Proposal for Risk-Adjusted Surveillance Strategies in Extrahepatic Bile Duct Cancer with Cost-Effectiveness Validation: A Real-World Multi-institutional Cohort Analysis.

PURPOSE: We developed a risk-adjusted surveillance strategy incorporating prognostic factors for disease-free survival (DFS) in patients with extrahepatic bile duct cancer after curative resection and lymph node dissection. METHODS AND MATERIALS: After ethical approval with informed consent waived, this retrospective analysis included 1,443 patients with extrahepatic bile duct cancer treated between 2000 and 2015 (median follow-up, 35.9 months). DFS, defined as the time from surgery to recurrence or death, was analyzed using multivariate Cox proportional hazards models to identify prognostic

Understanding sub-cellular mechanochemical correlations of the neonatal extrahepatic bile duct extracellular matrix by atomic force microscope-Infrared spectroscopy.

BACKGROUND: The neonatal extrahepatic bile duct (EHBD) is highly susceptible to injury and fibrotic remodeling, yet the sub-cellular mechanochemical basis of this vulnerability remains poorly understood. Atomic force microscopy coupled with infrared spectroscopy (AFM-IR) enables simultaneous mapping of stiffness and molecular composition at sub-micron resolution, providing new insight into extracellular matrix (ECM) heterogeneity. Here, we investigate how spatial variation in ECM structure and mechanics differs between neonatal and adult rat EHBDs to determine the mechanochemical features that

Perihilar Biliary Neuroendocrine Carcinoma Treated With Perioperative Chemotherapy and Radical Liver Surgery: A Thought-Provoking Case.

BACKGROUND Biliary neuroendocrine neoplasm (NEN) arising from the extrahepatic bile duct is rare, with an incidence of 0.2% among gastroentero-hepatopancreatobiliary NEN cases. Biliary neuroendocrine carcinoma (NEC) is an extremely rare high-grade malignancy that requires multidisciplinary treatment, including surgery, chemotherapy, and radiation. However, there have been only a few reports on the combined therapy for biliary NEC. CASE REPORT A 74-year-old man was referred to our hospital with a chief concern of obstructive jaundice. He was diagnosed with perihilar biliary NEC. The tumor prima

Feasibility of transumbilical and prior caesarean scar access in single-incision laparoscopic common bile duct exploration for extrahepatic bile duct stones.

Open common bile duct exploration (CBDE) for extrahepatic stones results in prominent abdominal scarring and substantial tissue trauma. Laparoscopic exploration offers minimal invasiveness and faster recovery and has proven superior to staged procedures; however, single-incision laparoscopic exploration remains inadequately studied. We retrospectively analysed data from 18 patients who underwent single-incision laparoscopic common bile duct exploration (SIL-CBDE) for extrahepatic bile duct stones between March 2022 and June 2024 in the Department of General Surgery at the 901st Hospital of the

Extrahepatic bile duct on MRCP: Does incidentally detected dilation in the absence of signs or symptoms of obstruction require further investigation?

OBJECTIVE: Dilation of the extrahepatic bile duct (EHD) on imaging, which includes the common hepatic and bile ducts, is often assumed to be due to obstruction. The purpose of this study was to determine if EHD diameter differs between patients with and without active EHD obstruction. METHODS: All patients ≥ 18 years who underwent MRCP 2017-2024 were eligible. Patients with EHD dilatation due to a known etiology were excluded. EHD diameter was measured from inner-wall to inner-wall perpendicular to the long axis of the duct by two investigators independently, then averaged. Biliary obstructio

Early-Onset Digestive System Cancers: Risk Factors and Clinicopathological and Molecular Features Across Organ Sites.

The incidence of early-onset cancers, commonly defined as cancers diagnosed before age 50 years, has been increasing globally over recent decades. In particular, the incidence of several early-onset digestive system cancers, including cancers of the esophagus, stomach, colorectum, liver, extrahepatic bile duct, gallbladder, and pancreas, has been reported to be increasing in multiple regions. To elucidate carcinogenic mechanisms and develop effective prevention, earlier detection, and treatment strategies, further evidence is needed on risk factors and clinical, pathological, and molecular cha

Retroperitoneal-First Laparoscopic Approach (Retlap) for Bile Duct Resection in Pancreaticobiliary Maljunction (PBM): A Novel and Reasonable Approach to Intrapancreatic Bile Duct.

