Clinical Characterization and Outcomes of Pneumonic Plague in Madagascar: A Case Series.

Pneumonic plague is rare compared to bubonic plague; data on clinical presentation and treatment outcomes are scant. We report on 13 prospectively-enrolled suspected pneumonic plague cases (7 laboratory-confirmed, one probable, 5 not-plague) treated with aminoglycoside plus ciprofloxacin for 10 days and discuss findings in the context of the available literature.

Quarantine, diplomacy, and epidemics: Jean-Étienne-Justin Schneider and the Bubonic Plague of Sīstān in 1906.

The 1906 Sīstān bubonic plague was one of the most significant public health crises of the late Qājār period. This study examines the role of Jean-Étienne-Justin Schneider (1854-1917), head of the Central Sanitary Council (Majles-e Ḥefẓ al-Ṣeḥḥa), in implementing quarantine policies and their impact on the institutionalization of modern medicine and the expansion of public health interventions in Iran. For this purpose, a historical-analytical approach was employed to examine resources from the Center for Documents and Diplomatic History of the Ministry of Foreign Affairs, as well as contempor

A prospective cohort study to describe the morphology of buboes in patients with bubonic plague using ultrasound imaging.

BACKGROUND: Bubonic plague, caused by Yersinia pestis, is characterised by painful, enlarged lymph nodes ("buboes"). Despite centuries of clinical recognition, bubo morphology has been described only through observation and palpation. This study aimed to characterise the sonographic features of buboes over time in confirmed bubonic plague and evaluate the validity of digital calliper measurements compared to ultrasound. METHODS/PRINCIPLE FINDINGS: We conducted a prospective cohort study at three rural health centres in Madagascar between January and March 2024. Participants with suspected bub

Black Death of 1348 in Florence and SARS-CoV-2 (COVID-19) in the 650th anniversary of the death of the writer Giovanni Boccaccio (1313-1375).

The 650th anniversary of the death of the Tuscan writer Giovanni Boccaccio (1313-1375) provides an opportunity to reread some pages of his masterpiece, the Decameron, from a historical-medical perspective. In this work, Boccaccio gives an account of the "Black Death", a devastating pandemic of bubonic plague, which reached Europe, Messina (Sicily), from Asia Minor in September of 1347. The plague travelled along the commercial route taken by Genoese sailors returning from their strategic trading posts at Kaffa on the Crimean Peninsula, which was under siege by the Mongols. The framework, the c

Environmental and Host Blood Interactions Shape Yersinia pestis Dynamics in the Rat Flea, Xenopsylla cheopis.

Yersinia pestis is the causative agent of bubonic plague, a zoonotic disease that is primarily transmitted by infectious fleas. Plague is endemic in regions around the world, including the United States, where optimal climate conditions support a stable, enzootic sylvatic transmission cycle. Epizootic outbreaks periodically occur with rapid spread of disease that increases the risk of human exposure. As fleas are ectotherms that are responsive to environmental conditions, it is likely that transmission efficiency varies under different ecological conditions, with optimal conditions capable of

Vascular-associated bacterial burden and neuroinflammatory transcriptional responses observed in models of pneumonic plague.

INTRODUCTION: Yersinia pestis is the etiologic agent of plague, and the disease is categorized into several forms, including bubonic, septicemic, and pneumonic. Plague meningitis is a rare but severe complication and estimated to occur in 6-11% of documented cases. It is most frequently observed in bubonic plague patients under 15 years old that receive inadequate or no antibiotic treatment. To date, there are no reports describing plague meningitis in laboratory animal models of pneumonic plague. METHODS: Therefore, we sought to use the BALB/c mouse pneumonic plague model to investigate cent

The 1820 Mallorca plague was not a classic bubonic outbreak.

The 1820 plague outbreak in eastern Mallorca-long treated as a local episode and traditionally classified as a late bubonic manifestation of the Second Plague Pandemic-offers a unique opportunity to reassess transmission dynamics in a prebacteriological context. Using a previously unexamined primary source consisting of daily counts of deaths, we combine historical analysis with a Bayesian epidemiological model to estimate the basic reproduction number ([Formula: see text]), the mean infectious period, and the case-fatality ratio in the towns of Son Servera and Capdepera (Mallorca, Spain). Real-time tracking of Yersinia pestis infection dynamics using a near-infrared fluorescent protein labeling system.

Live imaging is essential for elucidating dynamic infection processes. Here, we developed a near-infrared fluorescent protein (NIR-FP)-based labeling strategy that enables integrated visualization of bacterial behavior in both cellular and animal infection models. Using Yersinia pestis as a model pathogen, we constructed stable fluorescent strains by either chromosomal insertion of the gene encoding NIR-FP miRFP670nano3 under the control of a strong endogenous promoter to obtain Yp-nano3, or by introducing a plasmid expressing the same reporter to obtain Yp-p-nano3. Both labeled strains exhibi

Bayesian Parameter Estimation and Stochastic Extinction Analysis in a Seasonal Bubonic Plague Transmission Model.

