Primary Nonrefluxing Megaureter: Does Ureter Diameter Really Matter?

PURPOSE: A ureter diameter exceeding 6 mm is termed megaureter. This study aimed to evaluate the clinical features of primary non-refluxing megaureter (PNMU) and the importance of ureteral diameter in its management. METHODS: Clinical, radiological, and operative findings and long-term outcomes of PNMU followed or operated on between 2011 and 2023 were evaluated retrospectively. Patients with vesicoureteral reflux, a history of lower urinary tract pathology or surgery, and a follow-up <18 months were excluded. RESULTS: Among 32 patients (12.5% female, 87.5% male), 50% were diagnosed prenatal

Robotic Ureteroenteric Stricture Repair: A Decade of Surgical Experience and Technique Evolution.

BACKGROUND: Ureteroenteric strictures (UES) are a challenging complication after urinary diversion and may lead to infection, renal dysfunction, and the need for reintervention. Robotic repair has emerged as an effective minimally invasive option, but data remain limited, particularly in complex and heterogeneous cohorts. METHODS: We conducted a retrospective review of 40 patients who underwent robotic UES repair at a single tertiary center between July 2014 and May 2024. Seventeen patients were treated with the Xi system and 23 with the single-port (SP) platform, all by a single experienced

Recurrent Left Ureterosciatic Herniation With Obstructive Hydronephrosis: A Case Report.

Uretero-sciatic herniation is an exceptionally rare cause of ureteric obstruction, resulting from herniation of the ureter via the sciatic foramen. A 66-year-old woman presented with left flank pain and deteriorating renal function. Computed tomography intravenous pyelogram (CT IVP) demonstrated severe left hydronephrosis secondary to herniation of the ureter through the left sciatic foramen. The patient then underwent successful renal decompression with an endoscopic retrograde ureteric stent placed. Subsequent trial of stent removal failed, with left uretero-sciatic herniation and renal obst

Prophylactic ureteral suturing after ureterolysis in deep endometriosis: description of a conservative laparoscopic technique.

BACKGROUND: To describe a novel laparoscopic technique, prophylactic ureteral suturing (PUS), developed to reinforce structurally weakened ureteral segments following extensive ureterolysis for deep endometriosis, to avoid segmental resection or reimplantation. METHODS: In selected patients with deep ureteral endometriosis, circumferential periureteral fibrosis was dissected after cystoscopic insertion of a double-J ureteral stent. Laparoscopic ureterolysis restored ureteral mobility and revealed a mucosa-only segment, considered at high risk of postoperative stricture or rupture, resulting f

Influence of coil number on the retention strength of ureteral stents: an experimental investigation.

Ureteral stents are temporary implantable devices used to maintain urinary drainage and prevent obstruction. Most designs incorporate pigtail coils to reduce migration; however, the contribution of terminal coil configuration to retention strength remains unclear. This study evaluated the retention behavior of five commercially available Double-J ureteral stents commonly used in Indonesia, representing different intended indwelling durations and terminal coil configurations. Stent length, outer diameter, inner diameter, material, angular sweep, and estimated coil number were recorded. Terminal

SP Retroperitoneal Robotic Nephrectomy via Lower Anterior Approach for Xanthogran-ulomatous Pyelonephritis in a Horseshoe Kidney.

PURPOSE: Xanthogranulomatous pyelonephritis (XGP) represents an uncommon and challenging clinical scenario (1). XGP in a horseshoe kidney poses additional surgical complexity due to the anatomical anomaly (2). Robot-assisted radical nephrectomy (RARN) for XGP has been rarely reported (3), and single-port (SP) RARN via a retroperitoneal lower anterior approach (LAA) represents a recent innovation in the field (4). The aim of this video is to describe a case of SP RARN via LAA in a horseshoe kidney with XGP. MATERIALS AND METHODS: We report the case of a 51-year-old female (BMI 23 kg/m²) with X

Robot-Assisted Boari-flap after Orthotopic Neobladder Using the KangDuo Surgical Robot-01 System.

PURPOSE: Benign ureteroenteric anastomosis stricture (BUES) is a well-recognized long-term complication following urinary diversion (1). While endourological interventions are often first-line, their success rates are limited (2, 3). Open uretero-ileal reimplantation remains the gold standard but is technically challenging and carries high complication risks (2). Robotic surgery offers a promising alternative with comparable success rates and minimally invasive benefits (4). In addition to the da Vinci® system, several new surgical robotic systems have been developed, demonstrating comparable

Electromotive Drug Administration in the Porcine Ureter: First In Vivo Application for Ureteral Dilation.

OBJECTIVE: Retrograde intrarenal surgery is limited by the narrow caliber of the human ureter, resulting in both reluctance to deploy larger ureteral access sheaths and, at times, failed primary ureteral access, requiring placement of a ureteral stent and a delayed second procedure. In this study, we sought to evaluate the application of electromotive drug administration (EMDA) to deliver smooth muscle relaxants to the porcine ureter to facilitate acute ureteral dilation. METHODS: Eighteen juvenile female Yorkshire pigs were randomized into three treatment groups (alfuzosin, aminophylline, or

Innovations in Post-Transplant Urology: A Systematic Review of Robot-Assisted Ureteral Reconstruction Procedures after Kidney Transplantation.

PURPOSE: Ureteral complications are among the most common types of complications following kidney transplantation (KT). In the past, these complications were often managed using endoscopic and open surgical approaches. However, the robot-assisted approach has been recently implemented. Nevertheless, the available data regarding the potential impact of robotic surgery in this field are sparse. This systematic review (SR) aims to summarize the available evidence of robot-assisted ureteral reconstruction following KT, focusing on postoperative outcomes and complications. METHODS: Studies eligibl

Semantic segmentation deep learning model boosts surgeons' organ recognition in minimally invasive hysterectomy - a prospective multi-center reader performance study using pre-selected video clips.

BACKGROUND: Injury to the ureter and bladder during minimally invasive hysterectomy remains a serious complication, often resulting from insufficient intraoperative organ recognition. Artificial intelligence (AI)-based support systems may improve anatomical recognition and reduce such risks. We aimed to evaluate whether an AI-based anatomical recognition system for the ureter and bladder improves surgeons' organ recognition abilities across different experience levels without compromising specificity. MATERIALS AND METHODS: A deep learning model was trained on 13 934 ureter and 4940 bladder i

New Advancements in Prognostic Biomarkers for Upper Tract Urothelial Carcinoma.

As research on upper tract urothelial carcinoma (UTUC) has deepened, the value of biomarkers in the prognostic evaluation of UTUC has been gradually highlighted. As a high-grade epithelial tumor derived from the renal pelvis or ureter, UTUC has a significant prognostic challenge to patients, given its high invasiveness and recurrence rate for the formation of post-operative bladder cancer. To better predict the recurrence and metastasis risk of UTUC, this article provides a comprehensive review of hematologic, urologic, genetic, and histologic biomarker studies on the prognostic assessment of

A Novel Smart Tailorable Degradable Ureteral Stent with Dynamically Reconfigurable Hydrogel Network for Urologic Surgery.

Every year, millions of ureteroscopy patients must return for a second procedure to remove the ureteral stent, doubling both costs and infection risk. Biodegradable stents offer a promising solution by eliminating this second procedure, but achieving an optimal balance between robust mechanical performance, precise degradation kinetics, and excellent biocompatibility remains a major challenge. A novel tailorable biodegradable hydrogel ureteral stent (GAS) designed from Gelatin Methacryloyl (GelMA) and Alginate Methacryloyl (AlgMA), utilizing double network crosslinking and Hofmeister stretchin
