INTRODUCTION: We report our experience with Dorsal Onlay Oral Mucosa (DOOM) urethroplasty in the management of hypospadias strictures.
METHODS/TECHNIQUE: After anesthesia, via midline penososcrotal incision urethra was mobilized/rotated to expose 12 O'clock where stricturotomy was performed. Oral mucosa was harvested from the lower lip, defatted, and tailored to the stricture length. The graft was sutured dorsally as onlay using 6-0 PDS sutures (video). We report a unique case of isolated anterior urethral ischemic necrosis in a 71-year-old man after prolonged local cryotherapy for orchitis. Persistent orchitis unresponsive to antibiotics led to repeated ice-pack application. Surgical exploration revealed edematous but viable scrotal and testicular tissues; unexpectedly, the anterior urethra was completely necrotic. PURPOSE: Intrafraction motion management is recognized as a critical component of radiotherapy. While clinical trials often recommend its use, they rarely define motion management tolerances.
