The One-Per-Thousand Effect: Rare Catastrophic Events, Hindsight, and Defensive Medicine in Modern Obstetrics.

RATIONALE, AIMS, AND OBJECTIVES: Rare catastrophic obstetric outcomes are uncommon in high-income settings but may exert an influence on clinical practice that is disproportionate to their epidemiological frequency. This paper proposes the one-per-thousand effect, a conceptual framework describing how exceptional adverse events may progressively influence clinical judgement, organisational behaviour, and intervention thresholds. Although developed in obstetrics, the framework addresses broader questions of clinical decision-making under uncertainty, patient safety, and healthcare evaluation.

Clinical Factors and Biomarkers During Pregnancy and Risk of Cardiovascular Disease.

IMPORTANCE: Cardiovascular disease (CVD) is the leading cause of death among women worldwide. Pregnancy serves as a natural cardiovascular stress test and universal clinical encounter, yet few approaches leverage its insights to inform long-term cardiovascular risk. OBJECTIVE: To determine whether clinical measures and biomarkers obtained during pregnancy may identify women at risk of long-term CVD. DESIGN, SETTING, AND PARTICIPANTS: This was a registry-linked, population-based cohort study of all pregnancies reaching at least 22 weeks between June 2010 and October 2013 in Southern Denmark.

RSV ready? Exploring feasibility and acceptability of RSV immunization options in low- and middle-income countries.

BACKGROUND: Respiratory syncytial virus (RSV) is the leading cause of pneumonia-related mortality in infants younger than 6 months, with the majority of deaths occurring in low- and middle-income countries (LMICs). Two RSV preventive products-a maternal vaccine and a long-acting monoclonal antibody (mAb) for infants-were recently recommended by the World Health Organization. Understanding the acceptability and programmatic feasibility of delivering these products is essential for policy and implementation planning in LMICs. METHODS: PATH conducted a formative study in Kenya, Nepal, and Peru (

Black Women's Accounts of Clinician Interactions in the Context of High-Risk Pregnancies.

PURPOSE: Respectful maternity care practices are important to understand and support. Enhancing practice to promote positive experiences and optimal health among Black women experiencing a high-risk pregnancy is particularly urgent. Using principles of relationship-centered care through a critical race lens, our aims were to describe individuals' accounts of clinician interactions, including the role of racial concordance, and participant accounts of the short-term and potential long-term outcomes of these interactions. METHODS: We conducted a qualitative descriptive study (N = 14) between Se

Hypertensive disorders of pregnancy, maternal cardiovascular disease mortality and the role of familial predisposition: a Norwegian population-based sibling-comparison, sibling-spillover and negative-control cohort study.

Hypertensive disorders of pregnancy (HDP) are associated with increased maternal cardiovascular disease (CVD) mortality, with risks varying by HDP subtypes and subsequent pregnancy outcomes. The contribution of shared familial factors given this heterogeneity is unclear. We conducted a population-based study using Norwegian registries (1967-2020) including 1 106 658 women with complete pregnancy histories, of whom 628 345 had at least one full sibling. Women with HDP were classified into low-risk (gestational hypertension or term pre-eclampsia followed by no HDPs) and high-risk (all other patt

Foetal Reduction in a Cornual Heterotopic Pregnancy With Potassium Chloride Administration.

Heterotopic pregnancy is the simultaneous occurrence of an intrauterine pregnancy with an extrauterine pregnancy. Heterotopic pregnancies remain a diagnostic challenge, as many can be asymptomatic, or clinicians could be falsely reassured once an intrauterine pregnancy is sighted on an ultrasound. Given their rarity, there is also no global consensus on management guidelines. In this study, we illustrate a case of a heterotopic pregnancy with preservation of the intrauterine pregnancy and management of the ectopic pregnancy with potassium chloride (KCl) administration.

Biomarker screen-guided care for preterm birth risk in nulliparous pregnancies: a subgroup analysis of the PRIME randomized controlled trial.

OBJECTIVES: The PRIME randomized trial demonstrated improved neonatal outcomes using biomarker-guided risk stratification to target interventions to pregnancies at higher risk of preterm birth (PTB). This subgroup analysis evaluated its effectiveness in nulliparous pregnancies, a population with higher baseline risk and limited obstetric history to inform risk assessment. METHODS: PRIME (NCT04301518) was a 19‑center randomized controlled trial evaluating a screen‑guided care strategy using the IGFBP4/SHBG biomarker ratio versus routine care in otherwise low‑risk singleton pregnancies. Early assessment of maternal bleeding: development and validation of a prognostic model predicting death and near miss.

BACKGROUND: Bleeding after childbirth is a leading cause of mortality. Most deaths are in low-income and middle-income countries. Early identification of women at high risk of life-threatening bleeding is vital to guide the use of life-saving interventions. The aim of this study was to develop and validate a prognostic model to predict death and near miss from bleeding after childbirth and to use it to a create simple clinical score to identify women's level of risk at admission. METHODS: This observational prognostic study used multilevel logistic regression with data from 22 low-income, mid

Factors Associated With Thromboembolism in Neonates: A Systematic Review and Meta-Analysis.

IMPORTANCE: While venous thromboembolism (VTE) is increasingly recognized in neonates and prothrombotic risk factors are being identified, no risk assessment model currently exists to enable early identification of neonates at high risk of VTE. A thorough understanding of existing evidence is necessary to inform the development of a risk-assessment model. OBJECTIVES: To describe maternal, obstetrical, and neonatal risk factors associated with VTE in neonates. DATA SOURCES: A literature search of Medline, Embase, CINAHL, and clinicaltrials.gov databases was performed on March 16, 2025, for pe

High risk and low incidence diseases: Peripartum cardiomyopathy.

INTRODUCTION: Peripartum cardiomyopathy (PPCM) is a serious condition that carries with it a high rate of morbidity and mortality. OBJECTIVE: This review highlights the pearls and pitfalls of peripartum cardiomyopathy, including presentation, diagnosis, and management in the emergency department (ED) based on current evidence. DISCUSSION: PPCM presents in the last month of pregnancy and up to five months postpartum. The presentation is similar to other forms of heart failure with fatigue, dyspnea on exertion, and lower extremity edema, but these mimic many other symptoms typical of pregnancy

Effect of Intraoperative Sublingual Nitroglycerin on the Rate of Active Uterine Segment Incision in Preterm Cesarean Birth.

INTRODUCTION AND AIM: Active segment uterine incisions, including classical, T-shaped, and high transverse incisions, are often performed during preterm cesarean deliveries (CD), especially in patients delivering at less than 30 weeks of gestation, when the lower segment can have inadequate space for a safe maternal or fetal delivery. These uterine incisions increase the risk of maternal morbidity due to longer operative time, increased blood loss, and the rate of uterine dehiscence in future pregnancies compared to lower segment uterine incisions. Patients with uterine incisions in the active

"Actionable" risk for preterm birth: patterns and prediction in California singleton births 2016-2020.

BACKGROUND: Preterm birth (PTB, < 37 weeks of gestation) is the leading cause of child mortality in the United States (U.S.) and worldwide, and has substantial short- and long-term health consequences for mothers and infants. Each year, > 350,000 infants in the U.S. are born preterm, and rates continue to rise in parallel with maternal risk factors such as hypertension, diabetes, anemia, asthma, and mental health conditions. Evidence-based interventions exist for many of these conditions and are associated with improved pregnancy outcomes, including low-dose aspirin for preeclampsia prevention
