De novo development of JAK2V617F-positive polycythemia vera following allogeneic hematopoietic stem cell transplantation: a report of two rare cases.

INTRODUCTION: Post-transplant erythrocytosis following allogeneic hematopoietic stem cell transplantation (allo-HSCT) is rare and has been reported mainly in patients with aplastic anemia and, less frequently, after transplantation for other hematologic conditions. Here, we present two cases of JAK2V617F-positive polycythemia vera (PV) developing after allo-HSCT performed for chronic myeloid leukemia (CML) and myelodysplastic syndrome (MDS). CASE PRESENTATIONS: Case 1: The patient, who underwent allo-HSCT for the diagnosis of MDS. He was diagnosed with JAK2V617F-positive PV, 48 months after t

Increased glioblastoma risk in patients with myeloproliferative neoplasms and myelodysplastic syndromes.

Targeted therapies significantly extend the survival of patients with myeloproliferative neoplasms (MPNs) and myelodysplastic syndromes (MDS). This prolonged lifespan introduces an unintended clinical hazard. These patients face an elevated risk of developing primary central nervous system (CNS) tumors. The internal relationship between them remains to be explored. We integrated the Global Burden of Disease (GBD) and the Cancer Incidence in Five Continents (CI5) datasets to conduct spatial clustering mapping using Moran's I. Vector autoregression and Granger causality analysis were employed to

Germ line landscape stratification defines distinct molecular and prognostic groups in therapy-related myeloid neoplasms.

Exposure to chemotherapy (CT) and/or radiotherapy increases the risk of therapy-related myeloid neoplasms (t-MN), a heterogeneous group of disorders currently classified by treatment history rather than molecular features. Although germ line predisposition has been suggested in ∼20% of patients, its prevalence and clinical impact remain insufficiently defined. To address this, we analyzed 100 patients with t-MN by integrating clinical characteristics, prior treatment regimens, and genomic profiling. Somatic and germ line variants were identified using targeted next-generation sequencing (n = 3

Elucidating Mechanisms Underlying Fibrocyte-Mediated Myelofibrosis Induction in Myelodysplastic Syndromes.

BACKGROUND: The mechanism underlying myelofibrosis, which is associated with poor prognosis in myelodysplastic syndromes (MDS), remains unclear. METHODS: We investigated the mechanisms underlying myelofibrosis using peripheral blood mononuclear cells (PBMCs) from healthy donors, MDS92 cell culture supernatants and PolgA/+ and PolgA/A mice treated with high-dose romiplostim as an MDS model. We used the LEGENDplex™ immunoassay to analyze cytokine levels in MDS92 culture supernatants and serum samples from 12 patients with MDS or acute myeloid leukemia with myelodysplasia-related cytogenetic abn

Telomere content and genomics of myeloid neoplasia by whole-genome sequencing.

Telomere length shortening has been associated with genomic instability and acquisition of molecular lesions, but these processes have not been systematically studied across large cohorts of myeloid neoplasia (MN). As proof of concept for a novel, cross-validated whole-genome sequencing-based method of telomere content (TC) determination combined with mutations, transcriptomics, and functional assays, we studied TC in correlation with specific molecular features of a large cohort (N = 1804) of patients with MN, including acute myeloid leukemia (AML) and myelodysplastic syndrome. When compared

Risk of Cardiovascular and Cerebrovascular Diseases in Patients with Myeloid Neoplasms: Results from 1.8 Million Inhabitants of Northwest Italy.

BACKGROUND: Myeloid neoplasms are a group of pathologies characterized by clonal hematopoiesis and genetic mutations, often associated with inflammation and cardiovascular (CV) risk. Whereas targeted therapies have improved prognosis, they may also lead to CV adverse effects. The aim was to analyze the risk of developing CV or cerebrovascular disease (CVD) events in patients who received a diagnosis of myeloid neoplasm, considering four subgroups of myeloid neoplasms: chronic myeloid leukemia, myeloproliferative neoplasm, myelodysplastic syndromes (MDS), and acute myeloid leukemia. METHODS: D

Podocytic Infolding Gromerulopathy Developed at the Time of Transition from Chronic Myelogenous Leukemia to Myelodysplastic Syndrome.

