Laser microdissection-based multi-omics integration unveils the pathological atlas and tumor differentiation network of adamantinomatous craniopharyngioma.

Although adamantinomatous craniopharyngioma (ACP) is a histologically benign intracranial tumor, its invasive growth pattern, characterized by finger-like protrusions, leads to a poor prognosis. In-depth analysis of the heterogeneity of the pathological structure of ACP and the development and evolution of tumor cells is crucial for expanding precision treatment strategies. However, a detailed understanding of the complex pathological structural characteristics of ACP is lacking. Here, we used laser microdissection to obtain 40 samples from 6 different pathological structures for Smart-Seq2 se

Deep learning-based classification of gh-secreting pituitary neuroendocrine tumors granulation patterns using H&E-stained histopathology.

Accurate classification of granulation patterns in growth hormone-secreting pituitary neuroendocrine tumors (GH-PitNETs) is clinically essential but traditionally dependent on labor-intensive immunohistochemistry (IHC). To address this, we developed the Cross-Attention Pituitary Neuroendocrine Tumor Network (CA-PITNET), a deep learning model for automated subtyping directly from routine H&E-stained whole-slide images (WSIs). Utilizing 95 GH-PitNETs WSIs (54,610 patches) and 36 nonfunctioning PitNET WSIs (39,590 patches), the model was evaluated on two tasks: distinguishing GH-PitNETs from nonf

Clinicopathological evaluation of risk factors for recurrence in craniopharyngioma.

OBJECTIVE: Craniopharyngiomas are benign tumors that often exhibit aggressive clinical behavior. The aim of this study was to evaluate long-term surgical outcomes, identify risk factors for recurrence, determine predictors of subtotal resection (STR), and characterize multiple recurrence patterns to optimize treatment strategies. METHODS: The authors retrospectively analyzed 269 craniopharyngioma surgeries performed in 225 patients between 2010 and 2024, with a median follow-up of 73.0 months. Each surgery was classified as primary, first recurrence, second recurrence, or third and subsequent

MRI predictive value for adenoma detection in Cushing's disease and reader dependence: a multi-observer study.

OBJECTIVE: MRI detection of pituitary microadenomas in Cushing's disease is inconsistent, and its impact on surgical outcomes remains debated. Diagnostic performance may vary by reader expertise and interpretation strategy. The authors sought to assess interrater agreement and compare diagnostic accuracy across expert MRI interpretation conditions. METHODS: In this retrospective study at a tertiary referral center, preoperative pituitary MRI from 90 patients with biochemically confirmed Cushing's disease who underwent surgery (2012-2022) were independently interpreted by a trained pituitary n

Peptides in urology and sexual medicine: clinical effectiveness, dosing, indications, and safety: a narrative review.

INTRODUCTION: Peptide-based therapies are increasingly used in urology and sexual medicine, although adoption is often influenced by off-label prescribing, direct-to-consumer promotion, and limited long-term safety data. We review mechanisms, clinical evidence, dosing, safety, and regulatory status of these agents. METHODS: A structured search of PubMed, Scopus, and Google Scholar identified studies of peptide-based therapies in sexual medicine. Search terms included Bremelanotide, Melanotan II, Kisspeptin, Oxytocin, PnPP-19, Gonadorelin and growth hormone secretagogues, including GHRP-2, GHR

A neural circuit of food sight-promoted feeding and contextual memory in mice.

Feeding is regulated not solely by intrinsic energy demands, but also by various environmental cues. Food sight and odor are two essential food-related cues that promote human eating; nonetheless, the underlying neural mechanisms remain unknown. Here, we show that during energy deficit, food sight inhibits the ventromedial hypothalamus (VMH) neuronal activity, thereby promoting feeding and food-associated contextual memory in mice. VMH neurons receive direct GABAergic input from the suprachiasmatic nucleus (SCN) that is rapidly activated by food sight. Either knockout of VMH RuvB-like 2 (RUVBL

Prolactinomas in women of reproductive age: a proposed reproductive-intent-based framework for diagnosis and management.

