Case report: Hand-arm vibration syndrome in a dental technician.

Occupational exposure to vibration using hand-held tools may cause hand-arm vibration syndrome (HAVS) among workers. We report the case of a 39-year-old lady with a 5-year work exposure to repetitive wrist movements and vibration from hand tools in the dental fabrication laboratory, working as a dental technician. She presented with a 3-year history of chronic pain over the wrists and positive symptoms of numbness and tingling in the hands, pain and discomfort of the fingers during cold exposure, and poor grip strength. Symptoms over the right hand were worse than the left. She is right-hand d

Concordance between the Stockholm Workshop Scale and the International Consensus Criteria for grading the severity of neurosensory manifestations in hand-arm vibration syndrome in a Swedish clinical setting.

OBJECTIVES: Occupational exposure to vibration using hand-held tools may cause hand-arm vibration syndrome (HAVS). Correct diagnosis and grading of severity are crucial in protecting the individual's health and for workers' compensation claims. The International Consensus Criteria (ICC) has been suggested to replace the widely used Stockholm Workshop Scale (SWS). The aims were to, in a clinical setting, assess the concordance between the SWS and the ICC neurosensory severity grading of vibration injury, and to present the clinical picture according to symptoms, type of affected nerve fibres an

Vibration Exposure Safety Guidelines for Surgeons Using Power-Assisted Liposuction (PAL).

BACKGROUND: As power-assisted liposuction (PAL) gains in popularity, plastic surgeons operating these devices experience occupational exposure to hand-transmitted vibration, which can result in hand-arm vibration syndrome, a debilitating neurovasculopathy. OBJECTIVES: The objective of the study was to determine vibration exposure from the utilization of a PAL device during surgery to generate recommendations for safe use. METHODS: Vibration emission of a commonly utilized PAL system (MicroAire-650, Surgical Instruments, Charlottesville, VA) was examined employing a vibration data logger unde

Health effects associated with occupational exposure to hand-arm or whole body vibration.

Workers in a number of different occupational sectors are exposed to workplace vibration on a daily basis. This exposure may arise through the use of powered-hand tools or hand-transmitted vibration (HTV). Workers might also be exposed to whole body vibration (WBV) by driving delivery vehicles, earth moving equipment, or through use of tools that generate vibration at low dominant frequencies and high amplitudes, such as jackhammers. Occupational exposure to vibration has been associated with an increased risk of musculoskeletal pain in the back, neck, hands, shoulders, and hips. Occupational

Vibration and Ergonomic Exposures Associated With Musculoskeletal Disorders of the Shoulder and Neck.

BACKGROUND: According to the US Bureau of Labor Statistics, musculoskeletal disorders (MSDs) accounted for 32% of all nonfatal injury and illness cases in 2014 among full-time workers. Our objective was to review and summarize the evidence linking occupational exposures to vibration and awkward posture with MSDs of the shoulder and neck. METHODS: A literature search was conducted using the terms musculoskeletal disorders, vibration, and awkward posture. All types of observational epidemiologic studies, with the exception of case reports, published during 1998-2015 were included. Databases sea

Transcriptional Pathways Altered in Response to Vibration in a Model of Hand-Arm Vibration Syndrome.

OBJECTIVE: The aim of this study was to use an established model of vibration-induced injury to assess frequency-dependent changes in transcript expression in skin, artery, and nerve tissues. METHODS: Transcript expression in tissues from control and vibration-exposed rats (4 h/day for 10 days at 62.5, 125, or 250 Hz; 49 m/s, rms) was measured. Transcripts affected by vibration were used in bioinformatics analyses to identify molecular- and disease-related pathways associated with exposure to vibration. RESULTS: Analyses revealed that cancer-related pathways showed frequency-dependent change

Frequency-dependent effects of vibration on physiological systems: experiments with animals and other human surrogates.

Occupational exposure to vibration through the use of power- and pneumatic hand-tools results in cold-induced vasospasms, finger blanching, and alterations in sensorineural function. Collectively, these symptoms are referred to as hand-arm vibration syndrome (HAVS). Currently the International Standards Organization (ISO) standard ISO 5349-1 contains a frequency-weighting curve to help workers and employers predict the risk of developing HAVS with exposure to vibration of different frequencies. However, recent epidemiological and experimental evidence suggests that this curve under-represents

Neuropathic pain-like alterations in muscle nociceptor function associated with vibration-induced muscle pain.

We recently developed a rodent model of the painful muscle disorders induced by occupational exposure to vibration. In the present study we used this model to evaluate the function of sensory neurons innervating the vibration-exposed gastrocnemius muscle. Activity of 74 vibration-exposed and 40 control nociceptors, with mechanical receptive fields in the gastrocnemius muscle, were recorded. In vibration-exposed rats ∼15% of nociceptors demonstrated an intense and long-lasting barrage of action potentials in response to sustained suprathreshold mechanical stimulation (average of 2635 action pot

Mechanisms mediating vibration-induced chronic musculoskeletal pain analyzed in the rat.

UNLABELLED: While occupational exposure to vibration is a common cause of acute and chronic musculoskeletal pain, eliminating exposure produces limited symptomatic improvement, and reexposure precipitates rapid recurrence or exacerbation. To evaluate mechanisms underlying these pain syndromes, we have developed a model in the rat, in which exposure to vibration (60-80Hz) induces, in skeletal muscle, both acute mechanical hyperalgesia as well as long-term changes characterized by enhanced hyperalgesia to a proinflammatory cytokine or reexposure to vibration. Exposure of a hind limb to vibration

Hand-arm vibration syndrome in South African gold miners.

