Dietary supplement use throughout the colorectal cancer disease trajectory: a prospective cohort study.

PURPOSE: Dietary supplement use is common in the general population, but literature addressing supplement use throughout the disease trajectory in colorectal cancer (CRC) survivors is limited. METHODS: Data from the COLON study, a prospective cohort among participants diagnosed with stage I-IV CRC, were used. Participants were asked to fill out a dietary supplement questionnaire at diagnosis (n = 1934), 6 months post-diagnosis (n = 1769), 2 years post-diagnosis (n = 1533), and 5 years post-diagnosis (n = 1198) containing questions on type, brand, frequency, and doses of supplements. An Exosome-Based Affinity Screening Platform for Identifying Bioactive Compounds against Colon Cancer.

Exosomes, nanoscale extracellular vesicles carrying native membrane proteins, play crucial roles in intercellular communication and are critically involved in colorectal cancer progression and drug resistance, thereby offering a biologically relevant platform for targeted drug discovery. In this study, we developed an innovative immobilized exosome affinity screening platform (Exo@SiO2-NH2) using HT-29 cell-derived exosomes to selectively capture antitumor bioactive compounds from Sophora flavescens. Through UHPLC-MS/MS analysis, four compounds─Kushenol I, Kurarinone, 2'-O-MethylKurarinone, an

Hemobezoar-related jejunojejunostomy obstruction after minimally invasive Roux-en-Y gastric bypass: a systematic review.

BACKGROUND: Hemobezoar-related jejunojejunostomy (JJ) obstruction is a rare early complication after minimally invasive Roux-en-Y gastric bypass (RYGB). Although early small bowel obstruction after RYGB is well described, obstruction caused by hemobezoar formation remains poorly characterized. METHODS: This systematic review followed PRISMA 2020 guidelines and was registered in PROSPERO (CRD420261320591). PubMed/MEDLINE, Embase, and CENTRAL were searched from inception through February 2026. Eligible studies reported adults undergoing primary minimally invasive RYGB with early hemobezoar-rela

Biomarkers and future directions for immunotherapy in microsatellite-stable colorectal cancer: a systematic review.

The mechanisms of resistance to immune checkpoint inhibitors (ICIs) in microsatellite-stable (MSS) colorectal cancer (CRC) have 3 interconnected levels, which provide information to guide different biomarker strategies. There is a critical clinical need, as objective response rates continue to be < 10%. We suggest a biomarker stratification model on the basis of clinical actionability. POLE/POLD1 mutations, recommended by the NCCN Clinical Practice Guidelines in Oncology for Colon Cancer (Version 1.2025), represent true genomic "hard" indicators. While the benefit of ICI-containing combination

A stratified human colon model reveals SCFA cross-feeding-driven host-microbe dynamics associated with epithelial and immune responses.

Host-microbe interactions are essential for maintaining intestinal homeostasis, yet most in vitro models lack physiological oxygen gradients, mucosal architecture, and reproducibility. We developed the Mucosal Mimetic 3D-Host Aerobic Microbiota Anaerobic Co-culture (Mm 3D-HaeMan) system, a colon-mimetic, oxygen-stratified platform enabling stable co-culture of colonic epithelial cells, human peripheral blood mononuclear cells (hPBMCs), and obligate anaerobes. Oxygen profiling confirmed a stable gradient consistent with human colonic stratification. Using a canonical short-chain fatty acid (SCF

Microsampling for Therapeutic Drug Monitoring and Environmental Toxicology Applications in Clinical Laboratories.

Microsampling workflows for therapeutic drug monitoring and toxicology testing are rapidly increasing as health care explores more patient-centered and decentralized diagnostics. These approaches offer benefits for testing access, including small sample volume, simplified and remote collection. Widespread adoption requires addressing their inherent preanalytical and analytical challenges, such as stability, collection variability, hematocrit and matrix effects, calibration and quality strategies, and correlation with traditional laboratory methods. Although guidance documents are increasingly

Transanal natural orifice specimen extraction for restorative minimally invasive total mesorectal excision, enabled by transanal transection and single-stapled anastomosis: a propensity score-matched study of wound complications and postoperative pain.

BACKGROUND AND AIMS: In restorative total mesorectal excision (TME), transanal rectal transection and single-stapled anastomosis (TTSS) provide an access route that enables transanal NOSE before anastomosis. We evaluated wound complications and postoperative pain associated with SS-NOSE strategy compared with conventional double-stapled anastomosis and transabdominal specimen extraction. METHODS: This single-center, retrospective, propensity score-matched (PSM) cohort study included patients undergoing TME for low rectal cancer, as defined by the Low Rectal Cancer Development Program (LOREC),

Interobserver Variability in Intraoperative Classification of Acute Appendicitis and Postoperative Antibiotic Management.

