Late-onset cobalamin C defect with a rare MMACHC c.457 C > T variant: a case report and literature review.

BACKGROUND: Cobalamin C (cblC) defect, also referred to as combined methylmalonic acidemia and homocystinuria, is the most common inborn error of intracellular cobalamin metabolism. Late-onset cblC defect often presents with atypical and heterogeneous manifestations, particularly neuropsychiatric symptoms, which may lead to misdiagnosis and delayed treatment. CASE PRESENTATION: We report a 13-year-old girl with late-onset cblC defect who presented with progressive cognitive impairment, psychiatric and behavioral abnormalities, and motor dysfunction. Because of these atypical manifestations, s

Methylmalonic Acid in Skeletal Aging and Osteoporosis: Linking Vitamin B12 Metabolism to Mitochondrial Dysfunction.

Methylmalonic acid (MMA) is a functional marker of vitamin B12 insufficiency and an emerging metabolic factor associated with skeletal aging and osteoporosis. This review synthesizes epidemiological, experimental, and mechanistic evidence linking the vitamin B12-MMA axis to impaired bone homeostasis. Population studies associate elevated circulating MMA with lower bone mineral density, higher odds of osteoporosis, and lower knee extensor strength, while disturbed vitamin B12 metabolism has also been linked to altered bone turnover. Experimental MMA exposure and models of methylmalonyl-CoA muta

Targeted LC-MS/MS Analysis of Malonic Acid Biomarkers in Clinical Laboratory Applications.

LC-MS/MS enables sensitive quantification of malonic acid (MA), methylmalonic acid (MMA), and ethylmalonic acid (EMA) across clinical matrices. MA provides emerging diagnostic value in metabolic assessments, while MMA remains the primary marker of functional vitamin B12 deficiency. EMA is essential for evaluating mitochondrial and fatty-acid oxidation disorders. Advances in extraction and chromatographic strategies continue to improve analytical performance for these biomarkers.

Undertreatment of Wernicke encephalopathy in alcohol-related clinical contexts: a scoping review of diagnosis, thiamine regimens, and clinical management.

BACKGROUND: Wernicke encephalopathy (WE) is a potentially reversible neurological emergency that remains frequently missed and undertreated in patients with alcohol use disorder (AUD). Although thiamine is widely accepted as the cornerstone of prevention and treatment, diagnostic uncertainty, limited comparative evidence, and heterogeneous clinical recommendations contribute to substantial variability in real-world management. This scoping review aimed to map the available evidence on the diagnosis and clinical management of WE in patients with AUD or clinically significant alcohol use, with p

Subclinical thiamine deficiency in third-trimester pregnancy despite routine antenatal supplementation: a cross-sectional study in an urban setting, Bangkok, Thailand.

BACKGROUND: Thiamine deficiency disorders (TDD) remain a preventable yet underrecognized contributor to maternal and infant morbidity, particularly in low- and middle-income countries. During pregnancy, thiamine deficiency may go undetected despite routine antenatal care and can lead to serious neurological and cardiac complications in breastfed infants. However, data on thiamine status in urban populations receiving standard antenatal supplementation are limited. This study aimed to determine the prevalence of thiamine deficiency in third-trimester pregnant women and to identify associated fa

Folate Deficiency Drives Metabolic Dysfunction, Oxidative Stress, and Inflammation in HK-2 Renal Tubular Cells.

The growing aging population has been accompanied by an increasing prevalence of chronic kidney disease (CKD), highlighting the need to better understand modifiable dietary and metabolic factors that contribute to renal injury. Folate deficiency and excessive fructose intake have been implicated in renal inflammation; however, the underlying cellular mechanisms remain incompletely understood. In this study, we investigated the effects of folate deficiency or fructose exposure on inflammatory responses, lactate metabolism, oxidative stress, and mitochondrial function in human renal tubular epit

Vitamin B12 and folate intake and biomarker status among omnivore and vegetarian UK-dwelling women of childbearing age: pooled results from the National diet and nutrition surveys (2008-2019).

PURPOSE: Vitamin B12 is crucial for neurological function and maternal health, particularly during pregnancy and lactation. Women of childbearing age (WCBA) following unfortified plant-based diets are at increased risk of B12 deficiency, which can adversely affect both maternal and infant health. This study analysed NDNS data (2008-2019) to: (1) compare vitamin B12 intake and status, and folate intake and status, between omnivores and vegetarians; (2) examine the impact of supplementation; (3) explore predictors of B12 biomarkers, including diet, supplementation, and age, and (4) assess the pr

Mapping the Evidence on Vitamin D, Vitamin B12, and Folate Deficiencies and Their Potential Impact on Treatment Outcomes in Patients With Cancer in Sub-Saharan Africa: Protocol for a Scoping Review.

