Clinical Utility of Methicillin-Resistant Staphylococcus aureus Polymerase Chain Reaction Nares Swabs in Burn-Injured Patients.

Methicillin-resistant Staphylococcus aureus (MRSA) is a common pathogen in burn-injured patients. Many studies have evaluated the overall utility of MRSA nares polymerase chain reaction (PCR) swabs for antimicrobial stewardship; however, there is a paucity of data in burns. This study evaluated the clinical utility of MRSA PCR in burn-injured patients at an academic medical center. This retrospective, single-center chart review included admitted adult burn-injured patients from July 2020 to June 2024. The primary objective was to determine the overall negative predictive value (NPV) of MRSA PC

Extreme variability in linezolid concentrations in the ICU: A case for routine therapeutic drug monitoring.

PURPOSE: Linezolid is an essential antimicrobial for treating multidrug-resistant gram-positive infections in critically ill patients. However, its pharmacokinetics (PK) are highly variable, potentially leading to subtherapeutic exposure or toxicity. Therapeutic drug monitoring (TDM) plays a critical role in guiding individualized dosing. SUMMARY: We describe a 66-year-old intensive care unit (ICU) patient treated with linezolid for vancomycin-resistant Enterococcus faecium (VRE) peritonitis and empyema. Despite standard dosing (600 mg IV every 12 hours), linezolid concentrations remained bel

Evaluating First Positive Cultures in Burns: Rethinking Broad-Spectrum Antibiotic Choices.

Infection is a nearly universal complication among patients with major burns, yet guidance on early empiric antibiotic therapy remains limited. Broad-spectrum antibiotics are commonly initiated in the early phase of care but carry risks of antimicrobial resistance and drug toxicities. This single-center, retrospective study evaluated the microbiological profiles and antibiotic prescribing patterns associated with first positive cultures (FPCs) in patients with major burn injuries admitted to Canada's highest-volume adult burn center between January 1, 2018 and May 1, 2023. A total of 114 patie

Dose optimization of linezolid among surgical patients: A population pharmacokinetic study.

BACKGROUND: Linezolid is classified under the reserve group of antibiotics, and it exerts its antibacterial activity by disrupting protein synthesis. Clinically, linezolid is used for treatment of severe infections caused by methicillin-resistant Staphylococcus aureus (MRSA) and vancomycin-resistant Enterococcus (VRE). It is eliminated through both renal and hepatic routes. Being a narrow therapeutic index drug, linezolid needs special dosing considerations for safe and effective treatment. MATERIALS AND METHODS: A plasma concentration dataset of 94 patients with 347 samples was used for deve

Updated sequence of ramoplanin biosynthetic gene cluster from Actinoplanes ramoplaninifer ATCC 33,076.

Actinoplanes ramoplaninifer ATCC 33,076 is the only known producer of the glycolipodepsipeptide antibiotic ramoplanin. Ramoplanin (Rmp) interferes with peptidoglycan biosynthesis by binding to lipid II and consequently impeding the transglycosylation reactions. Rmp exhibits excellent antimicrobial properties against Gram-positive bacteria, including methicillin- and vancomycin-resistant strains of staphylococci and enterococci, as well as Clostridioides difficile, showing no cross-resistance with glycopeptides and β-lactams. Rmp is poorly absorbed in the gastrointestinal tract when administere

Fecal butyrate and deoxycholic acid quantitation for rapid assessment of the gut microbiome.

The intestinal microbiome is composed of myriad microbial species with impacts on host health that are mediated by the production of metabolites. While loss of bacterial species and beneficial metabolites from the fecal microbiome is associated with development of a range of diseases and medical complications, there are currently no clinical diagnostic tests that rapidly identify individuals with microbiome deficiencies. This method aims to rapidly quantify fecal concentrations of butyrate and deoxycholic acid, as depletion of these two metabolites are associated with adverse clinical outcomes

Molecular detection of antibiotic resistance genes in Staphylococcus aureus isolated from poultry farms of Kathmandu Valley, Nepal.

BACKGROUND: Antimicrobial resistance (AMR) has emerged as a critical global health challenge and is currently addressed through a "One Health" approach that investigates its occurrence in humans, animals, and the environment, as well as the transmission pathways linking these reservoirs. In commercial poultry farms, antibiotics are routinely used to treat bacterial infections that are economically significant as well as to promote weight gain. However, data on AMR in Staphylococcus aureus (S. aureus) of poultry origin remain scarce in Nepal. Antimicrobial resistance in S aureus, particularly t
