PURPOSE: The long-term failure rate of the arthroscopic Bankart repair may reach unacceptable values, raising the need to augment this classic procedure. Arthroscopic subscapularis augmentation is the tenodesis of the upper part of the subscapularis tendon to the anterior glenoid rim. The aim of the study was to evaluate the mid-term clinical and functional outcomes of patients operated with arthroscopic subscapularis augmentation of the classic Bankart repair due to recurrent anterior shoulder instability.
METHODS: This is a retrospective single-centre case series study with prospectively co

Scapular stress fracture (SSF) after reverse total shoulder arthroplasty is a unique and common complication that may have negative impact for long-term outcomes. Scapular stress reaction (SSR), characterized by pain and tenderness without radiographic evidence of fracture, can be precursor of SSF. We believe that early detection using SPECT/CT with proper management for SSRs including acromion, scapular spine, and coracoid process is crucial for better prognosis without additional complications by preventing SSFs. Therefore, we present 3 typical cases with SSR following reverse total shoulder

PURPOSE: Good initial fixation of glenoid component for reverse total shoulder arthroplasty (RTSA) relies on component placement and screw purchase in the scapula bone. This is especially difficult in an Asian population with small glenoid geometry. Optimal glenoid component roll angle and screw angulation to achieve the longest screws for best fixation has not been defined in the current literature.
METHODS: Computer 3D modelling of 133 scapulas with RTSA performed were analyzed to determine patient specific optimal glenoid roll angle (GRA) for the longest bi-cortical screws attainable. The

BACKGROUND: Humeral distalization is inherent to reverse total shoulder arthroplasty (rTSA) and is often produced with concomitant humeral lateralization via the level of the humeral head cut, implant positioning, implant neck shaft angle, and polymer insert thickness. Biomechanical data on the isolated effects of humeral distalization remain limited but could be important to consider when optimizing postoperative rTSA shoulder function. This study investigated the effects of isolated humeral distalization on shoulder biomechanics using a biorobotic shoulder simulator.
METHODS: Eight fresh-fr

Mechanical impingement of the rotator cuff tendons against the acromion (subacromial) and glenoid (internal) during shoulder motions has long been thought to contribute to tears. Clinically, the risk for impingement is thought to be influenced by scapular movement impairments. Therefore, our purpose was to determine the extent to which simulated changes in scapular orientation impact the proximity between the rotator cuff tendon footprint and the acromion and glenoid during scapular plane abduction. Specifically, shoulder kinematics were tracked in 25 participants using a high-speed biplane vi

Anatomy-based guidelines for shoulder surgery have established the routine preoperative evaluation of the humeral head. Despite recognized sex differences in humeral head size, there has been limited investigation into sex-specific variations in acromiohumeral contact surface (AHCS) arc length. This study aims to assess sex differences in the AHCS arc length within a sample of the Chinese population. We retrospectively analyzed 169 normal shoulder CT images from a single medical center, collected between 2011 and 2021. The AHCS arc length was defined as the distance from the superior glenoid t

Anatomic total shoulder arthroplasty is an established treatment for degenerative conditions involving the shoulder. One well-described complication of total shoulder arthroplasty is glenoid loosening, especially in younger and more active patients. As a result, several surgeons recommend against activities that may put patients at risk of implant failure after shoulder arthroplasty. Newer inlay and inset glenoid implants may have a role in treating arthritis in younger patients and may allow them to return to sports and high-impact activities, including weightlifting. These glenoid implants m

BACKGROUND: Previous research has established connections between the acromion index (AI), critical shoulder angle (CSA), acromiohumeral distance (AHD), and lateral acromion angle (LAA) with the incidence of rotator cuff tears (RCTs). Despite numerous international studies exploring the variability of shoulder joint morphology and parameters, research incorporating these parameters has yet to be conducted in Somalia. This study aims to analyze and describe the radiological parameters of acromion morphology in the Somali populations. Furthermore, to investigate the relationship of these paramet