Prolactinomas are the most common functioning pituitary tumors in women of reproductive age and can affect fertility, gonadal and skeletal health, and vision. Management becomes particularly complex when reproductive plans intersect with diagnostic uncertainty, tumor anatomy, pregnancy-related enlargement, and long-term treatment decisions. This focused narrative Mini Review synthesizes current guidelines, consensus statements, systematic reviews, and clinically informative cohort studies and proposes a reproductive-intent-based framework for organizing individualized care. Adequate diagnostic

Acromegaly as the Initial Presentation of Multiple Endocrine Neoplasia Type 1: A Case Report.

BACKGROUND Multiple endocrine neoplasia syndrome type 1 (MEN-1) is a rare autosomal-dominant disorder, caused by MEN-1 gene mutations, leading to tumors in multiple endocrine organs, classically the parathyroid glands, the anterior pituitary, and the pancreatic islet cells. Although primary hyperparathyroidism is usually the earliest manifestation, growth hormone-secreting pituitary adenomas rarely present as the initial clinical feature, potentially delaying recognition of the syndrome. CASE REPORT A 34-year-old woman presented with acromegalic features and hypercalcemia. Laboratory testing a

Leptin-sensitive hypothalamic Lepr+/Glp1r+ neurons limit hyperphagia and weight gain in diet-induced obesity.

Despite loss of responsiveness to exogenous leptin in obesity, endogenous leptin continues to restrain feeding, although the neural substrates that remain sensitive and mediate this effect remain unknown. Combining spatial transcriptomics with single-nucleus RNA sequencing in mice with diet-induced obesity (DIO), we here show that while most hypothalamic leptin receptor (Lepr)-positive neurons minimally respond to elevated leptin, a single subpopulation defined by glucagon-like peptide-1 receptor (Glp1r) co-expression retains robust leptin sensitivity. These Lepr+/Glp1r+ neurons project onto a

Pituitary adenomas (PitNETs): molecular pathophysiology, targeted drug delivery strategies and clinical therapeutic outcomes.

Pituitary neuroendocrine tumors (PitNETs), historically referred to as pituitary adenomas, comprise a heterogeneous group of neuroendocrine tumors with diverse hormonal phenotypes, molecular characteristics, and clinical behavior. Their management includes surgery, pharmacological therapy, radiotherapy, and, in selected refractory cases, systemic or molecularly targeted approaches. Advances in molecular endocrinology have identified recurrent alterations and dysregulated signaling networks, including GNAS, USP8, PI3K/AKT/mTOR, and MAPK-associated pathways, that contribute to the biological het

Exercise-driven glucocorticoid signaling acts as a systemic circuit breaker to reprogram microglia and resolve post-stroke inflammation.

Following ischemic stroke, neuroinflammation drives severe secondary immunopathology, yet how systemic interventions coordinate peripheral and central immunity for neuroprotection remains elusive. Here, we demonstrate that acute-phase exercise accelerates motor rehabilitation and neural repair by activating the hypothalamic-pituitary-adrenal (HPA) axis to function as a potent systemic circuit breaker. Systemically, exercise-driven glucocorticoids, validated via targeted receptor blockade, induce profound spleen atrophy to restrict the peripheral immune pool, effectively depriving the ischemic

Loss of Selenot in POMC Neurons Disrupts ER Homeostasis and Induces A Senescent-like Phenotype.

AIMS: Selenoprotein T (SELENOT) is an endoplasmic reticulum (ER)-resident thioredoxin-like oxidoreductase implicated in redox and calcium (Ca2+) homeostasis and protein quality control. Although SELENOT is highly expressed in the brain, its role in hypothalamic pro-opiomelanocortin (POMC) neurons, which regulate energy balance, remains poorly understood. We investigated whether SELENOT is required for maintaining ER homeostasis and neuronal integrity in POMC neurons. RESULTS: Using CRISPR-edited and siRNA-silenced POMC neuronal models together with a POMC-specific SELENOT knockout mouse, we d

Blockade of gastrin-releasing peptide receptor in the PVN alleviates maternal vascular and placental dysfunction in preeclamptic rat.

