[Pediatric Thyroiditis Secondary to Fourth Branchial Arch Anomaly: A Case Series].

INTRODUCTION: A fourth branchial cleft remnant is relatively rare and often misdiagnosed, primarily presenting as a sinus with an opening in the pyriform sinus, known as pyriform sinus fistula. Confirmation of the diagnosis is achieved through either a barium swallow test, or direct rigid laryngoscopy. We present our experience in diagnosing this anomaly. BACKGROUND: The series comprised 6 children, 4 boys and 2 girls 1-13 years of age, all with left cervical infection. Five patients presented with a primary episode of cervical infection, while in one case, it was a second infection. Diagnosi

Rhinoscleroma Masquerading as a Nasal Mass: A Diagnostic Challenge in a Young Adult.

Rhinoscleroma is a rare, chronic granulomatous disease of the upper respiratory tract caused by Klebsiella rhinoscleromatis. Although uncommon, it continues to persist in developing regions and often presents a diagnostic challenge due to its indolent course, nonspecific clinical features, and resemblance to other granulomatous or neoplastic conditions. We report the case of a 23-year-old male who presented with progressively worsening bilateral nasal obstruction, intermittent epistaxis, and persistent mucopurulent nasal discharge over a duration of six months. Endoscopic evaluation revealed m

Intranasal ectopic tooth: An unusual diagnostic finding with review of literature.

Intranasal ectopic teeth are rare developmental anomalies characterized by the eruption of a tooth within the nasal cavity and may present with nonspecific symptoms that can delay diagnosis. They are often associated with complications such as chronic infection, rhinolith formation, and recurrent sinonasal symptoms. We report the case of a 45-year-old man who presented with a 6-month history of persistent foul-smelling right-sided nasal discharge and intermittent epistaxis. Nasal endoscopy revealed a hard white mass on the floor of the right nasal cavity surrounded by calcified deposits, while

Celiac Disease Emerging After ASCT in Multiple Myeloma: A Rare Diagnostic Intersection and Literature Review.

BackgroundAutologous stem cell transplantation (ASCT) is a standard component of therapy for eligible patients with multiple myeloma. Persistent gastrointestinal symptoms after ASCT are commonly attributed to infection, medication-related toxicity, or disease relapse, whereas autoimmune enteropathies such as celiac disease are rarely considered.Case presentationWe report a 53-year-old woman with lambda light-chain multiple myeloma who achieved complete remission after bortezomib, lenalidomide, and dexamethasone induction followed by ASCT and lenalidomide maintenance. Four months after transpla

Full-Thickness Regeneration of the Hard Palate Using Bioengineered Mucoperiosteal Scaffolds in a Porcine Model.

Cleft palate is a congenital anomaly that causes functional and esthetic challenges, and the hard palate is essential for feeding, speech, and separation of the oral and nasal cavities. Yet, current reconstructive techniques do not restore its bony component. Building on previous in vitro work showing that decellularized palatal mucoperiosteum, microperforated with Quantum Molecular Resonance (QMR®) technology and recellularized with mesenchymal stem cells, preserves the collagen microenvironment, supports engraftment, and shows osteoinductive potential, this study evaluated the early feasibil

Robotic Splenectomy for Refractory Immune Thrombocytopenic Purpura.

Splenectomy is a well-established surgical option for selected patients with immune thrombocytopenic purpura and persistent thrombocytopenia despite medical therapy. Although laparoscopic splenectomy remains the standard minimally invasive approach, robotic platforms may offer advantages in technically challenging cases. We present a 40-year-old woman with a 4-year history of immune thrombocytopenic purpura, obesity, Hashimoto thyroiditis, and prior sleeve gastrectomy presented with severe thrombocytopenia and abnormal uterine bleeding. After failure of medical therapy, she underwent robotic s

Soft palate leukoplakia as an endoscopic finding associated with esophageal squamous cell carcinoma.

BACKGROUND: We previously reported that specific endoscopic soft palate findings, including a whitish epithelium, were associated with esophageal squamous cell carcinoma (ESCC) history. In the present post hoc analysis of the same cohort, we re-evaluated endoscopic images to identify lesions corresponding to leukoplakia and further characterize their endoscopic features and clinical relevance in relation to ESCC. METHODS: This single-center retrospective study included 284 patients who underwent upper gastrointestinal endoscopy at the Osaka International Cancer Institute between January and M

Endoscopic Endonasal Transclival Approach to Pontine Cavernous Malformations in the Pediatric Population.

BACKGROUND AND OBJECTIVES: Brainstem cavernous malformations (CMs) carry high hemorrhage risk and morbidity due to their eloquent location. Although modern microsurgical approaches emphasize safe entry zones, endoscopic endonasal transclival approaches (EETA) have emerged as a less invasive midline corridor for ventral pontine lesions, potentially reducing brainstem manipulation. However, pediatric experience remains limited. We evaluated the safety, feasibility, and early outcomes of EETA for pontine CMs in children and characterized anatomic and technical considerations unique to this popula

Plummer-Vinson syndrome in a 10-year-old boy from Côte d'Ivoire: An exceptional paediatric case with African context.

Plummer-Vinson syndrome (PVS) is characterised by the triad of dysphagia, iron-deficiency anaemia, and proximal oesophageal webs. While well described in adults, paediatric cases remain exceptionally rare, particularly in sub-Saharan Africa. We report a 10-year-old boy from Côte d'Ivoire, presenting with progressive dysphagia of 3 years' duration and severe iron-deficiency anaemia (haemoglobin 6.0 g/dL) with neither haemoglobinopathy nor ova or parasites. Upper gastrointestinal endoscopy demonstrated a semi-circumferential post-cricoid oesophageal web; with normal surrounding mucosa and histol

Multidisciplinary diagnosis and treatment of lingual thyroid presenting as hematemesis: A case report.

RATIONALE: Hematemesis is a common symptom of gastrointestinal disorders and is often associated with upper gastrointestinal issues. However, lingual thyroid, a rare ectopic thyroid condition, can also present with hematemesis, complicating diagnosis and increasing the risk of misdiagnosis. This paper reports a case of lingual thyroid in which the primary symptom was hematemesis. PATIENT CONCERNS: A 59-year-old male patient with dwarfism and congenital hypothyroidism underwent irregular thyroid hormone therapy. He was admitted to the hospital after experiencing hemoptysis and a sensation of a

Interstitial cystitis as the initial presentation of primary Sjogren's syndrome: A case report.

Interstitial Cystitis, mostly affecting middle-aged women, has been rarely associated with Sjögren's syndrome. We report a 51-year-old woman who presented with painful micturition, pollakiuria, lower abdominal pain, and urinary urgency. In addition, the patient exhibited xerostomia, keratoconjunctivitis sicca, and leukopenia. The cystoscopy and pathological examinations confirmed the diagnosis of interstitial cystitis. Subsequent testing revealed a positive antinuclear antibody, leading to referral to the department of Rheumatology and Immunology for a labial gland biopsy, which resulted in a

Concurrent amebic and cytomegalovirus colitis in immunocompromised patients: two case reports and literature review.

Immunosuppression from human immunodeficiency virus (HIV) or other comorbidities increases susceptibility to gastrointestinal infections such as Entamoeba histolytica and cytomegalovirus (CMV). Although each pathogen can independently cause severe colitis, co-infection is rare and may lead to rapid clinical deterioration if not recognized early. We report two cases of concurrent amebic and CMV colitis in immunocompromised patients. The first patient, a 56-year-old man with HIV, Kaposi sarcoma, and hepatitis C, presented with bloody diarrhea and hypotension. Colonoscopy revealed pancolitis with
