Acute coronary syndrome and coronary artery disease in wild-type transthyretin amyloid cardiomyopathy: a DIAMOND sub-study.

BACKGROUND: Patients with wild-type transthyretin amyloid cardiomyopathy (ATTRwt-CM) often have cardiovascular comorbidities, including acute coronary syndrome (ACS) and coronary artery disease (CAD). RESULTS AND MAJOR FINDINGS: At baseline, 9% of patients had a history of ACS and this was associated with increased risk of adverse outcomes, while this was not the case for obstructive CAD without ACS. Further prospective studies are needed to corroborate these findings.

Humeral Septal Aperture in Ancient Tombos Nubians.

OBJECTIVE: The majority of the published literature regarding the septal aperture (SA) dates to the last 200 years. The archeological literature is sparse. The collection from Tombos along the Nile River (1400-656 BCE) provides an opportunity for further study of SA in ancient populations. The purpose of this study was to determine the prevalence and shape of SA in the population as well as the size of the humerus and correlate that with and without SA in the Tombos population. METHODS: Adult humerus specimens from the Tombos skeletal population with intact distal humeri were studied using bo

The Stockholm Pilot study for Lung cancer Screening (Stockholm PLUS): feasibility of baseline low-dose CT lung cancer screening in a high-risk Swedish female population.

BACKGROUND AND PURPOSE: Low-dose CT screening of high-risk groups has been shown to reduce lung cancer mortality, and the European Council has therefore recommended that member states explore the feasibility and effectiveness of this approach. In this study, we evaluate the implementation of low-dose CT screening for lung cancer in a Swedish female population. Patient/material and methods: Women aged 54-74 years in the Southern Stockholm region were contacted via an electronic questionnaire. Individuals who met the same eligibility criteria as in the NELSON trial (a minimum smoking history of

Comprehensive non-invasive assessment of cardiac allograft vasculopathy and myocardial involvement in heart transplant patients using cardiac computed tomography.

RATIONALE AND OBJECTIVES: Cardiac graft rejection and allograft vasculopathy (CAV) are major causes of morbidity and mortality in heart transplant (HT) patients. While endomyocardial biopsy and invasive coronary angiography remain diagnostic gold standards, their invasiveness highlights the need for safer, noninvasive alternatives. Cardiac computed tomography (CT) has emerged as a promising modality for evaluating both coronary and myocardial status. OBJECTIVES: The primary aim of this study is to evaluate CT-measured extracellular volume (ECVCT) in HT patients compared to a control group and

Long-Term Clinical Outcomes of PCI Versus Medical Therapy in NSTEMI Patients with Prior CABG.

Background and Objectives: Patients with a prior history of coronary artery bypass grafting (CABG) who present with non-ST-segment elevation myocardial infarction (NSTEMI) represent a complex, high-risk subgroup due to advanced comorbidity burden and challenging coronary anatomy. Whether an invasive strategy offers meaningful benefit over conservative management in this population remains unclear. Therefore, this study aimed to compare long-term outcomes of percutaneous coronary intervention (PCI) versus medical therapy in NSTEMI patients with previous CABG and to identify independent predicto

Procedural Characteristics and Outcomes of Transvenous Lead Extraction in Patients With Prior Sternotomy.

BACKGROUND: A history of sternotomy has been debated as either a potential protective factor or a possible risk for worse outcomes in patients undergoing transvenous lead extraction (TLE). The safety and efficacy of TLE in this population remain underexplored. OBJECTIVES: This study sought to evaluate the impact of prior sternotomy on procedural outcomes in patients undergoing TLE. METHODS: Using a prospectively maintained registry, we included all patients undergoing TLE at our center between 1996 and 2022. Patients were categorized into 2 groups based on whether they had a prior sternotomy

Gentiopicroside activates PINK1-dependent mitophagy to inhibit ferroptosis and promote flap survival.

ETHNOPHARMACOLOGICAL RELEVANCE: Distal necrosis remains a major challenge in reconstructive flap surgery due to ischemia-reperfusion injury. Gentiana manshurica Kitagawa (Longdan), traditionally used to clear heat and dampness, relieve swelling and pain, and treat ischemic-inflammatory disorders, has a long history in Chinese medicine. Gentiopicroside, its bioactive component, exhibits anti-inflammatory and antioxidant activities, yet its effect on flap survival has not been fully elucidated. AIM OF THE STUDY: This work explored whether gentiopicroside improves flap viability and elucidated t

Every second counts: Association between activated clotting time and neurologic injury in neonatal ECMO.

BACKGROUND: Extracorporeal membrane oxygenation (ECMO) is a critical support modality for patients with reversible cardiopulmonary failure. Despite its benefits, ECMO is associated with neurologic adverse events (NAE), including seizures and intracranial hemorrhage. Data on modifiable risk factors for NAE in neonates remain limited. This study aimed to assess the incidence, nature, and predictors of NAE in a neonatal ECMO cohort, with a particular focus on anticoagulation state. METHODS: This retrospective single-center study included 53 neonates (<30 days old) who were supported by ECMO betw

Emerging Predictive Biomarkers of Immunotherapy Sensitivity in Patients with Non-Small Cell Lung Cancer.

In recent years, the therapeutic landscape of non-small cell lung cancer (NSCLC) has been transformed by immune checkpoint inhibitors (ICIs), which have led - in some patients - to unprecedented survival expectancy. Nevertheless, identifying patients most likely to benefit from ICI remains a major challenge. While PD-L1 expression and tumor mutation burden (TMB) represent established predictive biomarkers, their predictive ability still needs to be improved, which underscores the need for identifying additional (bio)markers for treatment selection. Recent research has highlighted multiple emer

Remodeling of Pediatric Tibial Shaft Fractures: What Should We Expect?

BACKGROUND: While the majority of tibial shaft fractures in pediatric patients are managed nonoperatively with closed reduction and casting, it is unknown how much remodeling occurs in young patients. We hypothesize that younger children would exhibit greater remodeling and lower residual angulation compared to older children six months post-injury. METHODS: Patients under age 17 years at a major pediatric health system with ICD-9 or ICD-10 codes for closed tibial shaft fractures between January 2009 and August 2023 were included. Patients with radiographs at baseline and six-month follow-up
