health and dynamic biomarker protocol.

Societies are aging rapidly in parallel with the increasingly earlier onset of serious diseases in younger populations. These and other factors are creating a substantial disparity between healthspan, the period of life where an individual is free from serious chronic disease or disability, and lifespan - expanding the morbidity span. Extending healthspan has thus become a major priority. To pursue an integrated strategy toward healthspan support, we launched DELTA, a prospective, open-label, interventional, and participatory N = 1 study (NCT06630637) conducted on a healthy individual (DELTA00

The effectiveness of facial exercises for facial rejuvenation: a systematic review.

BACKGROUND: Although aesthetic correction of facial aging had long been the exclusive domain of plastic surgeons and dermatologists, alternative nonmedical approaches to facial rejuvenation are becoming more popular, such as facial acupuncture, facial acupressure, and facial exercises. However, the effectiveness of these alternative approaches is still a topic of debate. OBJECTIVES: The authors review the evidence of the effectiveness of facial exercises for facial rejuvenation. METHODS: A literature search was performed in Medline, Web of Science, Science Direct, SciELO, and LILACS database

Do we have serological evidences that chilblain-like lesions are related to SARS-CoV-2? A review of the literature.

The outbreak of chilblain-like lesions (CLL) coincidentally to the COVID-19 pandemic is a topic of great concern. SARS-CoV-2 was initially hypothesized as the etiologic agent of CLL, but, since nasopharyngeal swabs seldom resulted positive, dermatologists' attention focused on the search for specific SARS-CoV-2 antibodies. Many papers were published contemporarily on this topic, reporting limited case series. We reviewed the English literature up to the first July 2020 and, excluding single case reports, we considered 13 studies that serologically investigated 220 patients. The presence of spe

An N-of-1 Randomized Controlled Trial of Interventions for Children With Inconsistent Speech Sound Errors.

Purpose The aim of this study was to test the hypothesis that children with inconsistent speech errors would respond differentially to 1 of 3 specific interventions depending on their primary underlying impairment: Children with deficient motor planning were expected to respond best to an auditory-motor integration (AMI) intervention, and children with deficient phonological planning were expected to respond best to a phonological memory and planning (PMP) intervention. Method Twelve participants were diagnosed with a motor planning (n = 7) or phonological planning (n = 5) deficit based on a c

How safe and effective is prescribing oral isotretinoin to treat acne in patients on renal dialysis? A systematic review.

BACKGROUND: Patients on renal dialysis often develop severe acne. In spite of this, the literature remains scarce about the use of isotretinoin for the treatment of acne in this group of patients, and many clinicians remain apprehensive and hesitant to use it. AIM: To systematically review the literature surrounding the safety and efficacy of isotretinoin for the treatment of acne in patients on renal dialysis. METHODS: Four electronic databases (MEDLINE, Embase, CINAHL, Emcare) were systematically searched in March 2021. The search strategy incorporated the terms 'isotretinoin', 'renal', 'k

Strengthening Rural Health With N-of-1 Trials and Single-Case Experimental Designs.

THE CHALLENGE: Rural and remote clinicians frequently make treatment decisions under conditions of uncertainty, exacerbated by limited access to specialist services, fewer locally available treatment options, and evidence that may not fully inform care for an individual patient. THE OPPORTUNITY: N-of-1 trials and related single-case experimental designs (SCEDs) offer a structured method for generating individual-level evidence to support treatment decisions yet remain rarely used in routine clinical practice. Beyond informing care for individual patients, aggregated N-of-1 trials can also gen

The state-of-the-art of N-of-1 therapies and the IRDiRC N-of-1 development roadmap.

In recent years, a small number of people with rare diseases caused by unique genetic variants have been treated with therapies developed specifically for them. This pioneering field of genetic N-of-1 therapies is evolving rapidly, giving hope for the individualized treatment of people living with very rare diseases. In this Review, we outline the concept of N-of-1 individualized therapies, focusing on genetic therapies, and illustrate advances and challenges in the field using cases for which therapies have been successfully developed. We discuss why the traditional drug development and reimb

Triple and quadruple cervical artery dissections: a systematic review of individual patient data.

BACKGROUND AND PURPOSE: Simultaneous dissection of three or four cervical arteries rarely occurs. As a result, limited information is available on clinical characteristics, underlying causes, treatment, and outcome of these patients. METHODS: We performed a systematic review of individual patient data on triple and quadruple cervical artery dissection (CeAD). We included all cases for whom, at minimum, data on age, sex and affected cervical arteries were available. RESULTS: Out of 1396 publications identified in the initial search, 52 were included, with data available on 96 patients. Mean a

Lithium carbonate in the treatment of movement disorders. A critical review.

In this paper, the efficacy of lithium therapy in the treatment of tardive dyskinesia is reviewed. It is noted that most of the published papers on this topic are anecdotal and single case reports with contradictory results. The well-designed studies indicate that lithium is useful in certain tardive dyskinesia patients and that the red blood cell lithium level may be related to the therapeutic response. Therefore, on the basis of the available information, we conclude that routine clinical application of lithium in the treatment of tardive dyskinesia is not warranted. However, further evaluat

The genetics and epigenetics of fatigue.

Fatigue is a common symptom and includes both physical and mental components. It can be associated with a variety of different syndromes and diseases, but in many cases is not associated with other comorbid conditions. Most humans have experienced acute fatigue in relation to different stressors. Acute fatigue typically decreases as the effect of the triggering factor is reduced and a normal homeostatic balance is restored. Fatigue that persists for 6 months or more is termed chronic fatigue. Chronic fatigue (CF) in combination with a minimum of 4 of 8 symptoms and the absence of diseases that