Mathematical models of plague dynamics provide insight into zoonotic transmission processes and epidemic persistence but often neglect stochastic effects and seasonal variability. In this study, we examine how seasonal variability influences zoonotic disease transmission through a rat-flea-human plague model. Periodic environmental forcing modulates rat abundance, flea survival, and biting activity, generating time-dependent rates that capture realistic ecological seasonality. To represent demographic randomness, we formulate a continuous-time Markov chain (CTMC) model and apply a multi-type b

The Plague of Athens: Investigating the Enigmatic Epidemic of Ancient Greece.

The ancient Greek historian Thucydides described in his book "History of the Peloponnesian War" one of the earliest epidemics in known human history, the "Plague of Athens". The plague, which lasted from 430-426 B.C., had a death toll estimated at around 75,000 - 100,000. The importance of the plague was immense. Pericles, the historical leader of Athens, was among its victims, and his death was just the beginning of the fall of the Golden Age of Athens. The enormous death toll from the plague weakened Athens, leading to its eventual defeat in the war against Sparta. So far, the cause of this

Gerbils without Borders: Invasiveness, Plague, and Micro-Global Histories of Science, 1932-1939.

In the 1930s, a series of bubonic plague outbreaks among humans cropped up in several villages at the border of Angola and Namibia. These outbreaks provoked deep concern, laying bare social and political tensions amongst neighboring imperial powers and Indigenous people within the region. Despite the appearance of this disease in what was then considered a recondite place, its spread sparked debate in transnational forums, such as the League of Nations and the Office International d'Hygiène Publique. Drawing upon archival records in Namibia, South Africa, Portugal, the United States, and the U

Evidence of a role for the F1 capsule of Yersinia pestis in enhancing transmission from mammals to fleas in a mouse model of bubonic plague.

The F1 protein capsule of Yersinia pestis is encoded by the caf operon on the 100 kb plasmid pFra and is produced at high levels in the mammalian host. Although the F1 capsule is not an essential virulence factor in many mammals, Y. pestis isolates lacking the caf operon are very rare in nature, indicating an essential biological role. Here, we show that F1-negative Δcaf mutants of Y. pestis are virulent in a mouse model but with a longer time to terminal disease. Bacterial loads in the spleen at the terminal stage of plague were not significantly different from the wild type. However, the lev

From History Books to Headlines: Plague in Modern Times.

Although often considered a historical disease, plague remains a modern-day public health concern with global relevance. Caused by the bacterium Yersinia pestis and transmitted to humans primarily through infected flea bites or contact with infected animals, plague persists worldwide, including parts of the southwestern United States. The disease presents in three main forms: bubonic, septicemic, and pneumonic. Bubonic plague, the most common form, is characterized by painful swollen lymph nodes known as buboes. Septicemic plague occurs when the infection enters the blood stream, while pneumon

Protection Elicited by Glutamine Auxotroph of Yersinia pestis.

Background/Objectives:Yersinia pestis is an important zoonotic pathogen responsible for the rare but deadly disease of people with bubonic, septic, or pneumonic forms of plague. The emergence of multidrug-resistant Y. pestis strains has attracted more and more researchers' attention to the search for molecular targets for antivirulence therapy, including anti-nutritional-virulence therapy. The glnALG operon plays a crucial role in regulating the nitrogen content within a bacterial cell. This operon codes for three genes: the structural gene glnA and the two regulatory genes glnL and glnG. In t

Attenuation of virulence in Yersinia pestis across three plague pandemics.

Yersinia pestis has spilled over from wild rodent reservoirs to commensal rodents and humans, causing three historically recorded pandemics. Depletion in the copy number of the plasmid-encoded virulence gene pla occurred in later-dated strains of the first and second pandemics, yet the biological relevance of the pla deletion has been difficult to test. We identified modern Y. pestis strains that independently acquired the same pla depletion as ancient strains and herein show that excision of pla from the multicopy pPCP1 plasmid is accompanied by the integration of a separate full pPCP1 harbor

Human genetic variation reveals FCRL3 is a lymphocyte receptor for Yersinia pestis.

Yersinia pestis is the bacterium responsible for plague, one of the deadliest diseases in history. To discover human genetic determinants of Y. pestis infection, we utilized nearly 1,000 genetically diverse lymphoblastoid cell lines in a cellular genome-wide association study. A nonsynonymous SNP, rs2282284 (N721S), in Fc receptor-like 3 (FCRL3) was associated with bacterial invasion of host cells (p = 9 × 10-8). Overexpressed FCRL3 facilitated attachment and invasion of Y. pestis and colocalized with Y. pestis at attachment sites. These properties were variably conserved across the FCRL famil