BACKGROUND: Hand-arm vibration syndrome (HAVS) is associated with the use of hand-held vibrating tools. Affected workers may experience symptoms of tingling, numbness, loss of grip strength and pain. Loss of dexterity may impair everyday activities, and potentially increase the risk of occupational accidents. Although high vibration levels (up to 31 m/s(2)) have been measured in association with rock drills, HAVS has not been scientifically evaluated in the South African mining industry. AIMS: The aim of this study was to determine the prevalence and severity of HAVS in South African gold min

Validity of self reported occupational exposures to hand transmitted and whole body vibration.

OBJECTIVES: To assess the accuracy with which workers report their exposure to occupational sources of hand transmitted (HTV) and whole body vibration (WBV). METHODS: 179 Workers from various jobs involving exposure to HTV or WBV completed a self administered questionnaire about sources of occupational exposure to vibration in the past week. They were then observed at work over 1 hour, after which they completed a second questionnaire concerning their exposures during this observation period. The feasibility of reported sources of exposure during the past week was examined by questioning mana

[The effect of the vibration and noise factor on the physical work capacity and autonomic nervous system function of workers in vibration-hazardous jobs].

An unexampled study was made of the peripheral vegetative incompetence syndrome developing in vibration disease. It is shown that chronic occupational exposure to vibration and noise results in damage to the segmentary apparatus of the vegetative nervous system. Vegetative inadequacies are manifested by impairement of cardiovascular functions. With exposure to vibration and noise, it is the sympathetic portion of the vegetative nervous system that is first affected. The service duration-related progression of the pathological process results in increase of the share of parasympathetic patholog

[Chrono-thermometry in the diagnosis of Raynaud-like vasculopathies caused by prolonged use of vibrating tools].

Raynaud's phenomenon may be secondary to several different pathological conditions. In some cases it is favoured by occupational exposure to vibration of the upper arms. The diagnosis of Raynaud's phenomenon requires clinical information together with specific technical investigations. This report presents the results of 232 chrono-thermometric tests performed on 157 workers exposed to vibrations during the use of different kinds of tools. Seventy-five subjects not exposed to vibrations of the upper arms but with suspected Raynaud's syndrome were also tested. Chrono-thermometry enabled us to d

[Occupational exposure to vibration: Raynaud's phenomenon and the vascular response to methacholine iontophoresis].

A homogeneous group of 34 subjects exposed to occupational vibratory stress were studied to determine their vasodilatory response to methacoline. The methacoline was administered by iontophoresis. Methacoline is an acetylcholine derivate with the same pharmacological properties of acetylcholine. The 34 study subjects were divided into 3 groups according to their plethysmographic response to a preliminary cold test: group 1, had a typical Raynaud's response; group 2 had normal response to cold, and group 3 had an "intermediate" response. The subjects in group 1 did not respond to methacoline wi

Age-related evaluation of peripheral circulation of workers with vibration exposure.

This study was designed to clarify the age variation of the indices in the accelerated plethysmography (APG) for fingers and set the standard ageing curve. The indices of APG of workers with exposure to vibration were compared with those of the standard ageing curves. The subjects were 815 male workers, ranging in age from 18 to 66, without exposure to vibration and without history of hypertension, circulatory diseases or diabetes. Their indices of APG are used establishing the standard ageing curves. The workers occupationally exposed to hand-arm vibration were also the subjects to evaluate p

A review of 132 consecutive patients referred for assessment of vibration white finger.

One hundred and thirty-two subjects were referred to the Institute of Occupational Health in Birmingham over a 2 year period for assessment as to whether they met the criteria for Prescribed Disease A11 (vibration white finger). A standardized questionnaire and checklist was used, and each patient with vibration white finger was staged according to the Taylor and Pelmear (1975) classification. One hundred and sixteen subjects (88 per cent) had a clinical picture consistent with a diagnosis of vibration white finger, of which 10 did not meet the DHSS criteria for prescription. For these 116 sub

Exposure to shock and vibration and symptoms in workers using impact power tools.

In The Netherlands damage to health due to occupational exposure to shock and vibration is seldom reported. A survey was therefore made of the nature, extent and severity of exposure to shock and vibration. This paper presents the results of an epidemiological study, by questionnaire, among workers using impact power tools. Data were collected about exposure time, symptoms and the subjective workload. A prevalence rate of 17% for symptoms of white finger was found. In the logistic regression symptoms of back pain showed an increased prevalence with increasing total time exposed to vibration. T

Occupationally acquired vibratory angioedema with secondary carpal tunnel syndrome.

After several years of occupational exposure to vibration, a man developed episodic vibratory hand swelling, erythema, and pruritus. He subsequently developed symptoms compatible with carpal tunnel syndrome, but his median nerve conduction velocity remained normal. After experimental vibration of the forearm, plasma histamine levels in the ipsilateral antecubital vein rose in association with localized angioedema and slowed median nerve conduction velocity. There was no evidence of other causes of angioedema. Occupational exposure to vibration may predispose to the development of vibratory ang

Urinary excretion of hydroxyproline in workers occupationally exposed to vibration.

Urinary hydroxyproline excretion was measured in 130 chain saw operators aged 28-59 and in 31 normal control subjects aged 26-59 with no occupational exposure to vibration. The results measured were expressed as a ratio of hydroxyproline to creatinine. (1) No significant correlation between hydroxyproline/creatinine ratio and age was observed among normal subjects. (2) In chain saw operators grip and pinch strength decreased gradually with an increase of the total chain saw operating time, while the prevalence of pain in the hands or motility disturbance in the elbow joints showed a tendency t
