When the Cure Turns Into a Culprit: Mesalamine and the Eosinophilic Lung.

We report a case of a 74-year-old man with a one-year history of ulcerative colitis in remission who developed low-grade fever, dry cough, progressive dyspnea, loss of appetite, and an eight-kg weight loss over 12 weeks. Oral mesalamine (2.4 g/day) had been initiated 12 weeks before presentation, with respiratory symptoms developing approximately three weeks after initiation. Despite initial antibiotic therapy, his symptoms progressively worsened, and he developed hypoxemia requiring hospitalization. Investigations revealed leukocytosis with marked peripheral eosinophilia, elevated C-reactive

Cardiac Manifestations of EGPA: An Overlap Phenotype of Inflammatory Myocardial Injury and Takotsubo-Like Syndrome.

BACKGROUND: Eosinophilic granulomatosis with polyangiitis (EGPA) often presents with eosinophilic myocarditis, but the spectrum of cardiac findings is scarcely reported. We describe a case of EGPA with an unusual presentation of acute left ventricular (LV) dysfunction associated with lymphocyte-predominant myocardial inflammation and Takotsubo-like syndrome with LV thrombus. CASE SUMMARY: A 42-year-old woman with history of asthma presented with chronic cough, dyspnea, and rash. Findings of peripheral eosinophilia, cutaneous vasculitis, mononeuritis multiplex, and myocardial inflammation conf

Unilateral eosinophilic fasciitis: a case report and review of the literature.

A 59-year-old Japanese man developed pruritus and swelling of the right dorsal foot after weeding his garden in a crouching position for approximately one hour. Over a period of five months, the swelling, as well as skin induration, progressed unilaterally from the right thigh to the lower leg and dorsal foot. Laboratory tests, including eosinophil fraction and inflammatory markers, were within normal limits, and computed tomography showed no evidence of deep vein thrombosis or malignancy. A full-thickness biopsy including the fascia revealed marked fascial thickening with fibrosis of the subc

An elusive diagnosis: pericardial effusion and hypereosinophilia revealing seronegative eosinophilic granulomatosis with polyangitis: a case report.

Eosinophilic granulomatosis with polyangiitis (EGPA) is a small- and medium-vessel necrotizing vasculitis characterized by asthma, eosinophilia, and multiorgan involvement. Cardiac manifestations, though uncommon, are more prevalent in antineutrophil cytoplasmic antibody (ANCA)-negative patients and carry significant morbidity. Diagnosis can be challenging due to variable presentations and the lack of a definitive test. We present a 60-year-old woman with asthma and chronic rhinosinusitis who developed progressive dyspnea and was found to have a large pericardial effusion with tamponade physio

Beyond the Induction Phase: Should We Keep the Proton Pump Inhibitor in Pediatric Eosinophilic Esophagitis?

BACKGROUND: Proton pump inhibitors (PPIs) are a common and effective management strategy for eosinophilic esophagitis (EoE) leading to successful remission in most cases. However, there is uncertainty regarding the ideal follow-up pharmacological treatment to maintain remission. We performed a cost-effectiveness and quality-adjusted life year (QALY) analysis. OBJECTIVES: To compare various approaches of pharmacologic treatments for EoE. METHODS: A Markov model was constructed to evaluate the cost-effectiveness and QALY of maintenance therapies for eosinophilic esophagitis, including PPI, top

Macrophages in non-infectious uveitis: cell origin, disease stage, evidence provenance, and therapeutic maturity.

Non-infectious uveitis (NIU) comprises a heterogeneous group of immune-mediated disorders in which myeloid cells influence leukocyte entry, inflammatory amplification, tissue injury, resolution, and relapse. The conventional dichotomy of classically activated (M1) and alternatively activated (M2) macrophages does not adequately capture this functional diversity. This narrative review organizes the evidence along three complementary dimensions: cell origin, disease stage, and evidence provenance; therapeutic maturity is evaluated separately. Experimental data support distinct roles for resident

Diffuse esophageal eosinophilia is associated with increased endoscopic severity and topical steroid non-response in patients with eosinophilic esophagitis.

BACKGROUND AND AIMS: Variability of esophageal eosinophil distributions and its impact on disease presentation and treatment outcomes in eosinophilic esophagitis (EoE) is unclear. We aimed to determine whether isolated proximal or distal involvement differs from diffuse involvement in EoE presentation and treatment outcomes. METHODS: This retrospective cohort study analyzed patients of all ages with newly diagnosed EoE from the UNC EoE Clinicopathologic database. Patients were classified as distal esophagus-only (≥15 eos/hpf), proximal-only (≥15 eos/hpf), and diffuse involvement (≥15 eos/hpf

[ASSOCIATION BETWEEN PEDIATRIC EOSINOPHILIC GASTROINTESTINAL DISORDERS AND NEURODEVELOPMENTAL DISORDERS: A CLINICAL INVESTIGATION].

