[Controlled clinical study of two viscoelastic substances].

Viscoelastic substances in correlation with intraocular hypotony have gained great significance in low-irritation cataract surgery within the last few years. The differences in their chemical properties, and thus in their effect on the eye, formed the grounds for a controlled clinical study comparing sodium hyaluronic acid, representing the group of glucosamine glykanes, and hydroxypropylmethyl cellulose (HPMC), representing the group of cellulose ethers. Two hundred cataract patients, excluding those suffering from diabetes mellitus, glaucoma and severe corneal damage, were examined on the 1s

A novel technique to facilitate dacryocystectomy using viscoelastic substances.

The procedure of dacryocystectomy is described in a patient with Wegener's granulomatosis where bone preservation is essential for future reconstructive procedures. With the aid of canalicular clamps, viscoelastic substance is used to distend the lacrimal sac thereby facilitating easier dissection. We describe the use of canalicular clamping and injection of a viscoelastic substance into the lacrimal sac to facilitate easier dissection during dacryocystectomy.

[Viscoelastic substances in cataract surgery. Principles and current overview].

The major aim for the application of viscoelastic substances in cataract surgery is prevention of corneal endothelial cell loss. This aim is fulfilled by deepening of the anterior chamber, mechanical endothelium protection against surgical trauma, absorption of ultrasound energy, and coating of intraocular lens. The first and mostly used viscoelastic in ophthalmic surgery is a 1% solution of sodium hyaluronate (NaHA) with a molecular weight of 4 million daltons, and a viscosity of 229,000 cP (Healon). For this substance, a good endothelium-protecting function in cataract surgery, especially ph

[Lamellar keratoplasty with air or viscoelastic substance injection].

PURPOSE: To analyze the results of very deep lamellar keratoplasty using dissection with air and a viscoelastic substance. METHODS: This was a prospective monocentric noncomparative study. Candidates for lamellar keratoplasty were enrolled between November 1998 and July 2000. Deep lamellar dissection was performed following air injection into the cornea to create a white emphysema of the stroma. The dissection was performed to the Descemet membrane. Whenever a large bulla was present in the recipient bed, the dissection of the deepest stromal lamellae was performed by injecting a viscoelastic

Collagel--a new viscoelastic substance for ophthalmic surgery.

A clinical trial comparing the viscoelastic properties of Collagel (human collagen) and Healon (sodium hyaluronate) was performed. Collagel was well tolerated and produced no untoward reaction in the eye. Flare cell measurement was performed during the seven day postoperative period. The initial peak and decline of flare and cell determination showed a similar course in groups treated with Collagel or Healon. A transient increase in intraocular pressure was seen. Endothelial cell loss following surgery was similar in the Collagel and Healon groups. There were no significant differences in intr

[New aspects of surgical treatment of glaucoma. Comparison of viscoelastic substances in chamber angle surgery].

The development of new viscoelastic substances opens up new perspectives for chamber angle surgery under direct view. The success of "Cyclodialysis ab interno" treating children with glaucoma is often reduced by scar formation of the cleft. Therefore four different viscoelastic substances were instilled during an animal experimental study into the cleft and compared in their effect. We used hyaluronic acid with different molecular weight (Healon and Healon GV) and cross-linked forms of hyaluronan as a fluid and a gel (Hylan fluid and Hylan gel). A cyclodialysis ab interno was performed on 14 e

Intraocular inflammation of denatured viscoelastic substance in cases of cataract extraction and lens implantation.

I present three successive pseudophakic cases that had intraocular inflammation (iritis) and bullous keratopathy presumably caused by denatured sodium hyaluronate (Healon). The denatured Healon was injected into the anterior chamber mixed with fresh Healon during routine planned extracapsular cataract extraction and intraocular lens implantation, when the cannula was reused after sterilization by disinfectants and autoclaving. A residuum of the viscoelastic substance remained inside the cannula and its nature was changed to a toxic chemical by the action of disinfectants and the sterilization