Myelodysplastic syndrome (MDS) often coexists with systemic autoimmune diseases. Chronic myelogenous leukemia (CML) is an independent risk factor for intrarenal arterial hyalinosis. We describe a case of glomerulopathy that developed at the transition from CML with 5q deletion (relatively rare) to MDS with 5q and 13q deletion. Kidney histology showed glomerular hyalinosis due to intrarenal arterial changes from CML, in addition to podocytic infolding glomerulopathy (PIG) due to systemic immune responses from MDS or tyrosine kinase inhibitors, or which was coincidental. To our knowledge, this i

Persistent monosomy 7 in Philadelphia chromosome-negative cells without disease progression over nearly two decades of follow-up in chronic myeloid leukemia.

Monosomy 7 (-7) is frequently associated with myelodysplastic neoplasm/syndrome (MDS) and acute myeloid leukemia (AML). In chronic myeloid leukemia (CML), the acquisition of -7 in Philadelphia chromosome-positive (Ph-positive) cells is considered a high-risk feature for progression to blast crisis. However, the clinical significance of -7 in Ph-negative cells remains less unclear. We report a patient with CML who developed a - 7 clone in Ph-negative cells during tyrosine kinase inhibitor (TKI) therapy. The -7 clone emerged after 14 months of TKI treatment, persisted for nearly six years, and s

Understanding chronic myelomonocytic leukemia to improve nursing care.

Chronic myelomonocytic leukemia (CMML) is a rare, aggressive type of leukemia that can exhibit myeloproliferative and myelodysplastic features and has a propensity to transform into acute myeloid leukemia (AML). CMML is characterized by specific cytogenetic abnormalities and identified laboratory findings that support the diagnosis. Clinical manifestations of the disorder depend on disease subtype. Current treatments are supportive with a goal of increasing time to transformation into AML and decreasing end-organ damage. The only known cure for CMML is allogeneic stem cell transplant. This art

Targeted Therapy for a Rare PDGFRB-Rearranged Myeloproliferative Neoplasm: A Case Report.

Myeloproliferative neoplasms (MPNs) are a heterogeneous group of diseases originating from hematopoietic stem cell transformation, characterized by the clonal proliferation of hematopoietic progenitors. A specific subset includes myeloid/lymphoid neoplasms with eosinophilia and tyrosine kinase (TK) gene fusions, particularly involving PDGFR A or B, which are sensitive to TK inhibitor treatment. We report a case of a 21-year-old patient with a myeloproliferative/myelodysplastic neoplasm, presenting with hyperleukocytosis, anemia, thrombocytopenia, and elevated LDH. Clinical perception and novel insights of chronic myelomonocytic leukemia: a 10-year multi-center retrospective study.

BACKGROUND AND OBJECTIVE: Chronic myelomonocytic leukemia (CMML) is a malignant clonal disorder characterized by both myelodysplastic syndrome (MDS) and myeloproliferative neoplasm (MPN) features. Due to its relatively low incidence, there remains a lack of consensus regarding diagnostic criteria and therapeutic strategies within the academic community, which poses significant challenges in clinical management. This study aims to investigate the clinical manifestations, diagnostic approaches, therapeutic interventions, and prognostic factors associated with CMML, with the goal of providing evi

JAK2V617F reprograms Hypoxia Inducible Factor-1 to induce a non-canonical hypoxia regulon in myeloproliferative neoplasms.

Hypoxia-inducible factors (HIFs) are master transcriptional regulators, central to cellular survival in hypoxia and frequently activated within malignancy. Whilst malignant context directs the role of HIFs within oncogenesis, these mechanisms are not well characterised. Applying the JAK2V617F myeloproliferative neoplasms (MPNs) oncogene-driver model, in which HIF-1α is stabilised in normoxia (20% O2), we sought to determine whether the modality of HIF-1 activation directs its function. Through direct analysis of hypoxia-activated vs. JAK2V617F-activated HIF-1 at the chromatin, we define a JAK2

The effects of the PHF6 gene mutation on myeloid neoplasms. A single-center cohort underpinned by a systematic review of literature.

