OBJECTIVE: To explore the relationship between light truck vehicles (LTVs) and non-fatal clinical outcomes, such as injury severity and length of hospital stay, among pediatric pedestrian victims of motor vehicle collisions.

METHODS: We conducted a retrospective cross-sectional study of pediatric pedestrian and micromobility users (e.g., bicycles, skateboards, scooters) aged 0 to 18 presenting to a major pediatric trauma center in Los Angeles from 2017 to 2022. Patient data were matched to California Highway Patrol's (CHP) Statewide Integrated Traffic Records System (SWITRS) database to determine which vehicle type was involved. Motor vehicle collisions (MVC) that involved passenger cars, SUVs, pickup trucks, and minivans were included. Of these, SUV's pickup trucks, and minivans were classified as LTVs.

RESULTS: Among 134 victims, 50 were struck by LTVs. Overall, there was a trend towards worse outcomes among victims struck by LTVs, and this was more pronounced in children younger than age 14. On multivariable regression, LTVs were significantly associated with worse GCS scores (aRR 3.95, 95 %CI 1.55-10.03, p = 0.004) and increased LOS (aRR 1.42, 95 %CI 1.05-1.93, p = 0.023) compared to passenger cars. No significant associations were observed for the presence of intracranial bleeding, admission to the intensive care unit, or whether the patient required surgery.

CONCLUSION: Pediatric pedestrian victims struck by LTVs experienced worse GCS scores and longer hospital stay, with children under the age 14 being especially more vulnerable. These findings highlight the need for vehicle-specific considerations in ED triage and support public health efforts to improve child pedestrian safety.
