Neurological Manifestations in People Living with Human Immunodeficiency Virus (PLHIV) in India: A Scoping Review and the Need for a National HIV-Neurology Special Interest Group.

BACKGROUND AND OBJECTIVES: Despite a low national prevalence of 0.20%, India's absolute burden of 2.544 million people living with human immunodeficiency virus (HIV) presents clinicians with varied neurological complications. While combination antiretroviral therapy (ART) has extended survival, the neuro-HIV profile in India, characterized by the predominance of HIV-1 clade C, remains distinct from that reported in Western cohorts. This scoping review aimed to identify patterns of neurological manifestations in India across the pre-ART, ART, and "treat-all" eras and to support the establishmen

Barriers to biomedical care: Qualitative insights into health-seeking behaviors and systemic challenges among people affected by leprosy in Tanzanian communities.

BACKGROUND: Delayed diagnosis in leprosy reflects both how people interpret symptoms and seek care, and how effectively health systems recognize and manage the disease. Qualitative evidence from Tanzania remains limited on how the interaction between patients and health care workers affect provision of care to people affected by leprosy in a timely manner. The main aim of this study was to explore health-seeking behaviors and systemic challenges affecting access to biomedical care among people affected by leprosy in selected Tanzanian communities. METHODS: Ethics Statement: Ethical approval f

Case Report: Analysis of antimicrobial therapy and pharmaceutical care of mixed Aspergillus and Mycobacterium tuberculosis infection following renal transplantation: a case study.

Invasive infection is a significant complication following renal transplantation. Due to long-term immunosuppressive therapy, renal transplant recipients are at high risk for fungal infections and tuberculosis, which are also major causes of mortality in this population. Triazole drugs, as first-line agents for invasive fungal infections, can cause fluctuations in the blood levels of immunosuppressants due to drug-drug interactions. Furthermore, the concomitant use of antituberculosis drugs further complicates the balance between maintaining immunosuppression and anti-infective therapy, often

Monoclonal Antibodies Targeting Bacillus Calmette-Guérin (BCG) Outer Membrane Proteins Confer Protection Against Tuberculosis.

Tuberculosis (TB), caused by the Mycobacterium tuberculosis complex (MTBC), is a persistent global health crisis, remaining the world's leading infectious killer. There were abundant outer membrane proteins (OMPs) distributed in the bacterial cell wall, while fewer proteins were identified and characterized as antigens that could induce protective antibodies against TB. The OMPs of BCG were extracted with the detergent N-octyl-β-D-glucopyranoside, and the extracted proteins were used to immunize mice to generate an antibody library. Subsequently, the library underwent HAT/HT screening, and fiv

Immunogenicity and protective efficacy of recombinant BCG expressing a multi-epitope vaccine against mycobacterium tuberculosis.

INTRODUCTION: Mycobacterium tuberculosis (Mtb) infects approximately one-quarter of the global population, of whom 5%-10% develop active tuberculosis (TB). Although Bacillus Calmette-Guérin (BCG) remains the cornerstone vaccine against severe infant TB, its efficacy in adults is limited, underscoring the need for novel TB vaccines. This study aimed to design a multi-epitope vaccine, tMEV, and validate its protective potential through bioinformatic prediction and in vivo experiments. METHODS: tMEV was constructed from dominant T- and B-cell epitopes derived from Mtb ESAT-6, PE35, PPE38, and PE

Metagenomic insights into the pulmonary microbiome of drug-sensitive and multidrug-resistant tuberculosis patients.

BACKGROUND: While the pulmonary microbiota may influence TB pathogenesis, direct comparisons between drug-sensitive (DS-TB) and multidrug-resistant (MDR-TB) patients remain scarce. This study explored microbiome differences between these groups. METHODS: Bronchoalveolar lavage fluid (BALF) from 30 prospective patients (11 DS-TB, 19 MDR-TB) was analysed via metagenomic sequencing. RESULTS: Patients with MDR-TB showed lower α-diversity and distinct β-diversity compared with patients with DS-TB. Streptococcus and Prevotella were relatively enriched in DS-TB, whereas Staphylococcus, Cutibacteriu

Inflammasome-associated immune dysregulation in TB-HIV coinfection.

BACKGROUND: Tuberculosis (TB) and HIV co-infection are characterized by dysregulated inflammation and increased risk of immune reconstitution inflammatory syndrome (IRIS) following antiretroviral therapy (ART) initiation. Inflammasomes are key regulators of innate immune responses, but their role in TB-HIV co-infection and IRIS remains incompletely understood. We investigated inflammasome-related gene expression and cytokine profiles in TB, HIV-1, and TB-HIV individuals, and their association with IRIS development. METHODS: Fifty-nine participants were stratified into four groups: TB-HIV (n =

Correlation between bacterial culture and IS900 qPCR to detect Mycobacterium avium subsp. paratuberculosis in the feces of sika deer.