BACKGROUND: During laparoscopy for suspected acute appendicitis (AA), surgeons classify the appendix as normal, phlegmonous, gangrenous, or perforated, with or without abscess. The surgeon's assessment directly influences postoperative antibiotic treatment and length of hospitalization. There is currently no objective gold standard for this classification. This study aimed to evaluate interobserver variability in the laparoscopic classification of AA. METHODS: Surgeons and surgical trainees viewed video recordings of laparoscopies from 30 patients suspected of AA. Each participant anonymously

A Janus adhesive sealant with extreme acid-tolerance for gastric perforation repair.

Although Janus adhesive sealants hold great potential for gastric perforation repair, current versions fail to achieve stable and durable sealing in highly acidic gastric fluid. Herein, we employ the homogeneous hydrophobic strategy for the first time to develop a Janus adhesive patch with extreme acid-tolerance for gastric perforation repair. The patch is fabricated by unilaterally immersing a Cu2+-crosslinked lipoic acid (LA) gel into a chitosan oligosaccharide (COS) solution (Cu@LA-COS). Owing to the homogeneously hydrophobic poly(LA) network that can durably repel both water molecules and

Comparative genomics and transcriptome analysis reveal unique features of xenobiotic-metabolizing enzymes in elephants.

Elephants, the largest terrestrial mammals, have distinctive metabolic features that may explain their increased sensitivity to xenobiotics, including environmental pollutants and pharmaceuticals. While previous in silico studies have identified unique patterns in elephant cytochrome P450 (CYP), Phase I xenobiotic-metabolizing enzymes, much less is known about their Phase II enzyme systems. This study combines comparative genomic, phylogenetic, and transcriptomic analyses to thoroughly examine Phase I and Phase II xenobiotic-metabolizing enzymes in African (Loxodonta africana) and Asian elepha

Gelatinization differentially impacts in vitro fermentation of high-amylose maize starch.

Some of the starch in a meal, depending on how it was thermally processed, may escape complete digestion in the upper gut and pass into the colon as available for microbial fermentation. This study uses an in vitro batch fecal model to examine the fermentability of high-amylose maize starch (HAMS) that has been gelatinized to different degrees (DG). Compared with native HAMS, more gases and acids were produced from gelatinized HAMS during the initial 4 h of fermentation, which was attributed to the stronger binding of bacteria to gelatinized HAMS. At the endpoint of fermentation, HAMS and HAMS

Macroscopic and Microscopic Organization of the Intestine in the Arabian Jerboa (Jaculus loftusi): Adaptive Significance for Desert Survival.

The Arabian jerboa (Jaculus loftusi) able to thrive in one of the harshest conditions on the earth, where water is scarce and food is often dry and fibrous. To understand how this small desert rodent manages such challenges, we examined the structural and functional modifications of the hindgut in eight adult male individuals collected from Aljouf desert in northern Saudi Arabia. The digestive tract was about 62.7 cm long, nearly five times the jerboa's body length with the small intestine making up the greatest share (55.9%). The large intestine was characterized by a sacculated cecum serving

Biotics-based modulation of gastric precancerous states: microbial, metabolic, and epithelial mechanisms.

Gastric precancerous states are important intermediate stages in gastric carcinogenesis, yet strategies that directly modify their persistent biological drivers remain limited. Current management mainly includes Helicobacter pylori eradication, endoscopic surveillance, and mucosal protective strategies. Biotics, including probiotics, synbiotics, postbiotics, and next-generation microbial products, have been investigated as potential strategies to modulate persistent microbial, metabolic, inflammatory, and barrier alterations. However, existing evidence derives from studies of H. pylori-associa

Gastric microbiota‑derived postbiotics as antimicrobial and anti‑biofilm agents.

BACKGROUND: The human gastric microbiota is recognized as a diverse and metabolically active ecosystem with significant roles in health and disease. Beyond probiotics, postbiotics, non-viable microbial products with antimicrobial and anti-biofilm properties, offer stable and safer therapeutic alternatives. Rising antimicrobial resistance and biofilm associated infections caused by pathogens such as Staphylococcus aureus and Candida albicans highlight the urgent need for new strategies. Based on this evidence, the present study aimed to isolate bacterial strains from gastric biopsies of patient

The role of interleukins and immunological parameters in patients with colon cancer: an optical insight.

The progression of colon cancer from benign adenoma to malignant tumors that invade and spread underscores the critical need for accurate and less invasive biomarkers. The primary objective of this work was to assess the diagnostic potential of combining immunological biomarkers and optical polarimetric characteristics in colorectal cancer. We specifically aimed to evaluate serum cytokine profiles as potential diagnostic tools and to explore tissue optical signatures in a preliminary analysis. Secondary objectives included examining postoperative immune changes and exploring the involvement of
