Obesity Paradox and Lung Cancer Mortality: The Contributing Roles of Airflow Limitation and Pre-Chronic Obstructive Pulmonary Disease.

RATIONALE: Increased body mass index (BMI; measured in kilograms per square meter) has been consistently associated with reduced mortality from lung cancer (LC), relative to low BMI, and termed the "obesity paradox." Although the basis of the obesity paradox remains unknown, mediating effects from sex, smoking status, diabetes mellitus (DM), and methodological issues (including bias) have been suggested causes. OBJECTIVE: Our aim was to examine whether respiratory comorbidity may contribute to this paradox. Methods: In this secondary analysis of 18,463 high-risk participants in the National L

Nonlinear Association Between Uric Acid to High-Density Lipoprotein Cholesterol Ratio and Diabetic Kidney Disease: Evidence from NHANES 2007-2018.

INTRODUCTION: The relationship between uric acid to high-density cholesterol ratio (UHR) and diabetic kidney disease (DKD) is unclear. The aim of this study was to investigate the relationship between UHR and DKD in patients with diabetes mellitus (DM). METHODS: Analyses were conducted based on National Health and Nutrition Examination Survey (NHANES) data from the 2007-2018 cycle. We used multifactorial logistic regression to analyze the association between UHR and DKD. We performed restricted cubic splines (RCS) analyses to explore whether there was a nonlinear relationship between the two.

Handgrip Strength and Trajectories of Preclinical Obesity Progression: A Multistate Model Analysis Using the UK Biobank.

PURPOSE: Grip strength has been increasingly recognized as a predictor of chronic disease risk and mortality. The aim of our study was to investigate the association of grip strength and the trajectories of preclinical obesity progression. METHODS: Data were collected from 93 275 participants in the UK Biobank. Preclinical obesity was diagnosed based on an excess of anthropometric parameters, defined as elevated body mass index combined with at least 1 abnormal measure among waist circumference, waist-to-hip ratio, waist-to-height ratio, or percentage body fat, in the absence of obesity-induc

GLP-1 Receptor Agonists and Blood Pressure: A State-of-the-Art Review of Mechanisms, Evidence, and Clinical Implications.

BACKGROUND: Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) are widely used for the treatment of type 2 diabetes and, more recently, for weight management among individuals without diabetes. AIM: This review synthesizes the current evidence on the mechanisms by which GLP-1 RAs affect BP, their clinical effects across populations, and the implications for patient care. We discuss subpopulations who may benefit from their BP-lowering effects, identify limitations in the existing evidence, and explore future directions for research. RESULTS: Beyond their metabolic effects, growing evidenc

Obstructive Sleep Apnea, Resting Heart Rate, and Glycemic Variability in Adults With Maturity-Onset Diabetes of the Young.

UNLABELLED: Obstructive sleep apnea (OSA) is a common condition strongly linked to increased cardiovascular risk and poor glycemic control. Little is known about OSA, cardiovascular risk, and glycemia in maturity-onset diabetes of the young (MODY), an inherited form of diabetes, which is different than both type 1 and type 2 diabetes. We assessed OSA, resting heart rate (RHR), an important prognostic marker of cardiovascular disease, and glycemic variability among the most common subtypes of MODY, glucokinase (GCK)-MODY, and transcription factor (TF)-related MODY (HNF1A, HNF4A, and HNF1B). Capturing metabolic syndrome: new thresholds for insulin resistance and novel body composition indices.

BACKGROUND: Despite being a defining feature of metabolic syndrome (MetS), clinical assessment of IR remains challenging, due to the costs of reference methods and the numerosity of IR indices. Furthermore, to which extent IR contributes to MetS, while controlling for altered body composition, is still largely unexplored. OBJECTIVES: The present work aims at proposing new cut points for IR among people with overweight and obesity, assessing the concordance of different IR definitions and investigating how IR interacts with body composition in predicting MetS status. SUBJECTS: 665 patients we

Mismatch between intact electrical excitability and lost heat pain in diabetic neuropathy.

Patterns of sensory involvement in diabetic neuropathy vary between studies and diagnostic approaches. Although some report early thermal deficits, others find predominant large-fiber changes, and hypersensitivity in early disease is inconsistently observed. Elevated heat pain thresholds (HPTs) may indicate either selective loss of heat transduction or advanced peripheral denervation of polymodal nociceptors. We examined whether thermal and mechanical pain functions align with psychophysical axonal excitability by combining German Research Network on Neuropathic Pain-quantitative sensory testi

BACKGROUND: The prevalence of diabetes mellitus is increasing in sub-Saharan Africa. Data on the prevalence of diabetes in Zimbabwe are scarce, and the etiologic types of diabetes are not well characterized. Classification of diabetes in Zimbabwe relies on clinical criteria at the time of diagnosis, and more detailed phenotype data are lacking. Furthermore, the prevalence of complications at diagnosis of diabetes and the incidence of complications during follow-up are not well documented in Zimbabwe. OBJECTIVE: The primary aim of this study is to characterize the etiological types of diabetes

Association between the insulin resistance indices and incident type 2 diabetes across different body mass index states: a cohort study and external validation from two East Asian populations.

BACKGROUND: Insulin resistance (IR) indices like the TyG index are predictors of type 2 diabetes (T2DM), but their comparative performance across BMI categories in East Asians is unclear. METHODS: This retrospective cohort study enrolled 114,293 Chinese adults without diabetes. Four IR indices (TyG, TyG-BMI, TG/HDL-C, METS-IR) were calculated. Their associations with incident T2DM were assessed using Cox models, restricted cubic splines, and machine learning, stratified by BMI. Findings were replicated in 15,453 Japanese adults from the NAGALA cohort. RESULTS: Over a mean 3.10-year follow-up

Corticosteroid Use and Atrial Fibrillation: Exploring Underlying Mechanisms, FAERS Disproportionality Analysis, and a Meta-Analysis of Clinical Studies.

Steroids are known to have proarrhythmogenic properties, even though they have been demonstrated to lower the incidence of atrial fibrillation following heart surgery. According to the American Heart Association (AHA), this negative impact was still inconsistent. Therefore, this study aims to elucidate the association between corticosteroid usage and atrial fibrillation. This systematic review employed the keywords "corticosteroids" and "atrial fibrillation," collected from PubMed, Scopus, Web of Science, and Google Scholar. Meta-analysis was performed using a random effects model through Mante
