Pushing the Limit for Marginal Grafts in the Era of Machine Perfusion: A Tale of HOPE From a Leading Italian Institution.

BACKGROUND: The advent of machine perfusion (MP) has significantly improved post-liver transplantation (post-LT) outcomes, potentially enabling the use of increasingly marginal grafts and expanding the organ donor pool. METHODS: We present a retrospective cohort study of consecutive adult patients who underwent LT between 2018 and 2023 at a leading Italian institution. The objective was to evaluate outcomes following the use of hypothermic oxygenated perfusion (HOPE) in high-risk grafts. RESULTS: A total of 507 patients were included in the final analysis, of whom 420 (83%) received extended

Surveillance biopsy detects nearly half of early rejections in living donor kidney transplants: an observational study from Nepal.

BACKGROUND: Protocol biopsies (PBs) are done at different time points after organ transplantation to explore any signs of damage to the allograft without manifesting clinically. There are no consensus guidelines on use of protocol biopsies or specific time points to perform a PBs after kidney transplantation but literature suggests that findings of PBs may be a guide to long term graft outcomes. It is specially important to know whether such interventions are worth undertaking in the developing world where patients pay for healthcare. We studied the results of PBs done in living donor kidney t

Association of posttransplant dialysis and survival after donation after circulatory death heart transplantation.

BACKGROUND: Renal failure is an important predictor of survival after heart transplantation. There is a paucity of literature exploring the association of renal impairment and donation after circulatory death (DCD) transplant outcomes. This study evaluated the effect of posttransplant renal failure requiring de novo dialysis on outcomes after DCD heart transplantation. METHODS: The United Network for Organ Sharing registry was queried to analyze adult DCD heart transplant recipients between January 1, 2019, and December 31, 2023. The cohort was stratified according to de novo posttransplant d

Transplant center practices that maximize access to living kidney transplantation: the Penn experience.

Living donor kidney transplantation (LDKT) provides superior outcomes for patients with end-stage kidney disease (ESKD) but remains underutilized due to persistent barriers including donor-recipient incompatibility, financial concerns, and limited donor outreach. This study describes a center-based strategy implemented at Penn Medicine's Center for Living Donation to specifically address these challenges and expand access to LDKT. Key components included the development of a dedicated infrastructure for donor evaluation, the integration of Living Donor Navigators to guide and support donors, a

Effect of Ultrasonic Dissection on Donor-Site Morbidity and Drainage in Latissimus Dorsi Free Flaps.

BACKGROUND: Donor-site morbidity, particularly prolonged drainage and seroma formation, remains a persistent challenge in latissimus dorsi myocutaneous (LDMC) flap surgery. Although the Harmonic scalpel allows simultaneous cutting and coagulation and has demonstrated advantages in other surgical settings, its potential to mitigate drainage-related morbidity during full-muscle LDMC flap harvest has not been fully characterized. This study evaluated its impact on donor-site outcomes in free LDMC flap reconstruction. METHODS: A retrospective review was performed of patients who underwent free LD

Managing the Costs of Kidney Paired Donation: A Survey of Contemporary U.S. Practice and Challenges.

BACKGROUND: Kidney paired donation (KPD) is increasingly used to provide access to living donor kidney transplantation (LDKT), but concerns related to managing costs may pose barriers to transplant center participation. To help inform discussions of effective cost-management strategies, we surveyed U.S. LDKT program staff on experiences, practices, and needs for managing KPD-related costs. METHODS: A survey instrument was designed by a multidisciplinary workgroup of professionals in transplant administration, finance, and clinical practice. We distributed the survey to staff at U.S. LDKT tran

Outcomes of Donation After Circulatory Death Lung Transplantation in the United States Based on Procurement Strategies: A United Network for Organ Sharing Analysis.