BACKGROUND: Pancreaticobiliary maljunction (PBM) is a congenital anomaly in which the pancreatic and bile ducts join outside the duodenal wall.1,2 Reflux of pancreatic juice induces chronic inflammation and epithelial regeneration, promoting carcinogenesis. Biliary dilation increases the risk of cholangiocarcinoma, and prophylactic resection of the extrahepatic bile duct and gallbladder is recommended even for asymptomatic patients.3,4 To prevent postoperative carcinogenesis, the intrapancreatic bile duct should be transected just above the pancreatobiliary junction. The authors describe a nov

The rationale against a "mandatory" extended hepatectomy in perihilar cholangiocarcinoma: meta-analysis.

INTRODUCTION: Traditional surgical teaching includes an extended hepatectomy for patients diagnosed with perihilar cholangiocarcinoma (pCCA). However, recent studies suggest it may not consistently lead to the best outcome. METHODS: A systematic review was conducted in PubMed, Embase, and Web of Science databases (inception - March 2025). Studies including at least 50 patients with pCCA who underwent liver and extrahepatic bile duct resection with caudate lobe were included. For each study, resections were reclassified as "Reference Surgery" (RS) or "Extended Surgery" (ES) according to the Ka

Eosinophilic cholangitis misdiagnosed as cholangiocarcinoma: a case report.

Eosinophilic cholangitis (EC) has a low clinical incidence, and cases with definitive pathological confirmation are extremely rare. Due to the lack of specificity in clinical and radiologic features, EC is frequently misdiagnosed as cholangiocarcinoma. We report a 65-year-old man admitted with progressive jaundice and persistent dull right upper-quadrant abdominal pain for more than 1 month. Laboratory tests showed a markedly elevated peripheral blood eosinophil count, significantly elevated bilirubin levels, and elevated tumor markers. MRI revealed intra- and extrahepatic bile duct dilatation

27-Hydroxycholesterol in bile duct tissue promotes cholangiocarcinoma progression through estrogen receptor signaling.

BACKGROUND: The oxysterol 27-hydroxycholesterol (27-HC) is widely produced in human tissues, functions as a selective estrogen receptor modulator (SERM), is implicated in the progression of estrogen receptor (ER)-positive cancers. While 27-HC is abundant in bile, its SERM role in extrahepatic cholangiocarcinoma (eCCA) remains unclear. Therefore, this study aims to test the hypothesis that 27-HC promotes eCCA cell proliferation via ER activation. METHODS: Oxysterol levels and the expression of ERs and proliferation-related genes were compared between eCCA tissues (N = 17) and noncancerous extr

Rapid, intense pericholedochal fibrosis after preoperative biliary drainage: A prospective histologic study in pancreatoduodenectomy specimens.

BACKGROUNDS/AIMS: Preoperative biliary drainage (PBD) is commonly used prior to pancreatoduodenectomy (PD), but its histologic effects on the extrahepatic bile duct are not well understood in humans. This study aimed to prospectively measure pericholedochal fibrosis (PCF) in PD specimens after plastic biliary stenting to assess its extent and clinical significance. METHODS: Consecutive patients undergoing PD were divided into two groups: those who received PBD (n = 22) and non-drained controls (n = 24). Patients who had neoadjuvant chemotherapy were excluded to focus on stent-related effects.

Complete Transection of the Common Bile Duct Caused by Blunt Abdominal Trauma: A Rare Case Report.

Common bile duct (CBD) injury after blunt abdominal trauma is rare and difficult to diagnose. Delayed recognition leads to severe morbidity. A 70-year-old Japanese man was admitted after sustaining blunt abdominal trauma. Ultrasonography revealed intra-abdominal fluid, suggesting bleeding. Contrast-enhanced computed tomography revealed pancreatic head injury, intra-abdominal bleeding, and pseudoaneurysm of the anterior superior pancreatoduodenal artery (ASPDA). Bile duct injury was not evident. The application of transarterial embolization (TAE) controlled the bleeding. Canulation into the pan

Treatment evolution and stage-specific survival in extrahepatic cholangiocarcinoma: a SEER study.

INTRODUCTION: Whether survival in extrahepatic cholangiocarcinoma (eCCA) has improved over time, and whether any gains reflect earlier stage at diagnosis or evolving treatment strategies, remains unclear. We evaluated temporal changes in stage, treatment patterns, overall survival (OS), and cancer-specific mortality (CSM) in eCCA. MATERIALS AND METHODS: Patients with primary extrahepatic bile duct cancer (C24.0), malignant behavior, and histology codes 8140/3 or 8160/3 diagnosed in the SEER database from 2004 to 2022 were included. Diagnostic year was grouped into 2004-2008, 2009-2013, 2014-2
