Background/Objectives: The Kidney Donor Profile Index (KDPI) is an important metric for evaluating the quality of donor kidneys and predicting post-transplant outcomes. The Estimated Post-Transplant Survival (EPTS) score is a tool for estimating kidney transplant candidates' long-term survival. However, their validity in Eastern European cohorts is yet to be explored. This study aimed to evaluate the predictive accuracy of the KDPI and EPTS in a local cohort. Methods: We conducted a seven-year retrospective observational study at a high-volume transplant center in Romania. Data from 353 patien

INTRODUCTION: Guidelines recommend that patients with a self-reported history of kidney stones or stones on imaging during living kidney donor (LKD) evaluation undergo 24-h urine stone risk testing. We examined eligibility decisions for LKD candidates at two high-volume academic transplant centers based on 24-h urine testing and imaging findings.
METHODS: We identified potential LKDs with a self-reported history of kidney stones or stones identified on imaging, who underwent 24-h urine collection. Patients who could not donate due to other medical conditions were excluded. Differences in char

Kidney transplant outcomes depend on various factors, including donor type. Brain death (BD) has been associated with significant kidney injury, with long-term sequelae. Deciphering the impact of BD versus circulatory death on kidney pathophysiology may help to prevent kidney transplant injury. We developed three rat models: 6 h of BD [donor after brain death (DBD, n = 10)]; donor after circulatory death (DCD, n = 10) with 20 min of warm ischemia; and 6-h BD with renal denervation before BD (DNRV, n = 6). RNA-Seq was performed on kidney tissue, followed by differential gene expression and path

BACKGROUND: An efficient organ donation programme must maximise transplantation following initiation of organ recovery procedures.
METHODS: We conducted a cohort study of deceased donors in Australia (2014-2021) using Australia and New Zealand Organ Donation Registry data to characterise kidney non-retrieval (post-incision) and non-utilisation (retrieved, not transplanted). Donor characteristics included kidney side (left/right), kidney-only procurement, kidney donor profile index (KDPI), cause of death, resuscitation, donation after circulatory/neurological determination of death (DCDD/DNDD)

INTRODUCTION: Transplantation with a kidney from a living donor has superior long-term patient and graft survival compared to a kidney from a deceased donor. For a kidney donor, feelings of stress may exist at different stages of the donation process. The aim of the study was to evaluate living kidney donors' perceived and measured stress levels before and after donation.
METHODS: In this prospective observational cohort study with longitudinal follow-up, kidney donors were invited to answer three self-rating questionnaires targeting stress, vital exhaustion, and depressive symptoms the day b

PURPOSE: We aimed to evaluate kidney retransplantation outcomes to optimize patient care and reduce the risk of further retransplantation.
METHODS: We conducted a retrospective longitudinal analysis of the Scientific Registry of Transplant Recipients (SRTR) database for kidney retransplants performed in the United States from January 2000 to May 2023. Patients were divided into subgroups based on transplant date and number of previous transplants. Outcomes were assessed by patient and allograft survival.
RESULTS: A total of 34,230 patients were evaluated (12,224 in 2000-2011, 22,006 in 2012-

Background/Objectives: Living kidney donors (LKDs) can be at risk of chronic kidney disease (CKD). Several conditions are associated with hyperfiltration, such as solitary kidney, obesity, and high protein consumption. In this regard, we aimed to evaluate the risk factors for kidney function decline in LKDs and the effect of daily protein intake. Methods: Data collected from 220 living kidney donors (mean age: 48.35 ± 12.4 years; 55% female) between 2016 and 2023 were evaluated. One-year and long-term outcomes were evaluated following donor nephrectomy, along with the risk factors for GFR decl

The Australia and New Zealand Live Kidney Donor (ANZLKD) Registry is the binational registry for recording live kidney donor outcomes in Australia and New Zealand. In this retrospective analysis, it is demonstrated that only 2.93% of living kidney donors in Australia across 2004-2021 have adequate follow-up data recorded in ANZLKD. Response rates recorded through ANZLKD gradually decline from year after donation, starting at 38% at 1 year and reaching 1.6% at 15 years. Factors associated with adequate follow-up in ANZLKD included reduced eGFR and later donation year. This is amongst the first