The mutation of the plant homeodomain finger protein 6 gene (PHF6MUT) in patients with myeloid neoplasms (MNs) is rare and appears to play a role in prognosis, though this is still under debate. We conducted a retrospective analysis of a cohort of 313 patients diagnosed with MNs. We also performed a systematic review (SR) of the literature to evaluate the prognostic role of PHF6 gene status in MNs. We identified 15 patients with PHF6MUT. In the multivariate analysis, PHF6MUT was associated with higher mortality compared to PHF6wild - type (hazard ratio [HR] = 1.02; 95% confidence interval [CI]

Novel therapies in myeloid neoplasms show limited benefit and increased costs over 15 years of follow-up in Southern Finland.

Over the past two decades, targeted therapies have been adopted in the treatment of myeloid neoplasms. However, their impact on overall survival (OS) and healthcare spending remains uncertain. We performed a comprehensive retrospective observational study on patients with acute myeloid leukemia (AML, n = 684), myelodysplastic syndrome (MDS, n = 899) and myelofibrosis (MF, n = 276) diagnosed between 2009 and 2023 in a large hospital district in Southern Finland. During this 15-year period, the median age increased across all cohorts. OS remained unchanged after adjusting for age, comorbidity in

A novel approach to defining progression in MDS and precursor myeloid conditions in the MDS Natural History Study.

Patients at risk for progression from myeloid precursor states (idiopathic cytopenia of undetermined significance [ICUS] and clonal cytopenia of undetermined significance [CCUS]) to myelodysplastic syndrome (MDS) are not well defined. Epidemiologic risk factors, clonal changes for MDS development, and evolution from one MDS state to another were available in the MDS Natural History Study (NHS), a prospective cohort enrolled at diverse US sites. Extensive clinical data and biospecimens were collected at baseline and follow-up. A total of 1177 (56%) individuals with MDS or MDS-related precursor

Risk of Transformation to Acute Myeloid Leukaemia and Myelodysplastic Syndromes in Patients With Myeloproliferative Neoplasms Over Attained Age and Time Since Diagnosis: A Nationwide Cohort Study.

BACKGROUND: Individuals with polycythemia vera (PV), essential thrombocythemia (ET), and primary myelofibrosis (PMF) face transformation risks to acute myeloid leukemia (AML) and myelodysplastic syndromes (MDS). While older age is linked to increased risk, it remains unclear whether risk increases with age or with disease duration. OBJECTIVES: To examine how age and disease duration affect AML and MDS transformation in MPN. METHODS: We used Swedish nationwide register data to model transformation rates as continuous functions of age and disease duration for each subtype and outcome. Cumulati

Deciphering the role of HMGA2 and let-7 microRNAs in myelodysplastic neoplasms with bone marrow fibrosis.

Myelodysplastic neoplasms (MDS) are heterogeneous clonal disorders with increased risk of transformation to acute myeloid leukemia. The HMGA2 gene, a chromatin-binding regulator suppressed by the let-7 miRNA family, has been implicated in malignant transformation and fibrosis. In this study, HMGA2 was investigated in patients with MDS, while four members of the let-7 family (let-7a, let-7b, let-7c, let-7d) were specifically assessed in those with bone marrow fibrosis. HMGA2 showed no association with established prognostic scores or survival, while in MDS with bone marrow fibrosis, distinct le

Mutant SRSF2-associated impaired erythropoiesis is defined by increased mTORC1 signaling due to FYN missplicing.

Somatic mutations in RNA splicing regulators, including the serine/arginine-rich protein SRSF2, are frequently observed in myeloid malignancies. Using mouse models and primary human samples, we investigated the impact of SRSF2 mutations on erythropoiesis. We found reduced erythropoiesis in Srsf2P95H versus wild-type mice upon stress-induced erythropoiesis and identified that SRSF2 mutations correlate with reduced hemoglobin in JAK2-mutant patients with myeloproliferative neoplasms (MPN). Consistent with this, Jak2V617F-Srsf2P95H versus Jak2V617F mice displayed reduced red blood cell counts and
