Automated antimicrobial susceptibility testing in gram-negative bacteremia: A comparative study of VITEK® REVEAL™ and BD phoenix™.

Bloodstream infections (BSIs) due to Gram-negative bacteria can rapidly escalate to life-threatening conditions, making the early selection of the appropriate antibiotics crucial for saving lives. Current conventional laboratory antimicrobial susceptibility tests for identifying effective antibiotics typically require 16-24 hours in addition to the sub-culture time, prolonging time-to-result (TTR) and delaying appropriate therapy. In this study, we compared the fast antimicrobial susceptibility test (AST) system VITEK® REVEALTM (VITEK REVEAL) with BD PhoenixTM (BD Phoenix) on 192 prospectively

Multisite clinical evaluation of the iFAST rapid antimicrobial susceptibility test direct from positive blood cultures containing gram-negative organisms.

UNLABELLED: iFAST is a label-free rapid antimicrobial susceptibility test (AST) that delivers qualitative results in under 4 h from a positive blood culture bottle. The technique measures changes in the electrical properties of thousands of individual bacteria following exposure to antibiotics. We report the diagnostic accuracy from the first clinical study of the iFASTone rapid AST system in three different centers according to ISO 20776-2:2021. Compared to the reference laboratory broth microdilution method, categorical agreement was 97.8% and 97.2% for the Enterobacterales and non-fermentin

Microbial etiology and antimicrobial resistance patterns in neonates admitted to a tertiary neonatal intensive care unit in Mogadishu, Somalia.

OBJECTIVE: Neonatal sepsis continues to be one of the leading causes of morbidity and death, particularly in low- and middle-income countries (LMICs). This study aimed to evaluate the microbiological etiology, temporal trends, and antibiotic resistance patterns in a tertiary neonatal intensive care unit (NICU) over a six-year period. METHODS: This retrospective study included neonates admitted to a tertiary neonatal intensive care unit between 2018 and 2024. Early-onset infections were defined as infections occurring within the first 72 hours of life, while late-onset infections were defined

Aeromonas hydrophila Necrotizing Soft-Tissue Infection With Acute-on-Chronic Calcaneal Osteomyelitis Following Freshwater Exposure in a Patient With Poorly Controlled Diabetes Mellitus.

Aeromonas hydrophila is a gram-negative bacterium found in freshwater environments that can cause rapidly progressive necrotizing soft-tissue infections (NSTIs), particularly in patients with predisposing host factors such as diabetes mellitus. The organism rarely affects deeper structures such as bone, which can make diagnosis and treatment more difficult. We report the case of a 68-year-old man with poorly controlled type 2 diabetes mellitus, heart failure with reduced ejection fraction, and prior calcaneal trauma who developed an NSTI with acute-on-chronic calcaneal osteomyelitis after stag

Antimicrobial susceptibility concordance between colonising and bloodstream ESBL and carbapenemase-producing Enterobacterales in allogeneic stem cell transplant recipients: a 10-year retrospective cohort study.

BACKGROUND: Surveillance cultures for multidrug-resistant Gram-negative bacteria are increasingly used to guide empirical therapy in allo-HSCT recipients, yet whether the antimicrobial susceptibility profile of the colonising isolate reliably predicts that of the infecting strain remains poorly characterised. The aim of the study was to evaluate antimicrobial susceptibility concordance between colonising and bloodstream extended-spectrum β-lactamase-producing Enterobacterales (ESBL-E) and carbapenem-resistant Enterobacterales (CRE) isolates in allo-HSCT patients, assess surveillance culture pe

Changes in observed bloodstream infection epidemiology during a single-set blood culture restriction: an interrupted time series analysis.

During the 2024 US national blood culture bottle shortage, many hospitals implemented restrictions limiting blood culture collection to conserve supplies. The impact of these constraints on the microbiologic epidemiology of bloodstream infections (BSIs) remains incompletely characterized. We conducted an interrupted time series analysis of BSI episodes identified at two quaternary hospitals between June 2023 and June 2025, encompassing a systemwide restriction limiting clinicians to one blood culture set per patient every 24 h from June through December 2024. We grouped blood cultures into dis

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Association of specific HLA alleles in patients with interstitial cystitis suggesting autoimmunity.

BACKGROUND: Bacteremia frequently complicates cytostatic treatment of hematological malignancies. Initial antibiotic treatment is chosen empirically before the results of cultures are available. Rational choices depend on updated knowledge of microbial resistance patterns. We have examined microorganisms in blood cultures over a 10-year period and compared them with the preceding 5-year period. MATERIAL AND METHODS: We analyzed isolates from blood cultures in patients with a hematological malignant disorder treated in the Hematology Unit, St. Olavs Hospital during the years 1995 - 2005. Clinical and microbiological characteristics of skin and soft tissue infections of the extremities with bacteremia in patients with malignancy.

INTRODUCTION: Although skin and soft tissue infections (SSTIs) of the extremities are primarily caused by Gram-positive cocci (GPC), some cases are caused by Gram-negative rod (GNR). In addition, malignancy is a recognized risk factor for GNR infection. Nevertheless, information on the clinical and microbiologic characteristics of SSTIs of the extremities caused by GNR in patients with malignancy is limited. METHODS: Clinical and microbiological characteristics of patients with malignancy who developed bacteremic SSTIs of the extremities at a single cancer center over eight years were reviewe

Not Your Typical Pneumonia: A Rare Case of Delftia acidovorans Pneumonia With Concurrent Malignant Squamous Cell Carcinoma.

Delftia acidovorans (D. acidovorans) is classified as a Gram-negative, aerobic non-fermenting bacillus that is commonly found in outdoor environmental elements, such as soil and water. In normal circumstances, it is considered a non-pathological environmental organism, rarely implicated in clinical settings with a low incidence of patient infection. D. acidovorans infections, including pneumonia and sepsis, are often associated with increased susceptibility to secondary infections from other Gram-negative bacteria, especially in patients with solid or hematologic malignancies. Henceforth, this

Hand hygiene versus additional non-sterile gloves and gowns use to prevent sepsis in preterm infants colonized with multi-resistant drug bacteria: the study protocol of the cluster-randomized, cross-over, non-inferiority BALTIC trial.

BACKGROUND: Infections are highly relevant for neonatal mortality and long-term morbidities in survivors. Therefore, it is an urgent need to optimize and evaluate infection prevention and control (IPC) strategies. Several infection outbreaks in German neonatal intensive care units (NICUs) required rapid responses by hospitals and improved future preparedness. As a consequence, German authorities recommended weekly colonization screening on NICUs. This screening aims to detect multidrug-resistant organisms (MDRO) and bacteria with high transmissibility. According to these guidelines, infants co