BACKGROUND: Eosinophilic gastrointestinal disorders (EGIDs) are classified into eosinophilic esophagitis (EoE) and non-EoE EGIDs, with the latter being more common in Japan. Although recent studies have suggested an association between EGIDs and neurodevelopmental disorders (ND), domestic data remain limited. This study examined the relationship between ND and non-EoE EGIDs. METHODS: We evaluated 33 children diagnosed with non-EoE EGIDs and determined the prevalence of comorbid ND. Clinical characteristics-including sex, age at onset, treatment patterns, and relapse-were examined according to

The impact of gluten-containing and gluten-free diets on intestinal inflammation: Evaluation of fecal calprotectin in a healthy rat model.

OBJECTIVE: Gluten-free diets are increasingly adopted by individuals without gluten-related disorders, despite limited evidence for anti-inflammatory benefit. We evaluated whether varying gluten exposure influences intestinal inflammation in healthy rats using fecal calprotectin (fKp) as the outcome. PATIENTS AND METHODS: Forty newly weaned male Wistar albino rats were randomized to four isocaloric, iso-nitrogenic diets for 12 weeks (n=10/group): (1) standard wheat-containing control, (2) gluten-free animal protein-based diet, (3) control diet supplemented with 6% vital wheat gluten, and (4)

Mapping PANSS factor dynamics: a novel framework for tracking therapeutic response in schizophrenia.

OBJECTIVE: To study the factor structure of PANSS score dynamics in patients with schizophrenia spectrum disorders (SSD) and its relationship to individual patient characteristics. METHODS: A single-center retrospective observational study was conducted. We analyzed medical records of patients with SSD from Russian Mental Health Research Center (Moscow, Russia), in whom neuroinflammation biomarkers had been monitored and item-level PANSS scores were available. Dynamics of individual PANSS items after therapy adjustment were examined via exploratory factor analysis (FA; n=182). The relationshi

Lactoferrin as a Natural Modulator of Immunity and Iron Homeostasis: A Review of Current Knowledge.

Background Lactoferrin is an iron-binding glycoprotein found in secretions and neutrophil granules that regulates iron availability in conjunction with host defense. Furthermore, it modulates innate and adaptive immune responses, and affects iron handling during inflammation. Consequently, there is renewed interest in lactoferrin because of its potential therapeutic applications in iron deficiency, inflammatory disorders, and infectious diseases. Summary This review summarizes the current knowledge regarding lactoferrin as a modulator of immunity and iron homeostasis, with an emphasis on recen

Clinical and Laboratory Dynamics of Bear Meat-Associated Trichinellosis in Hokkaido, Japan.

Trichinellosis is a rare infection associated with bear meat consumption found in Japan, where limited access to parasite-specific antibody testing makes definitive diagnosis challenging. In the present study, 19 patients, comprising 16 confirmed and three probable cases, treated at a tertiary care hospital in Hokkaido, Japan, during three independent outbreaks in 2018, 2019, and 2025, were retrospectively analyzed. Patients prepared brown bear meat as a roast (n = 18) or a cutlet (n = 1) and consumed it as a main dish (n = 7) or in smaller portions of a few slices (n = 12). Ten patients consu

Exploration of miRNAs promoting eosinophilia and type 2 immunity.

BACKGROUND: MicroRNAs (miRNAs) are evolutionary conserved, small non-coding RNAs that regulate gene expression and modulate cellular responses. In this study, we profiled miRNAs regulated by interleukin-33 (IL-33), a key inflammatory mediator of allergic responses and type 2 immunity that promotes bone marrow eosinophil development and subsequent airway recruitment. METHOD: C57BL/6 mice were exposed to recombinant IL-33 intranasally to induce airway eosinophilia. Cellular composition was determined by differential cell counts in bronchoalveolar lavage fluid and bone marrow. RNA profiling was

The Use of Montelukast in Pediatric Asthma and Anti-Inflammatory Effects - Easy Choice or a Balancing Act?

Montelukast is a selective cysteinyl leukotriene receptor 1 antagonist with an established, though nuanced, role in the management of asthma and allergic rhinitis. By inhibiting leukotriene-mediated signaling, montelukast targets a pathway involved in bronchoconstriction, mucus secretion, vascular permeability, eosinophil recruitment, cytokine release, and airway remodeling. Available evidence indicates that montelukast may reduce several markers of allergic inflammation, including fractional exhaled nitric oxide, peripheral and tissue eosinophilia, eosinophil-derived mediators, adhesion molec
