Functional roles of interstitial cells of Cajal in the GI tract of rats.

Interstitial cells of Cajal (ICC) are distributed through the gastrointestinal (GI) tract, but the functional role of these cells comes primarily from studies of mice. Whether the functions of ICC are similar in larger animals is largely speculative. We investigated whether the Kit mutation in Ws/Ws rats had consequences on ICC populations in the stomach, small intestine, and colon and whether loss of ICC resulted in functional defects similar to Kit mutations in mice. Immunohistochemical labeling with c-KIT or ANO1 antibodies revealed loss of intramuscular ICC (ICC-IM) and reduced myenteric I

Artificial intelligence real-time automated recognition of the gastric antrum cross-sectional area and motility rhythm via bedside ultrasound: a pilot study.

The cross-sectional area (CSA) of the gastric antrum and its motility rhythm reflects the gastrointestinal function of critically ill patients. Monitoring the CSA and motility rhythm is crucial but remains time-consuming and operator dependent. This study aimed to develop an artificial intelligence (AI) system for real-time automated recognition of the gastric antrum CSA and motility rhythm using bedside ultrasound. Gastric antrum ultrasound videos were prospectively collected from West China Hospital to establish training and validation datasets. The AI system's predictions were validated aga

Deer velvet powder-induced antral stricture mimicking infantile hypertrophic pyloric stenosis in a 3-month-old infant: a case report.

Antral stricture is a rare cause of gastric outlet obstruction in infants, typically linked to infantile hypertrophic pyloric stenosis (IHPS). We report a case of a 3-month-old male with a 1.5-month history of progressively worsening, nonbilious vomiting. Although initial imaging suggested IHPS, intraoperative findings revealed a sealed antral perforation with dense adhesions, leading to a distal gastrectomy with Billroth I reconstruction. A detailed retrospective history disclosed that the infant had been given deer velvet powder, suspected to have contributed to the perforation and subsequen

Advancing care in malignant gastric outlet obstruction: a contemporary review of management strategies.

PURPOSE OF REVIEW: The aim of this review is to summarize the most pertinent data on management of malignant gastric outlet obstruction (MGOO), with an emphasis on recent updates. RECENT FINDINGS: Traditionally, options to restore luminal patency in patients with MGOO were surgical gastrojejunostomy (SGJ) and endoscopic insertion of an enteral stent. The latter was reserved for patients with poor performance status or anticipated survival less than 2 months. Endoscopic gastroenterostomy (EUS-GE) is a newer technique that aims to mimic a SGJ and involves placement of a lumen apposing metal ste

Ectopic Insulinoma in the Pyloric Antrum Portion of the Stomach: A Case Report and Review of the Associated Diagnostic Challenges.

Ectopic insulinomas are rare neuroendocrine tumors that present with diagnostic challenges because of their atypical location. We report the case of a 64-year-old woman with recurrent hypoglycemia caused by an insulinoma originating from the gastric pyloric antrum. The diagnosis was confirmed through clinical assessment, biochemical testing, imaging studies, and a histopathological examination. Laparoscopic distal gastrectomy with D1 lymphadenectomy was performed to resolve the hypoglycemic episodes. This case highlights the importance of considering ectopic insulinomas in the differential dia

Comparing the effect of three levels of end-expiratory pressure during facemask ventilation on gastric insufflation in patients with obesity: a randomized controlled trial.

BACKGROUND: We compared the effect of three levels of end-expiratory pressure on the incidence of gastric insufflation during face-mask ventilation in patients with obesity. METHODS: This randomized controlled trial included adult obese patients undergoing elective non-cardiac surgery under general anesthesia with neuromuscular blockade. Patients were randomized to receive either zero-end-expiratory pressure (ZEEP group), 4-cmH2O positive end-expiratory pressure (PEEP) (low-PEEP group), or 8-cmH2O PEEP (high-PEEP group) during volume-controlled mask ventilation. A Novel Therapeutic Thin Endoscope Facilitates Gastric Endoscopic Submucosal Dissection in the Cardia and Pylorus: A Case Series Study (With Video).

Endoscopic submucosal dissection (ESD) is an established technique for treating superficial gastric neoplasms. However, performing ESD in the cardia and pylorus remains technically challenging due to the narrow lumens and steep angulations. Recently, a novel therapeutic thin endoscope, featuring a 7.9-mm outer diameter, 160° downward angulation, 3.2-mm working channel, and water jet function, was developed. We aimed to evaluate the feasibility of this novel endoscope for ESD in the cardia and pylorus. We retrospectively reviewed the data from gastric ESD cases in the cardia and pylorus using t

Case Report: Ultrasonographic and computed tomographic features of presumed gastric plasmacytoma (plasma cell tumor) with ulceration in two dogs.

Gastric plasmacytoma is rare in dogs, with only four previous reports providing limited descriptions of abdominal ultrasound (AUS) and computed tomography (CT) findings. This study presents the AUS and CT imaging features of two histopathologically suspected gastric plasmacytomas. Combining the four previous and two new cases, these tumors predominantly occurred in the greater curvature and pyloric antrum as solitary round masses connected to the submucosal or transmural layers. Pre- and post-contrast tissue attenuation (Hounsfield unit; HU) differences exceeded 40, with variable AUS echogenic

Human gastroids to model regional patterning in early stomach development.

The human stomach features distinct, regionalized functionalities along the anterior-posterior axis1,2. Historically, studies on stomach patterning have used animal models to identify the underlying principles3-7. Recently, human pluripotent stem (hPS)-cell-based gastric organoids for modelling domain-specific development of the fundic and antral epithelium are emerging8-10. However, recapitulating self-organized fundic-antral patterning in early stomach organogenesis remains challenging, presenting a considerable barrier for advancing knowledge of stomach organogenesis. Here we report human g

Region-specific genomic variation and functional divergence of Helicobacter pylori clinical isolates from the gastric antrum and corpus.

The genetic variability of Helicobacter pylori contributes to differences in the severity of gastrointestinal diseases. Within the stomach, H. pylori exhibits diverse strain patterns and genetic variations that enable the evolution of new virulence factors, development of antibiotic resistance, and evasion of the host immune system. However, the comprehensive analysis of whole-genome sequences and their functional impact on gastric epithelial cells remains limited. In this study, we performed whole-genome sequencing, de novo assembly, and comparative genomic analysis on two pairs of H. pylori

High-grade pancreatic intraepithelial neoplasia occurring in heterotopic pancreas of the gastric antrum: A case report.

RATIONALE: Heterotopic pancreas (HP) is a rare congenital developmental anomaly in which pancreatic tissue appears in a location other than the normal anatomical location of the pancreas. Most cases are no specific symptoms before surgery, and the pathological features are mostly benign, while malignant cases are rarely reported. HP with Pancreatic Intraepithelial Neoplasia (PanIN) is particularly rare in the literature. At present, the treatment of HP is controversial, especially when the lesion lacks obvious malignant characteristics, making it difficult to decide on surgical intervention.

Predictive Value of Preoperative Ultrasound for Gastric Wall Thickness in Bariatric Procedures.

BACKGROUND AND OBJECTIVES: Staple line leakage is a major complication after bariatric surgery, often requiring prolonged and complex treatment. While ultrasonography has been used for gastric cancer screening, its role in evaluating gastric wall before bariatric surgery is not well studied. This study aimed to measure antral gastric wall thickness preoperatively using ultrasonography to provide objective data that may guide stapler selection and reduce complication rates. METHODS: Between December 2024 and May 2025, 42 bariatric surgery patients underwent preoperative ultrasonography to meas
