The medical physicist's role in the artificial intelligence era: an updated EFOMP curricular and professional framework.

PURPOSE: Five years after the European Federation of Organizations for Medical Physics (EFOMP) published its first curricular framework for artificial intelligence (AI) in medical physics, the updated core curricula for radiology, radiotherapy and nuclear medicine have incorporated AI as a core competency rather than an addendum, a change consistent with the framework's principal recommendation. This paper examines the resulting educational landscape and defines the next stage of AI competency for the profession. MATERIALS AND METHODS: Focus Group 3 of the EFOMP Artificial Intelligence Specia

Correlation of size specific dose estimates with organ dose in abdominal spectral computed tomography.

BACKGROUND: In abdominal spectral Computed Tomography (CT), organ dose assessment is important for radiation safety but remains challenging due to the complexity of Monte Carlo (MC) simulations. PURPOSE: This study aimed to compare the correlations of three size-specific dose estimate (SSDE) metrics (including the central slice SSDEcenter, the mean of all slices SSDEmean, and the organ-specific SSDEorgan) with organ doses and to establish a rapid estimation method for organ dose using linear regression in abdominal spectral CT. METHODS: Monte Carlo N-Particle (MCNP6) was used to model the Ge

Stereotactic Ablative Body Radiotherapy Versus Microwave Ablation for the Treatment of Early-Stage Hepatocellular Carcinoma.

BACKGROUND: Stereotactic ablative body radiotherapy (SABR) shows promise as curative therapy for early-stage hepatocellular carcinoma (HCC). We retrospectively assessed the efficacy of SABR and microwave ablation (MWA) in a multi-centre Australian cohort, comparing local recurrence (LR), mortality, and toxicity. METHODS: This was a retrospective cohort study of 215 early stage HCC tumours treated with SABR across 4 referral centres. Cohort comparison was made to single centre data of MWA in 121 tumours. RESULTS: The main aetiologies of liver disease were hepatitis C and alcohol. The SABR coh

Optimizing photon counting detector CT reconstructions for automatic vessel tracing in prostate artery embolization.

BACKGROUND: Prostate artery embolization (PAE) requires precise visualization of complex pelvic arterial anatomy and can be aided by advanced vessel navigation tools. Photon counting detector (PCD) CT provides simultaneous ultra-high resolution and material-specific imaging, potentially improving automatic vessel tracing workflow for procedural planning and guidance. PURPOSE: To identify, within a PCD-CT workflow, the reconstruction that provides the most robust and consistent automatic vessel tracing for pre-procedural PAE planning. METHODS: Ten patients underwent PCD-CT angiography of the

Evaluation of registration accuracy using short-arc cone-beam computed tomography for image-guided radiotherapy in breast cancer.

BACKGROUND: Image-guided radiotherapy (IGRT) using cone-beam computed tomography (CBCT) enables highly accurate imaging registration; however, increased radiation exposure remains a concern. Recently, short-arc CBCT (SA-CBCT), which reduces projection data and reconstructs images, has become available. Although previous studies have shown that image quality deteriorates when projection data are reduced, no study has directly evaluated the clinical usefulness of SA-CBCT or its effect on registration accuracy. PURPOSE: We verified the registration accuracy of SA-CBCT in breast tangential irradi

Interventional radiology in bone metastases: Current concepts and perspectives.

The skeleton is a frequent site of metastatic spread, leading to skeletal-related events (SREs) such as pathological fractures, spinal cord compression, and refractory pain that profoundly impact patient survival and quality of life. Traditionally managed by a triad of radiotherapy, surgery, and systemic treatments, the landscape of bone metastasis care has undergone a major paradigm shift. Interventional radiology (IR) has emerged as the "fourth pillar" of oncology, evolving from a palliative last-resort tool into a proactive specialty integrated into precision medicine and multidisciplinary

Comparative Accuracy of 18-Gauge versus 20-Gauge Ultrasound-Guided Fine-Needle Aspiration in the Detection of Persistent Lymph Node Metastasis in Head and Neck Squamous Cell Carcinoma after Chemoradiation.

BACKGROUND AND PURPOSE: Viable malignant disease is uncommon in patients with persistent adenopathy after definitive chemoradiotherapy (CRT) for head and neck squamous cell carcinoma (HNSCC). Preoperative ultrasound-guided fine-needle aspiration (USFNA) can prevent unnecessary neck dissection. Postradiation fibrosis can complicate the approach, resulting in an inadequate aspirate volume when using standard 20- or 22-gauge needles. We assessed the comparative 18- and 20-gauge diagnostic accuracy of USFNA in detecting persistent viable nodal malignancy in patients with HNSCC with nodal metastasi

Amide proton transfer-weighted (APTw) CEST MRI in clinical routine for single time point diagnosis of pseudoprogression in IDH-wildtype glioblastoma.

BACKGROUND: Differentiating progressive disease (PD) from treatment-related effects (TRE) in glioblastoma remains challenging, particularly at single time point evaluations. TRE can occur at any disease stage, and its underlying biology is poorly understood. This study evaluates the clinical feasibility and diagnostic performance of amide proton transfer-weighted (APTw) MRI in this challenge. METHODS: Following the integration of APTw MRI into the routine clinical workflow for brain tumor imaging, we screened a total of 870 scans from 626 patients. APTw signal (voxel-based measurement) was au