BACKGROUND: Micronutrient deficiencies, particularly of vitamin D, vitamin B12, and folate, are increasingly recognized as important, yet underexplored contributors to adverse outcomes in oncology care. These micronutrients are essential for immune regulation, DNA synthesis and repair, cell proliferation, and metabolic homeostasis, all of which are frequently disrupted in patients with cancer. Deficiencies may arise due to cancer-related metabolic changes, malabsorption, anorexia, and treatment-related toxicities, potentially influencing therapeutic response, treatment tolerance, survival, and

Pellagra-Fine arts and literature: A short journey from Pietro Belloti and Johann Wolfgang Goethe to John Wesley Caroll and Amedeo John Engel Terzi.

Pellagra is a non-hereditary disorder of the niacin and tryptophan metabolism. Cutaneous and gastrointestinal signs and symptoms are early signs of the disease, but neuropsychiatric findings follow, if the disease is not recognized and treated soon enough. Although pellagra had not been identified as a separate entity before 1735, artists noticed the characteristic cutaneous manifestations. An example is Pietro Bellotti's masterpiece Vecchia popolana con un ragazzo (Old Peasant Woman with a Boy; ca 1680-1690) with a most realistic style. In 1786, the German writer and scientist Goethe traveled

From analytical bias to threshold-based discordance: An ID-LC-MS/MS reference method reveals and corrects hyperhomocysteinemia classification discordance in China.

BACKGROUND: Homocysteine (Hcy) is a critical biomarker for cardiovascular risk stratification, but significant between-method variability in China can result in classification discordance relative to candidate reference measurement procedure (cRMP), including both positive discordance and negative discordance of hyperhomocysteinemia. We aimed to establish a cRMP and evaluate its ability to correct method-dependent classification discordance. METHODS: An isotope-dilution liquid chromatography-tandem mass spectrometry (ID-LC-MS/MS) cRMP for serum Hcy was developed and validated. Five routine me

N-Homocysteinylation of Cofilin-1 Aggravates Epileptic Pathophysiology via Disruption of Actin Dynamics.

Elevated homocysteine (Hcy) levels contribute to the onset and progression of childhood epilepsy. However, Hcy-lowering therapies provide limited benefits, suggesting additional pathogenic mechanisms. Here, we identified N-homocysteinylated Cofilin-1 (N-Hcy-CFL1) as a molecular link between elevated Hcy levels and childhood epilepsy. We detected N-Hcy-CFL1 in patients with epilepsy and in a mouse model of hyperhomocysteinemia. Mass spectrometry identified Hcy-dependent lysine modifications in CFL1, and functional assays, including actin depolymerization, the F/G-actin ratio, and axonal elongat

[Consequences of celiac disease in an unaccompanied minor Eritrean refugee: severe intramedullary hemolytic anemia due to vitamin B12deficiency].

Unaccompanied minor refugees (UMRs), particularly from regions like Syria, Eritrea, and Iraq, face significant healthcare challenges in the Netherlands. Their unique medical and psychosocial needs necessitate a culturally sensitive diagnostic approach and coordinated care. Chronic conditions linked to nutritional deficiencies, such as celiac disease, are of particular concern. Celiac disease is an immune-mediated disorder triggered by gluten, leading to inflammation and malabsorption in the intestines. We present a case of a 16-year-old Eritrean UMR who, after a lengthy migration, arrived with

Vitamin B12 in early childhood: a phenotype-guided approach to deficiency and unexpectedly elevated concentrations.

UNLABELLED: Vitamin B12 deficiency can cause preventable but potentially irreversible neurological injury in infancy, whereas unexpectedly elevated total vitamin B12 creates a different diagnostic challenge. This review outlines a phenotype-guided approach to both findings, emphasizing the mother-infant dyad. Infant deficiency commonly reflects low maternal status, including unrecognized malabsorption despite an omnivorous diet. Neurological manifestations may precede anemia or macrocytosis. Methylmalonic acid and total homocysteine therefore help resolve borderline or clinically discordant vi

[Hereditary folate malabsorption due to an SLC46A1 mutation with cerebral folate deficiency and refractory epilepsy: first case reported in Argentina].

Hereditary folate malabsorption (HFM) is an autosomal recessive metabolic disorder caused by pathogenic variants in SLC46A1, the gene encoding the proton-coupled folate transporter. This defect impairs intestinal folate absorption and its transport into the central nervous system (CNS), leading to cerebral folate deficiency even with normal serum levels. Clinically, it presents with megaloblastic anemia, gastrointestinal symptoms, immunological alterations, and progressive neurological involvement. Measurement of 5-methyltetrahydrofolate (5-MTHF) in cerebrospinal fluid (CSF) is the specific di

Maternal hyperhomocysteinemia promotes adverse pregnancy outcomes via mtDNA-cGAS-STING signaling-mediated trophoblast mitophagy.

OBJECTIVES: Hyperhomocysteinemia (HHcy) is a recognized risk factor for adverse pregnancy outcomes. However, the underlying mechanisms precipitating placental dysfunction, particularly the potential involvement of mitochondrial DNA (mtDNA), remain largely elusive. This study aims to investigate the association between maternal HHcy and adverse pregnancy outcomes mediated by reduced placental mtDNA copy number (mtDNAcn), and to elucidate the underlying molecular mechanisms driving HHcy-induced placental dysfunction. METHODS: We prospectively investigated the associations between maternal HHcy,
