Identification of predictive factors for reversal of cerebral vasculopathy in an original longitudinal cohort study in newborns with sickle cell anaemia.

Cerebral macrovasculopathy (CV) is a major complication in children with sickle cell anaemia (SCA) and usually requires a long-term transfusion programme (TP) to prevent stroke. This study aimed to identify factors predicting reversal of CV on TP in a single-centre newborn cohort. Among 375 patients, 50 presented CV and received TP. Median age at diagnosis was 4.9 years [3.6-6.1], with median follow-up of 13.3 years (915 patient-years). Transcranial Doppler (TCD) velocities and magnetic resonance angiography (MRA) normalised in 35 patients, that is, they had reversal of CV (RCV group). Fifteen

Interpreting antibody identification tests with large language models: Critical role of tabular data formatting.

BACKGROUND: Antibody identification testing is essential for preventing hemolytic transfusion reactions from unexpected red cell antibodies. Although automated interpretation is desirable to improve efficiency and consistency, conventional algorithmic approaches show limited accuracy, particularly in complex cases. Large language models (LLMs) offer a potential alternative; however, their performance is known to be sensitive to tabular data, which is a standard format for antigen profiles in antibody identification. STUDY DESIGN AND METHODS: We generated 100 simulated antibody identification

Recent advances in lateral flow immunoassay for virus detection: current achievements, challenges and future prospects.

Infectious diseases continue to pose significant global public health challenges, highlighting the urgent need for rapid, accessible, and scalable diagnostic strategies. Lateral flow immunoassay (LFIA), characterized by its rapid response, low cost, and instrument-free operation, has been widely adopted for infectious disease detection. However, it is still predominantly regarded as a point-of-care testing (POCT) tool, while its broader role in public health systems remains underappreciated. In this review, LFIA is repositioned as a point-of-need (PoN) self-testing platform with the potential

Surgeon Perspectives on Intraoperative Cell Salvage in Liver Resection for Cancer: A National Survey of Hepatopancreatobiliary Surgeons.

BACKGROUND: Intraoperative cell salvage reduces exposure to allogeneic blood transfusion during major surgery. However, its use in liver resection for cancer remains limited due to concerns regarding tumor-cell dissemination and oncologic safety. The extent to which these concerns influence surgical practice is not well characterized. METHODS: We conducted a national cross-sectional survey of Canadian hepatopancreatobiliary surgeons to assess familiarity, use, and perceptions of IOCS during liver resection for cancer. The anonymous electronic survey was distributed through national HPB networ

Measurement of Coagulation Factor Activity and Bacterial Growth in Canine and Feline Fresh Frozen Plasma With Unintended Exposure to Room Temperature.

BACKGROUND: To assess the viability of canine and feline plasma for the purpose of transfusion therapy after freezer malfunction and inadvertent thawing of numerous fresh frozen plasma (FFP) units resulted in prolonged exposure to room temperature. KEY FINDINGS: After a plasma freezer alarm malfunction, numerous units of canine and feline FFP thawed over a period of approximately 10 h. Hemostatic capacity was assessed with measurement of prothrombin time (PT), partial thromboplastin time (PTT), and coagulation factor analysis on a representative unit of canine and of feline FFP. Toxin-induced coagulopathy following a MacMahon viper bite.

The management of a snakebite is often challenging due to limited experience, limited access to intensive care, delays in presentation and high variability in venom composition and specific antivenom availability. Classified within the genus Eristicophis, the MacMahon viper is a venomous snake species for which only two detailed case reports exist. Its venom has predominantly haemotoxic properties while neurotoxic and cytotoxic effects are less pronounced. This case illustrates the effects of Eristicophis macmahonii venom on both haemostasis and cardiovascular stability, in addition to a signi

Severe Transfusion-refractory Cytopenia Mimicking Bone Marrow Failure in a Patient Living with HIV after Antiretroviral Therapy Interruption.

Severe thrombocytopenia is a frequent but diagnostically challenging complication in people living with HIV (PLWH). We report the case of a 43-year-old woman with HIV who developed life-threatening hematochezia and profound thrombocytopenia after the interruption of antiretroviral therapy. Although bone marrow aspiration showed hypocellular fatty marrow, preserved leukocyte counts and the overall clinical context supported immune thrombocytopenia secondary to uncontrolled HIV replication. Treatment with intravenous immunoglobulin and high-dose corticosteroids led to independent transfusion and

Guillain-Barré Syndrome.

