BACKGROUND AND HYPOTHESIS: UK guidance recommends targeted screening for latent tuberculosis infection (LTBI) in kidney transplant candidates based on epidemiological or clinical risk factors, whereas international guidelines favor systematic screening. We aimed to determine whether a targeted screening strategy would miss cases compared with systematic interferon-gamma release assay (IGRA) screening in a UK kidney transplant cohort.
METHODS: We conducted a single-center retrospective cohort study of adult kidney transplant candidates assessed at the Royal Liverpool University Hospital betwee

INTRODUCTION: Detecting latent tuberculosis infection (LTBI) is becoming more important in cases where the risk of disease progression is high and where anti-tuberculosis (TB) treatment is necessary. The aim of this study is to find out the immunological evidence of LTBI in terms of release of interferon-gamma (IFN-γ), using interferon-gamma release assays (IGRAs) in contacts with the index TB case and without contact individuals.
METHODS: This cross-sectional study was conducted from March 2023 to January 2024. Participants were recruited according to the inclusion and exclusion criteria, an

IMPORTANCE: Tuberculosis preventive therapy is central to reducing tuberculosis, and foreign-born individuals account for most US tuberculosis cases. Current US Preventive Services Task Force guidance recommends testing and treating all foreign-born individuals regardless of age or time since immigration, yet the risks of disease progression and of treatment-related harm are not uniform across these groups.
OBJECTIVE: To evaluate the cost-effectiveness and health outcomes of tuberculosis infection treatment strategies among immigrants from high-burden settings, stratified by age and time sinc

Immune checkpoint inhibitor therapy has transformed the prognosis of multiple solid-organ malignancies; however, such advances are not without significant adverse events known as immune-related adverse events. A novel pattern of autoimmune-like hepatitis following exposure to certain medications, including statins, isoniazid, nitrofurantoin, anti-tumor necrosis factor-alpha therapy and minocycline, has recently been recognized. We present the case of a 66-year-old male patient with extensive-stage small-cell lung carcinoma who developed acute liver injury while receiving concomitant atezolizum

INTRODUCTION: Latent tuberculosis infection (LTBI) screening plays a vital role in global efforts to eliminate tuberculosis (TB). While LTBI treatment has been associated with lower rates of reactivation of TB and all military branches screening programs for LTBI screening, there is limited military data on factors associated with screening positivity and treatment completion.
MATERIALS AND METHODS: This retrospective study examined positive Quantiferon-Plus (QFT-Plus) assays in the San Antonio Military Health System between January 2022 and June 2024. Electronic medical records were reviewed

Using single-cell transcriptomics of bronchoalveolar lavage cells from Mtb/SIV co-infected rhesus macaques on cART, we reveal profound immune dysregulation during early SIV co-infection of latent tuberculosis. SIV induces a sharp decline in CD4+ T cells, NK, and NKT cells, with incomplete recovery of Mtb-specific TH1 effector responses despite viral suppression. Instead, a persistent TH17-skewed environment emerges, alongside sustained myeloid inflammation driven by Type I interferon signaling and pro-inflammatory regulators such as KLF6 and NFKB1. Ligand-receptor network analyses demonstrate

Bacille Calmette-Guérin (BCG) provides limited protection against tuberculosis relapse. We investigated whether adjunctive immunotherapy targeting B cells enhances BCG-mediated protection in a murine model of latent tuberculosis. While BCG alone had modest effects, adjunctive immunotherapy with recombinant APRIL significantly reduced relapse. Protection was associated with selective remodeling and activation of marginal-zone B cell compartments, whereas effects on T cell populations were comparatively modest. These findings support further investigation of B cell-targeted immunomodulation to i

Prostatic tuberculosis is a rare manifestation of genitourinary tuberculosis and may closely mimic clinically significant prostate cancer on multiparametric MRI, creating considerable diagnostic uncertainty. We report the case of a 46-year-old Nigerian man undergoing follow-up for latent tuberculosis infection who reported a progressive reduction in ejaculatory volume while attempting conception. He had minimal lower urinary tract symptoms (International Prostate Symptom Score: 1/35), a prostate-specific antigen level of 3.94 µg/L, and a palpable prostatic nodule on digital rectal examination.

OBJECTIVES: Tuberculosis (TB) is an airborne infectious disease that requires early detection and contact management to prevent community transmission. Timely diagnosis and treatment of latent tuberculosis infection (LTBI) can reduce the risk of progression to active TB. This study analyzed the results of TB contact investigations conducted in 2025 to evaluate the status of contact management and provide baseline evidence for future TB control policies.
METHODS: Household and congregate setting contacts of patients with pulmonary TB registered in the Korea Tuberculosis Network System in 2025

Tumor necrosis factor-alpha (TNF-α) inhibitors, such as adalimumab, are integral in the managing refractory ankylosing spondylitis (AS). However, their immunosuppressive effects elevate the risk of reactivation of latent tuberculosis (TB), especially extrapulmonary TB (EPTB), which can present with non-specific symptoms and mimic malignancy. We report the case of a 40-year-old man with ankylosing spondylitis who had been on long-term adalimumab therapy. He presented with a low-grade fever and weight loss. An 18F-FDG PET/CT scan revealed intense FDG uptake in the peritoneum, omentum, and mesent

Rasmussen's aneurysm is a rare but potentially fatal complication of pulmonary tuberculosis. It represents a pseudoaneurysm of a pulmonary arterial branch caused by erosion of the arterial wall secondary to chronic inflammation within a tuberculosis cavity. Although uncommon, especially in non-endemic regions, rupture of this aneurysm can result in massive hemoptysis and carries a high mortality if not promptly recognized and managed. We report a case of a healthcare worker in the United States, a non-endemic region, who presented with massive, life-threatening hemoptysis. She was found to hav

INTRODUCTION: Latent tuberculosis infection (LTBI) represents a major global reservoir for active disease, characterized by persistent, metabolically inactive bacilli that are difficult to eradicate. Novel therapeutic approaches targeting bacterial persistence and host immune responses are needed.
METHODS: A rabbit model of latent-like mycobacterial infection was established using aerosolized Mycobacterium aurum. The rabbits were treated with isoniazid and rifapentine (HR), ethanolic extract of Allium cepa (EAC), and combination therapy. Treatment efficacy was evaluated using colony-forming u

Tuberculosis (TB) continues to pose a significant public health challenge. An effective immune response against Mycobacterium tuberculosis (M.tb) is widely dependent on T cell memory responses, particularly in Latent tuberculosis infection (LTBI) and active tuberculosis disease (ATB). We quantified and phenotypically characterized peripheral blood mononuclear cells (PBMCs) from Southern Thai cohort after stimulation with established M.tb specific antigen: PPD and novel antigens: IrtA, PE9 and PPE68. We compared immune responses in 60 individuals (ATB n = 20, LTBI n = 20 and healthy controls (H