Mycobacterium avium subsp. paratuberculosis (MAP) infection in deer causes paratuberculosis (PTb; Johne's disease), a slowly progressive chronic granulomatous enteritis. Accurate and rapid detection of MAP shedding by subclinical animals is essential for effective control of infection in deer herds. The VetMAX MAP 2.0 qPCR assay targeting IS900 was developed for cattle, sheep, and goats. However, the performance of the assay in sika deer (Cervus nippon) has not been evaluated. Here, we describe the use of the VetMAX MAP 2.0 qPCR assay on fecal samples from sika deer for the detection of MAP an

Cerebral Manifestation of Miliary Tuberculosis Mimicking a Parasitic CNS Infection.

The neuroradiological findings of central nervous system (CNS) manifestations of miliary tuberculosis can be misinterpreted as parasitic infections due to their low prevalence in the European population. We present the case of a 30-year-old woman in whom CNS tuberculosis was initially misdiagnosed as a parasitic infection due to the absence of pulmonary symptoms. She immigrated to Germany from Madagascar and had lived in Europe for 14 months by the time she presented to our department with an incidental MRI finding suggestive of a manifestation of toxoplasmosis. The T1-weighted axial brain MRI

Drug-resistant tuberculosis in Nepal: the sixth national prevalence drug-resistant survey, 2024.

BACKGROUND: Tuberculosis (TB) remains a major public health challenge in Nepal, characterized by increasing drug-resistant TB (DR-TB) and constrained surveillance capacity. The sixth national DR-TB prevalence survey was therefore implemented to address this gap. This study aimed to estimate the national prevalence of DR-TB and identify risk factors among new and previously treated pulmonary TB patients. METHODS: A national cross-sectional survey was conducted from December 2022 to January 2024, enrolling bacteriologically confirmed pulmonary TB patients using stratified cluster sampling at 49

Subnational spatial co-distribution of tuberculosis and diabetes mellitus across Ghana's 16 regions following administrative reorganisation: An ecological analysis.

BACKGROUND: National administrative boundary reorganisations, increasingly common across sub-Saharan Africa, create discontinuities in surveillance data alignment that complicate subnational burden estimation for infectious and non-communicable diseases alike. Ghana's expansion from 10 to 16 administrative regions in 2019 provides a tractable case for demonstrating how publicly available surveillance and survey data can be used to characterise subnational co-distribution of tuberculosis and diabetes mellitus (DM) under a revised boundary structure. DM is an established risk factor for active T

A Wolf in Disguise: Tuberculous Spondylitis Masquerading as Modic Changes With Reactivation After Epidural Steroid Injection - A Case Report.

BACKGROUND: Epidural steroid injections (ESIs) are commonly used for chronic low back pain but can rarely lead to serious infectious complications. Reactivation of latent tuberculous spondylodiscitis after ESI is uncommon. Our case highlights diagnostic challenges of underlying spinal infection prior to interventional pain procedures. CASE REPORT: A 43-year-old woman presented with low back and radicular pain. There were no constitutional symptoms, and baseline laboratory findings were unremarkable. Lumbar magnetic resonance imaging (MRI) showed Modic-type degenerative changes and nerve root

Outcomes of non-tuberculous mycobacterial infection of the bone, joint and soft tissue: a seven-year multicentre retrospective study.

PURPOSE: Extrapulmonary nontuberculous mycobacterial (EPNTM) infections of bone, joint, and soft tissue are difficult to treat and lack species-specific outcome data. We sought to describe antimicrobial regimens and surgical management by Mycobacterium spp. and to identify predictors of a clinical cure within 12 months in this adult cohort with EPNTM. METHODS: We conducted a multicentre retrospective cohort study across five tertiary hospitals in Australia and New Zealand 2014-2020. Adults with culture-confirmed EPNTM infection from sterile or intraoperative specimens were included. Primary o

Identification of mitochondrial metabolism-associated tuberculosis key genes and construction of a diagnostic model based on WGCNA.

Despite recent advances, tuberculosis (TB) remains a complex global health challenge, with its causative bacterium, Mtb, actively manipulating host cellular metabolism to survive. Therefore, studying mitochondrial metabolism-related genes is crucial, as they offer a promising avenue for identifying diagnostic biomarkers and developing new host-directed therapies to combat the disease. This study integrated transcriptomic datasets from three TB cohorts in the Gene Expression Omnibus (GEO) database (training set: GSE83456; validation sets: GSE42826 and GSE19444). Differential analysis was perfor

Targeting Latent Tuberculosis Using Hypoxia-Activated Prodrugs of Bedaquiline Analogues.

The increasing emergence of antimicrobial resistance (AMR) is reducing the effectiveness of existing antibiotics against bacterial infections, including tuberculosis (TB), a deadly disease caused by Mycobacterium tuberculosis (M.tb). Approximately 90% of infected individuals develop latent tuberculosis infection, for which current treatment options are limited. Latent tuberculosis infection is characterised by the formation of hypoxic granulomas, which restrict oxygen and drug penetration, highlighting the need for therapies effective under hypoxic conditions. In this study, a hypoxia‑activate