OBJECTIVE: This article reviews the epidemiology, clinical symptoms, examination findings, pathogenesis, diagnostic criteria, and recent guidelines of Guillain-Barré syndrome (GBS) and its variants, including information on outcome measures, management, and ongoing research. LATEST DEVELOPMENTS: CSF examination and electrodiagnostic testing are useful in some cases to support the diagnosis, particularly when GBS variants are suspected. Antibody testing is of limited value in typical sensorimotor GBS, but useful when GBS variants and nodopathies are considered. According to evidence-based guid

Vancomycin-induced immune hemolytic anemia: a case report and literature review.

Drug-induced immune hemolytic anemia (DIIHA) is a rare but potentially life-threatening adverse drug reaction. Although more than 150 drugs have been reported to induce DIIHA, vancomycin, a glycopeptide antibiotic widely used in clinical practice, is rarely associated with hemolysis, making it prone to underdiagnosis or misdiagnosis. This article reports a case of vancomycin-induced DIIHA in a 64-year-old female patient after right revision knee arthroplasty (RKA). This study investigated the serological tests associated with vancomycin and tracks drug antibody titers through longitudinal moni

[Consequences of celiac disease in an unaccompanied minor Eritrean refugee: severe intramedullary hemolytic anemia due to vitamin B12deficiency].

Unaccompanied minor refugees (UMRs), particularly from regions like Syria, Eritrea, and Iraq, face significant healthcare challenges in the Netherlands. Their unique medical and psychosocial needs necessitate a culturally sensitive diagnostic approach and coordinated care. Chronic conditions linked to nutritional deficiencies, such as celiac disease, are of particular concern. Celiac disease is an immune-mediated disorder triggered by gluten, leading to inflammation and malabsorption in the intestines. We present a case of a 16-year-old Eritrean UMR who, after a lengthy migration, arrived with

Single-session therapeutic plasma exchange as salvage therapy for pegaspargase-induced severe acute pancreatitis accompanied by multiple organ dysfunction in a pediatric patient with B-cell precursor acute lymphoblastic leukemia: a case report.

BACKGROUND: Pegaspargase-related severe acute pancreatitis (SAP) combined with persistent multiple organ dysfunction syndrome (MODS) is a life-threatening adverse reaction in children receiving acute lymphoblastic leukemia (ALL) chemotherapy. Standard intensive supportive care often fails to halt progressive organ damage, while clinical data regarding therapeutic plasma exchange (TPE) as a rescue intervention in pediatric populations remain scarce. Guidelines released by the American Society for Apheresis (ASFA) do not recommend routine TPE administration for all patients diagnosed with acute

Role of prokinetic drugs prior to endoscopy in acute upper gastrointestinal bleeding: a systematic review and meta-analysis.

BACKGROUND: Acute upper gastrointestinal bleeding (UGIB) is a common gastrointestinal emergency in which retained blood and clots may impair endoscopic visualization, delay diagnosis, and increase the need for repeat endoscopy. Pre-endoscopic prokinetic therapy, particularly erythromycin, may improve gastric clearance before endoscopy, but its effects on clinical outcomes remain uncertain. OBJECTIVE: To evaluate the efficacy and safety of pre-endoscopic prokinetic therapy in adults with acute UGIB, with attention to the strength of evidence for erythromycin and metoclopramide. METHODS: We co

Tissue calcinosis in dermatomyositis - potential role of combination therapy with JAK inhibitors. Case report and literature review.

Calcinosis is a disabling and difficult-to-treat complication of adult dermatomyositis (DM), juvenile dermatomyositis (JDM), and systemic sclerosis (SSc), for which no standardized therapy is currently available. Growing evidence implicating JAK/STAT signaling in dermatomyositis has prompted interest in Janus kinase inhibitors (JAKi) as a therapeutic option. We describe a 52-year-old woman with long-standing juvenile-onset dermatomyositis and extensive, treatment-refractory calcinosis who received off-label baricitinib after multiple previous therapies. After one month, the patient reported im

Emergency whole-tank transfer of cord blood units after cryogenic failure: A safe rescue strategy.

BACKGROUND: Cryogenic storage of cord blood units (CBUs) is critical to ensure long-term viability and safety. Loss of vacuum in liquid nitrogen (LN2) tanks is an exceptionally rare but high-risk event that may lead to accelerated LN2 evaporation and potential warming of stored units. CASE REPORT: We describe an emergency rescue transfer following suspected vacuum loss in a BioArchive cryogenic tank at a public cord blood bank. The tank contained 2880 CBUs, including 14 directed units. Progressive LN2 consumption suggested structural failure, and in situ repair was not feasible. A rapid trans