Preeclampsia (PE) is characterized by placental ischemia-induced systemic inflammation, oxidative stress, and maternal vascular dysfunction. We previously reported that upregulated gastrin-releasing peptide (GRP) and its receptor (GRPR) within the hypothalamic paraventricular nucleus (PVN) of PE rats contribute to the progression of PE. However, whether such central upregulation contributes to the peripheral organs damage in PE remains unknown. This study investigated the protective effects of the GRPR antagonist ICI-216140, administered via PVN microinjection, on mesenteric arteries and place

HPLC-Quantified Styphnolobium japonicum Extract Attenuates Microglial Inflammation and Hypertension in L-NAME-Treated Rats.

Neuroinflammation in the hypothalamic paraventricular nucleus (PVN) can contribute to sympathetic excitation and hypertension. We evaluated a Styphnolobium japonicum bud extract (SJBE; historically Sophora japonica) quantified for rutin and quercetin by targeted high-performance liquid chromatography (HPLC). The extract was tested in angiotensin II (Ang II)-stimulated BV-2 microglia, BV-2/mHypoA-2/10 soluble-factor co-cultures, an in vitro high-sodium neuronal-stress condition, and rats with Nω-nitro-L-arginine methyl ester (L-NAME)-induced nitric-oxide-deficiency hypertension. Validation of a point-of-care assay for simultaneous quantification of equine adrenocorticotropic hormone and corticotropin-like intermediate peptide.

BACKGROUND: Plasma adrenocorticotropic hormone (ACTH) concentrations are used to diagnose equine pituitary pars intermedia dysfunction (PPID). The reference chemiluminescent immunoassay for immunoreactive ACTH (iACTH) cross-reacts with corticotropin-like intermediate peptide (CLIP), which is also abnormally high in PPID. HYPOTHESIS/OBJECTIVES: To compare the performance of a novel point-of-care (POC) immunoassay that concurrently measures ACTH and CLIP against the reference iACTH assay. ANIMALS: Twenty-eight university-owned horses (13 PPID, 15 non-PPID). METHODS: Prospective observational

Drug Repurposing for Treating NCOR-Related Intellectual Disability.

Nuclear receptor corepressors (NCOR1/2) mediate the transcriptional activity of methyl-CpG binding protein 2 (MECP2), and genetic variants in NCOR1/2 or MECP2 associate with autism or intellectual disability. However, it is unknown whether heterozygous NCOR mutations impair cognitive functions and how NCOR-related intellectual disability can be treated. Here, we constructed a knock-in (KI) mouse line carrying a heterozygous NCOR1 mutation (NCOR1 c.2182+2T>G) found in a pediatric patient. These NCOR1KI/+ mice showed social avoidance, stereotypic behaviors, and memory deficits, similar to hetero

Macroprolactinoma with an atypical course.

Prolactinomas are among the most common pituitary adenomas. The typical symptoms of hyperprolactinemia associated with a prolactinomaprimarily include symptoms of hypogonadism resulting from the suppression of the hypothalamic-pituitary-gonadal axis.In men and postmenopausal women, symptoms related to the mass effect of the tumor usually predominate. In some cases, however, prolactinomas may initially present with atypical symptoms, which can make it difficult to establish the correct diagnosis. An extensive literature search covering the past 30 years was conducted using the PubMed database. T

Gut microbiota and the gut-brain axis in autism spectrum disorder and attention-deficit/hyperactivity disorder: A scoping review of shared and disorder-specific mechanistic pathways.

BACKGROUND: Autism spectrum disorder (ASD) and attention-deficit/hyperactivity disorder (ADHD) are among the most prevalent neurodevelopmental conditions worldwide, yet the biological mechanisms driving their onset and phenotypic variability remain incompletely understood. The gut-brain axis (GBA), a bidirectional network linking intestinal microbial communities to central nervous system function through endocrine, immune, neural, and metabolic pathways, has emerged as a candidate mechanism through which environmental factors interact with genetic susceptibility to shape neurodevelopmental out
