Exploring a Single Music Therapy Session to Reduce 22q11.2 Deletion Syndrome Child and Adolescent's Outpatient Clinic-Related Anxiety.

AIM: This study explored whether a single music therapy session could reduce 22q11.2 deletion syndrome (DS) children and adolescents' clinic-related anxiety when visiting the hospital outpatient clinic for health appointments. METHODS: The research employed a within-subjects, single-session, intervention design. The effect of the 30-min music therapy session on children and adolescents' (henceforth children) anxiety was assessed at baseline and the end of the session. A triangulated approach examined children's anxiety through parent and child self-report questionnaires, electrocardiograph (h

Time poverty of families of children with 22q11DS: a healthcare professional perspective.

INTRODUCTION: 22q11 deletion syndrome (22q11DS) is the most common microdeletion in humans, leading to a wide range of variable clinical manifestations that can affect any part of the body. Most individuals with 22q11DS are diagnosed during childhood and the responsibility of care falls on their primary caregivers, usually their parents, who need to balance the needs of all family members including themselves. This affects parental wellbeing. This study investigates the perspective of healthcare professionals (HCPs), in Europe and Canada, on the parents' struggle with time invested in caring f

A Novel Method for the Evaluation of Hypercalciuria in Individuals With Chronic Hypoparathyroidism.

OBJECTIVES: Guidelines recommend avoiding hypercalciuria during treatment of chronic hypoparathyroidism, but measurements of 24-hour calcium excretion (24-hour urine calcium [24uCa]) have limitations. We evaluated calcium excretion per volume of filtrate (ECa/Ccr), which does not require timed collections, as an alternative to 24uCa to detect hypercalciuria. METHODS: This was a post-hoc analysis of the PaTH Forward trial (NCT04009291). Fifty-nine adults with chronic hypoparathyroidism managed with conventional therapy were enrolled in a randomized, double-blind, placebo-controlled 4-week peri

Site-specific fracture risk in chronic hypoparathyroidism: systematic review and meta-analysis.

Chronic hypoparathyroidism (HypoPT) is characterized by sustained parathyroid hormone (PTH) deficiency, leading to reduced bone turnover, increased bone mineral density (BMD), and altered skeletal microarchitecture. Despite preserved or elevated BMD, fracture risk in HypoPT remains controversial. We conducted a systematic review and meta-analysis to assess site-specific fracture risk and the influence of disease duration in patients with chronic HypoPT. Fifteen observational studies, including 13,563 HypoPT subjects and 137,245 controls, were analyzed. HypoPT subjects had a significantly highe

Intratrabecular bone remodeling - a previously overlooked mode of remodeling hyperactivated by parathyroid hormone.

Currently, no consensus exists on either terminology, definition, or the biological significance of the phenomena intratrabecular tunneling. Despite this, observations of intratrabecular tunneling are frequently reported in literature covering diseases or treatments involving parathyroid hormone or kidneys. A few attempts to quantify this phenomena have been made mainly based on bone resorption, despite the concurrent presence of bone formation. This study demonstrates that intratrabecular tunneling is a previously unrecognized physiological mode of intratrabecular bone remodeling occurring ac

Impact of cervical thymectomy on multiple endocrine neoplasia type 1 patients undergoing parathyroidectomy for primary hyperparathyroidism.

BACKGROUND: Cervical thymectomy is commonly performed during parathyroidectomy for primary hyperparathyroidism in multiple endocrine neoplasia type 1. Recently, there have been several single-institutional studies that have found no added benefit to the addition of thymectomy and question its impact on postoperative outcomes. We aimed to evaluate the outcomes of concomitant cervical thymectomy in multiple endocrine neoplasia type 1 patients undergoing parathyroidectomy for primary hyperparathyroidism using a large cohort of patients. METHODS: This retrospective study used the TriNetX research

[Observation of an index case of an autosomal dominant familial hypoparathyroidism revealed by recurrent hypocalcemia: a case report].

Primary hypoparathyroidism is a rare disorder responsible for recurrent hypocalcemia, the identification of which may lead to family screening. It can present with sensory and neuromuscular clinical manifestations that may, in severe cases, be life-threatening, or it may remain asymptomatic and be discovered incidentally. Its etiologies are diverse, and the main challenge lies in genetic investigation. Management must be both symptomatic and etiological. We here report the case of a 42-year-old woman presenting with recurrent hypocalcemia associated with low parathyroid hormone (PTH) levels, w

[New interdisciplinary S3 guidelines on deletion syndrome 22q11.2, a genetic disease predisposing to neuropsychiatric morbidity].

The deletion syndrome 22q11.2 is one of the most frequent genetic syndromes in humans, with a prevalence of 1:2500 [1]. The syndrome is particularly relevant for neurology, psychiatry and child and adolescent psychiatry due to a massive genetic predisposition to neuropsychiatric disorders that has been proven in recent years. Somatic symptoms are highly variable and can be diverse. Congenital heart defects, otorhinolaryngologic (ENT) abnormalities, parathyroid dysfunction and orthopedic abnormalities are particularly frequent and typical. In addition, many affected individuals suffer from susc

Continuous subcutaneous recombinant PTH(1-34) infusion improves serum calcium and phosphate homeostasis in children with autosomal dominant hypocalcemia type 1 refractory to standard-of-care treatment.

OBJECTIVE: Autosomal dominant hypocalcemia type 1 (ADH1) results from activating CASR variants, causing parathyroid hormone (PTH) suppression and abnormal renal calcium handling. Management is challenging, as hypoparathyroidism is associated with inappropriate renal calcium wasting, leading to hypercalciuria, nephrocalcinosis, and renal impairment. We evaluated long-term outcomes of continuous subcutaneous PTH(1-34) infusion (CSPI) in children with ADH1 refractory to standard of care therapy (SOC). DESIGN: Single-center observational study. METHODS: Five children with genetically confirmed A

Hypoparathyroidism Post Thyroidectomy: Prevention, Evaluation, Management, and Application of Recent Guidelines.

BACKGROUND: Postsurgical hypoparathyroidism (HypoPT) accounts for ∼75% of all HypoPT cases. It is recognized that this condition is largely preventable. Several studies have identified risk factors increasing the risk of developing postsurgical HypoPT, including patient-related factors, surgical factors, and, in some studies, presurgical factors. This narrative review highlights evidence-based guidelines for postsurgical HypoPT and provides strategies for its prevention, diagnosis, and management. METHODS: We searched PubMed, Embase, Google Scholar, and the Cochrane Library from inception thr

PTH-Independent CaSR Inhibition with the Oral Calcilytic Encaleret Improves Calcium Homeostasis in Post-Surgical Hypoparathyroidism: A Phase 2 Open-Label Proof-of-Concept Trial.

The calcium-sensing receptor (CaSR) maintains calcium homeostasis by regulating parathyroid hormone (PTH) secretion and directly modulating renal calcium reabsorption. In the kidney, PTH and renal CaSR signaling exert opposing effects on tubular calcium reabsorption and urinary calcium excretion, complicating efforts to define their independent contributions to overall calcium regulation. To examine the PTH-independent effects of CaSR inhibition on renal calcium handling, we administered a calcilytic (CaSR antagonist) to individuals with post-surgical hypoparathyroidism (PSH).