BACKGROUND: Donation after circulatory death (DCD) is increasingly utilized to expand the donor pool. Within DCD, direct procurement and perfusion (DPP) and thoracoabdominal normothermic regional perfusion (TA-NRP) have emerged as distinct procurement strategies. Yet, outcomes remain unclear especially in combination with ex vivo lung perfusion (EVLP). METHODS: The United Network for Organ Sharing database was queried for adult lung transplants performed between December 2019 and March 2025. Recipients were stratified as donation after brain death (DBD), DCD DPP, or DCD TA-NRP. Propensity sco

Wait Time Modifications for Black Transplant Candidates Affected by Race-Based Kidney Function Estimation.

IMPORTANCE: Until 2021, national guidelines upheld race-based equations that assigned higher kidney function estimates to Black patients, delaying subspecialist referral and transplant waitlisting. In 2023, the Organ Procurement and Transplantation Network (OPTN) mandated that US kidney transplants programs submit wait time modifications for Black candidates who were disadvantaged by these equations. OBJECTIVE: To evaluate whether implementation of the OPTN wait time modification policy was associated with changes in kidney transplant rates by race and ethnicity in the US. DESIGN, SETTING, A

Between breaths of grief, a gift: Understanding families' consent to organ donation in New Zealand intensive care units.

BACKGROUND: Consent plays a pivotal role in organ donation. In intensive care units (ICUs), families make organ donation decisions on behalf of their deceased relatives while navigating grief, emotional distress, and challenging clinical circumstances. Globally, donation opportunities do not proceed due to clinical and organisational constraints, as well as the family's decision. This raises questions about what shapes family decisions and how healthcare professionals can support the decision-making process. In New Zealand (NZ), research examining the experiences of families of potential decea

Organ Assessment and Repair Centers: Navigating Practical Hurdles in a New Era of Transplantation.

Solid organ transplantation remains limited by the availability and quality of donor organs. Despite advances in donor management and allocation policy, a substantial proportion of recovered organs are discarded, and increasing reliance on extended-criteria and donation after circulatory death donors introduces additional uncertainty regarding graft viability. Traditional preservation using static cold storage provides a limited opportunity to assess organ function or intervene therapeutically before transplantation. Recent advances in machine perfusion technologies have begun to reshape this

Expanding Access to Liver Transplantation Through Cost-effective Normothermic Machine Liver Perfusion: An Organ Procurement Organization-based Model and its Clinical Impact.

BACKGROUND: The shortage of transplantable livers, especially from donation after circulatory death (DCD) donors, remains a major challenge. Normothermic machine liver perfusion (NMLP) can further evaluate and potentially rehabilitate marginal grafts, but high costs and logistical barriers restrict its adoption. METHODS: A nonprofit organ procurement organization (OPO) integrated NMLP into its infrastructure, creating a cost-effective, equitable model for liver graft evaluation. This article describes the OPO-based approach and reports transplant outcomes from 1 regional partner center betwee

Developing a Clinically Practical Biomaterial Platform for Endogenous Liver Regeneration.

Chronic liver disease remains a major global health burden, with liver transplantation as the only definitive therapy despite severe limitations in donor availability, surgical morbidity, and patient eligibility. Although the liver has substantial intrinsic regenerative capacity, endogenous repair is often insufficient in chronic injury, cirrhosis, and acute-on-chronic liver failure. As a result, regenerative strategies that restore liver function without whole-organ replacement are increasingly pursued. This review examines controlled release biomaterial-based liver regeneration platforms, pa

Longitudinal profiling of γδ T cell dynamics after human lung transplantation links donor and recipient subsets to clinical outcomes.

INTRODUCTION: Long-term survival after lung transplantation lags behind that of other solid organ transplants, underscoring the need to better understand its complex immune responses to prevent complications and improve clinical outcomes. While most post-transplant immune studies on T lymphocytes have focused on αβ T cells, the role of mucosal-tissue-enriched γδ T cells remains largely unexplored in lung transplantation. METHODS: We performed a longitudinal analysis of the presence, chimerism, and phenotype of γδ T cells in 13 lung transplant recipients, tracking their dynamics in peripheral

Center Geography or Center Practice? Decomposing Geographic Variation in Access to Kidney Transplantation Before Versus After Circles.

