INTRODUCTION: Neurological manifestations in patients with inborn errors of immunity (IEI) may occur during the disease course or, in some cases, represent the initial presenting feature. Increased awareness of these manifestations may facilitate earlier diagnosis and more effective management of IEIs, thereby preventing irreversible neurological morbidity. This study aimed to investigate the relationship between IEI subtypes and neurological involvement and to highlight the role of neurological symptoms in early diagnosis.
METHODS: This study included patients younger than 18 years who were

Severe combined immunodeficiency (SCID) pigs have become a promising large animal model for biomedical research, offering significant advantages over traditional mouse models due to their anatomical, physiological, and genetic similarities to humans. Humanized SCID pig models can potentially improve preclinical research in areas such as cancer immunotherapies, stem cell therapies, and transplantation methods, yet often lack significant lymphocyte development, including evidence of B cell and myeloid cell development. This work aims to increase the extent of humanization of the SCID pig. CRISPR

BACKGROUND: Although developed to identify severe combined immunodeficiency (SCID), T cell receptor excision circle (TREC)-based newborn screening also detects T cell lymphopenia of varying etiologies.
OBJECTIVE: To identify perinatal factors associated with abnormal TRECs in infants without SCID or thymic aplasia/hypoplasia.
METHODS: Newborns (birthdates: October 2011-December 2021) with abnormal TRECs, without SCID or thymic aplasia/hypoplasia, were compared with normal TREC controls matched for gestational age, birthweight, and neonatal intensive care unit status.
RESULTS: A total of 949

BACKGROUND AND SIGNIFICANCE: The interleukin-2 receptor gamma chain gene (IL2RG) encodes for the common γ chain (γc) protein, that is a shared signaling component of multiple interleukin receptors, including IL-2, IL-4, IL-7, IL-9, IL-15, and IL-21, and plays a pivotal role in lymphocyte development, homeostasis, and function. Mutations in IL2RG cause X-linked severe combined immunodeficiency (X-SCID) and a broad spectrum of related phenotypes ranging from typical SCID to leaky or atypical presentations, sometimes mimicking common variable immunodeficiency or immune dysregulation syndromes. INTRODUCTION: Common Variable Immunodeficiency (CVID) is a heterogeneous immune disorder characterized by a broad range of clinical manifestations. Its etiology is not yet fully understood. To gain insights into the immunological background of CVID in patients without a clearly defined genetic cause, we employed a genealogical research approach that included family members. This strategy aims to provide a more comprehensive understanding of immune dysregulation in CVID, of potential hereditary patterns, and subclinical immunological traits within families.
METHODS: Fifty-eight participants fr

BACKGROUND: Restricted mean survival time (RMST) endpoints are becoming commonly used as trialists look to analyse time-to-event outcomes without the restrictions of the proportional hazards assumption. An additional benefit of RMST endpoints which has so far remained unexplored is their capability to combine treatment main effects and treatment-by-covariate interaction terms into single one-dimensional estimators under both proportional and non-proportional hazards. By utilising RMST estimators, trialists may assess treatment effects associated with multiple covariates, including interaction

Allogeneic haematopoietic stem cell transplantation (HSCT) is a curative therapy for severe combined immunodeficiency (SCID). Conditioning improves donor engraftment and freedom from immunoglobulin replacement (IgR) but increases the risks of acute and late toxicity. Treosulfan, a reduced toxicity alkylating agent, has emerged as an alternative to busulfan. In this UK multicentre study, we evaluated outcomes of 104 infants with SCID who underwent first HSCT following treosulfan-fludarabine conditioning between 2006 and 2022. After a median follow-up of 5.4 years, 5-year overall survival (OS) a

INTRODUCTION: Defects in RAG1/RAG2-mediated V(D)J recombination cause severe combined immunodeficiency (SCID), a disorder characterized by the absence of mature T and B lymphocytes resulting from impaired antigen receptor gene rearrangement. Although evolutionary analyses demonstrate strong conservation of the catalytic core domains of RAG1/RAG2 across jawed vertebrates, structural divergence among species highlights the need for physiologically relevant large-animal models to evaluate human disease mechanisms. Pigs represent an advantageous model for human pathologies because of their close i