Recipients of kidney transplants often outlive the function of the renal allograft will need ESRD management. Patients face a higher risk of mortality in the period of transition from failing allograft to dialysis. Long term risk of cardiovascular complications and risk of infections and cancer with use of long-term immune suppression contribute to poor outcomes. Patients with failing transplants appear to have poorer control of CKD complications and are more likely to initiate hemodialysis using a catheter. Outcomes of peritoneal dialysis in the setting of the failing allograft in general are

Living kidney transplantation is an established treatment for end-stage kidney disease (ESKD). While rare, the donors may develop de novo glomerular diseases post-donation. We report a case of membranous nephropathy (MN) in an 80-year-old male, diagnosed 9 years post-donation. The patient had a history of early gastric cancer treated with endoscopic submucosal dissection one year post-donation. Routine follow-up revealed asymptomatic proteinuria (4.27 g/gCr). The kidney biopsy demonstrated MN with IgG1/IgG3-dominant deposits and NELL-1 positivity. Recent evidence suggests NELL-1-positive MN ma

BACKGROUND: Accurate glomerular filtration rate (GFR) estimation is crucial for evaluating living kidney donors, especially when measured GFR (mGFR) is unavailable. This study compares the Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI), full age spectrum (FAS), and European Kidney Function Consortium (EKFC) creatinine- and/or cystatin C-based estimated GFR (eGFR) equations against iohexol mGFR to determine the optimal equation for donor eligibility assessment in a US population.
METHODS: 1210 kidney donor candidates were retrospectively analyzed, comparing eGFR equations based on

INTRODUCTION: It is possible in many countries to not only become a living solid organ donor, but to become a serial living solid organ donor, a process in which an individual subsequently donates a liver lobe after donating a kidney, or vice versa. The major ethical issues that surround uncompensated living single solid organ donation (the doctor's duties to respect autonomy, of beneficence, and of non-maleficence) have been well described, and this process is generally considered ethically permissible if the donor has sufficient health, and if their decision is voluntary, fully informed, and

BACKGROUND: A recent Organ Procurement and Transplant Network policy change removes hepatitis C virus (HCV) status and race from the Kidney Donor Profile Index (KDPI) calculation, thereby lowering the KDPI of HCV nucleic acid testing positive (NAT+) kidneys and increasing their allocation priority. However, even in the era of direct-acting antivirals, high KDPI HCV NAT+ kidneys exhibited higher discard rates compared with their HCV NAT- counterparts, and outcome data for this "high-risk" group remain limited. This study aims to address this knowledge gap by providing comprehensive outcome data

INTRODUCTION: Living kidney donation plays a crucial role in addressing organ shortage. Careful and holistic donor assessment is required to minimize donor risks. While acceptance patterns are well documented, donor candidates beyond the threshold remain largely unexamined. This study aims to analyze donor evaluations to identify reasons and trends for donor decline.
METHODS: In this retrospective observational study, we analyzed prospectively collected data of 275 potential living kidney donor evaluated at our center between 2007 and 2021, assessing demographic and health characteristics, as

BACKGROUND: The panorama of living kidney donors (LKDs) in Portugal has changed over the past decade, including kidney-paired exchange programs, undirected donations, and more complex kidney transplants, which could affect the predisposition to donation. We sought to describe the sociodemographic characteristics of a cohort of LKDs at our center and assess their perceptions of the donation process.
MATERIAL AND METHODS: LKDs who donated between January 2018 and December 2019 underwent a predonation psychosocial evaluation and a follow-up evaluation in May 2022, which included questionnaires a

Kidney transplantation from donors with HIV has recently become standard clinical practice, but the plasma inflammatory profile is not well characterized. Thirty-two cytokines and chemokines were evaluated among donors with HIV (n = 63) and without HIV (n = 41). Wilcoxon rank sum test was used to compare cytokines between groups. Donors with and without HIV were generally similar in terms of characteristics, except those with HIV had a non-significantly lower kidney donor profile index, reflecting better graft survival, creatinine, and body mass index. Most cytokine and chemokine levels were s

What is the survival of a life insurance applicant who is planning to or has previously donated a kidney? The medical director needs to assess short- and long term risk stratification in each scenario. For expectant and immediate post-donor applicants, a waiting period of 3 months is appropriate for insurability. Long-term post-donor applicants are, at best, standard risks due to the absence of renal reserve. For those with accompanying impairments such as hypertension and diabetes, which accelerate renal deterioration, adding debits to the basic rating is appropriate.