BACKGROUND: Before KAS250 (circles-based allocation), donor service area (DSA) of listing was the largest contributor to deceased donor kidney transplantation (DDKT) rate disparities. Both before and after KAS250, it is unclear to what extent DSA-level disparities are attributable to center-level practice variation. We aimed to disentangle contributions to DDKT rate variation from: (1) center practices, (2) kidney distribution within sharp policy boundaries (DSAs, OPTN Regions), and (3) other geographic variation in kidney scarcity. METHODS: With national transplant registry data, we studied

Impact of Donation After Circulatory Death on Waitlist Duration and Early Outcomes in Heart Transplantation for Patients With Durable Mechanical Circulatory Support.

OBJECTIVE: While heart transplantation primarily relies on donors after brain death, the use of donors after circulatory death is increasing to address donor shortages. Although favourable outcomes with donation after circulatory death (DCD) have been reported, its safety in patients bridged with durable mechanical circulatory support (MCS) remains uncertain. DESIGN: Using the United Network for Organ Sharing database, we identified adult recipients supported with durable MCS who underwent isolated primary heart transplantation between 2019 and 2023. Outcomes were compared between recipients

Donor liver weight in normothermic machine perfusion: Does size really matter?

BACKGROUND: Large livers undergoing normothermic machine perfusion are believed to demonstrate altered homeostasis and heterogeneous graft perfusion during normothermic machine perfusion; these features of large livers may dissuade their use for normothermic machine perfusion and transplantation, reducing the donor pool. However, it remains unclear if liver size has an impact on transplant outcomes following normothermic machine perfusion. METHODS: A retrospective, single-center review of all transplanted livers that underwent normothermic machine perfusion (OrganOx metra) over a 32-month per

Family perspectives on declining organ donation: A qualitative study.

INTRODUCTION: With a national consent rate of 53%, Australian families decline donation in much higher numbers than other high-income countries. Yet, few studies have focused specifically on families who declined donation. OBJECTIVE: The aim of this study was to examine the experiences of families who declined donation after the death of a relative. METHODS: A qualitative study using in-depth interviews was conducted to elicit detailed narrative accounts. Interviews were conducted between April 2022 and May 2025 in South Australia, Victoria, Queensland, and New South Wales. Thirty-three rece

Chronological Changes in Causes of Death in Brain Death Donors: A 7-Year Analysis in the Tuscany Region.

OBJECTIVES: During the past 5 years, donation from donors after circulatory death has increased by 4 % worldwide, whereas donation from donors after brain death has remained unchanged. Organs from circulatory death donors are known to be less performative than organs from brain death donors, and the pool of brain death donors remains the main source of organ transplants in many countries. Chronological changes in the criteria for brain death in donors have occurred in many countries. MATERIALS AND METHODS: We describe chronological changes in causes of death in 2877 brain death donors consecu

Trends, variation, and predictors of coronary angiography in potential cardiac organ donors.

PURPOSE: Performance of invasive coronary angiography (ICA) to screen for coronary artery disease (CAD) in younger potential heart donors is discretionary and thus may vary across regions. We examined the variation, predictors, and outcomes of ICA in potential heart donors in the United States. METHODS: Using the Scientific Registry of Transplant Recipients (SRTR), our cohort included brain-dead potential heart donors from 2018 to 2023. Older donors (> 50 years for men, > 55 for women) were excluded; the remainder were stratified as young (< 35) or middle-aged. We further stratified by "low"

Opinion: Gavage Administration of MXene as a Route-Specific Alternative to Intravenous Injection into the Bloodstream of Laboratory Animals for Reducing Systemic Nanotoxicity Risks in Immunosuppression and Post-Transplantation Models with Bile Acid Modification.

Intravenous injection of engineered material dispersions into the bloodstream of laboratory animals has been a popular method for in vivo immunosuppression. It is also one of the primary approaches for delivering immunosuppressive and anti-inflammatory drugs within the early stages of donor cell/organ transplantation. Due to their systemic interaction with blood circulation and immune cells, the administration is reported to act effectively in suppressing host immune responses. Recent studies on immunomodulatory nanomaterials, including MXenes, have reported their potential for immunosuppressi