Transplantation of donor hematopoietic stem and progenitor cells (HSPCs) is a well-established curative treatment for various blood and immune diseases, including severe combined immunodeficiency (SCID). However, it comes with significant toxicities, including graft-versus-host disease (GvHD) and tissue damage resulting from the use of genotoxic chemotherapy-containing conditioning regimens. Autologous transplantation using gene-modified HSPCs eliminates GvHD but currently still relies on genotoxic conditioning. Further, gene modification of HSPCs has commonly utilized integrating viruses, whi

This review finds that knowledge of primary immunodeficiencies (PID), also termed inborn errors of immunity (IEI), has expanded rapidly with advances in genetic and immunological methods. Precise diagnosis is crucial for targeted treatment in this patient group. Patients may present not only with infections but also with immune dysregulation such as autoimmunity, inflammation, or malignancy, occurring in both childhood and adulthood. Whole genome sequencing combined with functional immunological analyses has greatly improved diagnostics and guided therapy, leading to significantly better progn

Autoimmune and primary immunodeficiency disorders represent a growing global health burden influenced by a complex interplay of genetic, environmental, and socioeconomic factors. This paper offers a geoepidemiological analysis of these conditions, highlighting regional differences in incidence, diagnosis, and outcomes. Immunodeficiencies such as Common Variable Immunodeficiency (CVID), complement deficiencies, Selective IgA Deficiency, and Severe Combined Immunodeficiency illustrate the influence of healthcare access and genetic variation on detection and management. For instance, CVID is disp

Recombination-activating genes (RAG1 and RAG2) encode lymphoid-specific proteins that are essential for V(D)J recombination during early T- and B-lymphocyte development. Biallelic mutations in these genes result in a broad spectrum of primary immunodeficiency phenotypes, ranging from classical severe combined immunodeficiency (SCID) to combined immunodeficiency, immune dysregulation, autoimmunity, and inflammatory complications. The immunological phenotype varies widely, from T-B-NK+ severe combined immunodeficiency (SCID) to combined immunodeficiency (CID), or near-normal T and B cell counts,

BACKGROUND: There is a lack of clinical data on hematopoietic stem cell transplantation (HSCT) in Ataxia-Telangiectasia (A-T) patients due to the underlying chromosomal instability that leads to low tolerance to chemotherapy. To effectively manage cancer and immune risks, there is a need for improved HSCT protocols, novel therapies, and long-term monitoring. This report describes a 16-year-old boy with A-T and T-ALL who achieved long-term leukemia-free survival after HSCT using a tailored, drug-monitored conditioning regimen. His results were analyzed in the context of a systematic review of t

PURPOSE: Cancer risks of individuals heterozygous for an ATM pathogenic or predicted pathogenic variant (PV/PPV) remain imprecise to guide optimal clinical management. Therefore, we aimed to estimate these risks in different family settings.
METHODS: Data were collected on 141 ataxia-telangiectasia families, 398 hereditary breast and ovarian cancer families, and 96 families with a history of pancreatic cancer enrolled in French nation-wide epidemiological studies CoF-AT2, TUMOSPEC, or GENESIS. Hazard ratios (HR) and cumulative risks were estimated using a modified segregation analysis method.

BACKGROUND: POLD1 defect with immunodeficiency is characterized by T-cell and NK-cell lymphopenia, with the patients having a higher susceptibility to herpetic and viral respiratory tract infections. Patients considered for HSCT may be at increased risk of regimen-related toxicity due to impaired DNA repair. Here, we describe the first case of successful HSCT in a patient with POLD1 deficiency who presented at a relatively older age for immunodeficiency and underwent HSCT despite concerns about DNA-related toxicity from the conditioning regimen.
METHODS: We report the first successful HSCT in

Good's syndrome (GS), a rare immunodeficiency disorder characterized by thymoma and hypogammaglobulinemia, presents diagnostic and therapeutic challenges due to recurrent infections. We report a 53-year-old male farmer with GS complicated by recurrent pulmonary infections and COVID-19. Initial management focused on antiviral/anti-infective therapy and corticosteroids, but persistent hypogammaglobulinemia, B-cell depletion, and thymoma history were overlooked. Diagnosis was confirmed upon integrating the thymoma history, immunological profiling, and bronchial alveolar lavage-next generation seq