BACKGROUND: Stuve-Wiedemann syndrome (SWS) is a rare skeletal abnormality with extensive postnatal literature but limited prenatal studies. Our group had published a diagnostic algorithm to identify prenatal cases, yet, the challenge continues, especially when there is no family history of a similar condition.
METHODS: We retrospectively analyzed our experience of prenatal diagnosis of SWS over an 8-year period with ethical approval. Literature review of articles published until July 30, 2023 from PubMed, GeneReviews, and Genetics Home Reference using search parameters, "SWS," "prenatal," and

Glenohumeral instability remains a frequent pathology, specifically in athletes and active patients. As such, several treatment options have been described. In the setting of significant glenoid bone loss (i.e., >20%), off-track Hill-Sachs lesions, and failed previous soft-tissue-based repairs, glenoid bone-augmentation techniques must be considered. These techniques restore stability by a triple blocking effect of the bony graft, the capsulolabral complex repair, and the dynamic sling effect of the conjoined tendon. The classic Latarjet procedure consists in performing a coracoid osteotomy al

For a given body mass, hominoids have longer clavicles than typical monkeys, reflecting the lateral reorientation of the hominoid glenoid. Relative length of the clavicle varies among hominoids, with orangutans having longer clavicles than expected for body mass and gorillas and chimpanzees having shorter clavicles than expected. Modern humans conform to the general hominoid distribution, but Neandertals and Upper Paleolithic Homo sapiens have longer clavicles than expected for their size and exhibit marked positive allometry in clavicle length. Relative to clavicle length, adult and newborn h

Glenoid implantation in revision arthroplasty of the shoulder remains a technical challenge in the case of severe bone loss. Metal glenoid augmentation and patient-specific glenoid implants represent new treatment options with great potential. Virtual planning techniques allow a differentiated evaluation of the extent of the bone loss and the planning of different augmentation options. Although not clearly defined so far, a patient-specific implant can be indicated if adequate fixation of the base plate in the native bone appears questionable. The planning is carried out with the support of th

Glenoid pathologies are one of the main indications for shoulder arthroplasty revisions. In most cases, the glenoid bone defect is associated with insufficiency of the rotator cuff, so that only reverse total shoulder arthroplasty can be considered as a revision implant. For stable permanent fixation of the reverse glenoid component, to avoid inferior glenoid notching, and to achieve optimal function, physiological restoration of three-dimensional glenoid anatomy is necessary. In order to approach the individual glenoid pathologies in a differentiated and optimal manner, it is important to cla

Here we present an updated inventory and study of pectoral girdle remains recovered from the Sima de los Huesos (SH) site. Here, we describe the key morphological traits of adults and, for the first time, subadult specimens. Because morphological traits can change with age, we also discuss some shortcomings related to age estimation in postcranial fossil specimens. Adult clavicles from the SH are long with a low robusticity index and marked curvatures in the superior view. Among these traits, only extreme clavicular length seems to characterize subadult individuals. Neandertals share all these

Although three-dimensional (3D) glenohumeral (GH) motion has generally been expressed only by rotational elements, its mechanistic details, including GH rotations, remain unknown owing to a lack of geometric investigations. This study aims to investigate the positional relationship between the contact path and humeral tuberosities at the GH joint during arm elevation and to consider the mechanism of GH kinematics. Shoulder kinematics were captured using two-dimensional and 3D single-plane image registration techniques in 15 young healthy subjects during flexion, scaption, and abduction. The gl

BACKGROUND: Hypoplastic glenoid morphology in the setting of glenohumeral osteoarthritis is a rare yet complex surgical problem. Treatment of this patient population with anatomic total shoulder arthroplasty (aTSA) remains controversial. Furthermore, there is no gold-standard approach, with limited guidance for surgeons on the need for glenoid version correction in the setting of a dysplastic glenoid. The purpose of this study was to evaluate mid- to long-term outcomes and reoperation rates of aTSA for the treatment of primary glenohumeral osteoarthritis with Walch type C glenoid deformity.

Medication-related osteonecrosis of the jaw (MRONJ) has been an integral part of the maxillofacial patient population for some time. The therapeutic concept ranges from conservative approaches over less extended decortications to major jaw resections, which can result in a considerable loss of quality of life. Based on three case reports, this paper presents the long-term history of patients with MRONJ of the mandible, whose disease ultimately resulted in partial or total mandibular resection and subsequent multisegmental reconstruction using a microvascular anastomosed bone flap. Furthermore,
