Regulation of Diverse Endogenous Metabolic and Signaling Pathways by UDP-Glucuronosyltransferases (UGTs) Revealed by Metabolomics of Knockout Mice.

Uridine 5'-diphospho-glucuronosyltransferase (UGT) enzymes play a critical role in hepatic and other metabolism of endogenous and exogenous small molecules. The addition of glucuronic acid via UGT increases the polarity and subsequent solubility of small molecules, helping direct them for biliary excretion and renal excretion by organic anion transporter OAT3 (SLC22A8). Mutations in UGTs are associated with disorders in biliary metabolism (e.g., Gilbert's syndrome, Crigler-Najjar syndrome). Here, we used comprehensive serum metabolomic profiling of pan Ugt1-knockout (Ugt1-/- ) mice to define t

Development of a Physiologically Based Model of Bilirubin Metabolism in Health and Disease and Its Comparison With Real-World Data.

Bilirubin is a breakdown product of erythrocytes and plays a crucial role in elimination of heme-containing proteins. After its synthesis in the reticuloendothelial system, unconjugated bilirubin is released into plasma and taken up into the liver. In hepatocytes, bilirubin is conjugated and excreted into the gastrointestinal tract via bile, where it is further converted to urobilinoids. There are various genetic factors causing abnormal bilirubin levels in plasma, such as Gilbert syndrome, Crigler-Najjar syndrome, Dubin-Johnson syndrome, and Rotor syndrome. To better understand bilirubin meta

Between Crigler-Najjar Syndrome Type II and Gilbert Syndrome: Expanding the Spectrum of Uridine Diphosphate Glucuronosyltransferase 1A1 (UGT1A1)-Related Hyperbilirubinemia.

Crigler-Najjar syndrome (CNS) and Gilbert syndrome (GS) are inherited non-hemolytic unconjugated hyperbilirubinemias caused by mutations in the uridine diphosphate glucuronosyltransferase 1A1 (UGT1A1) gene, resulting in reduced or absent bilirubin conjugation. These disorders form a phenotypic spectrum, with CNS type I (CNSI) representing the most severe form, CNS type II (CNSII) an intermediate phenotype, and GS the mildest. Differentiation between CNSII and GS can be challenging due to overlapping bilirubin levels and genetic variants. We report the case of a female newborn of Nepali descent

From Childhood Icterus to Adolescent Gallstones: Clinically Diagnosed Crigler-Najjar Syndrome Type II.

Crigler-Najjar syndrome type II (CNS-II) is an uncommon cause of persistent unconjugated hyperbilirubinemia resulting from partial deficiency of hepatic UDP-glucuronosyltransferase activity. We report the case of a 19-year-old male who presented with intermittent jaundice since childhood and recent worsening of scleral icterus. Laboratory evaluation revealed isolated unconjugated hyperbilirubinemia with normal liver enzymes. Genetic testing was unavailable; however, serum bilirubin levels declined significantly following phenobarbital therapy, confirming the diagnosis of CNS-II. Abdominal ultr

Liver Transplantation Outcomes in Crigler-Najjar Syndrome in Iran: A Single-Center Retrospective Cohort Study Over 20 Years.

BACKGROUND AND AIMS: Crigler-Najjar Syndrome (CNS) is a rare autosomal recessive disorder caused by uridine diphosphate glucuronosyltransferase (UGT1A1) deficiency, leading to unconjugated hyperbilirubinemia. Without treatment, patients are at high risk of kernicterus and irreversible neurological damage. Liver transplantation (LT) is the only curative option. This study aims to evaluate long-term outcomes of LT in CNS patients over a 20-year period in Iran. METHODS: In this retrospective cohort study, we analyzed 69 patients with CNS who underwent LT between 2000 and 2023 at Namazi Hospital

Case Report: Retransplantation for Adult-Onset Bilirubin Encephalopathy After Auxiliary Partial Liver Transplant for Crigler-Najjar Syndrome Type 1 in Childhood.

Crigler-Najjar syndrome (CNS) type 1 is a genetic disorder that leads to severe unconjugated hyperbilirubinemia. Auxiliary partial orthotopic liver transplantation (APOLT) is preferred for metabolic liver diseases as a portion of the native liver is retained. Reported here are details of a 32-year-old male who developed bilirubin encephalopathy due to graft failure 27 years after APOLT performed at 5-year-old for CNS type 1. Initially, the bilirubin level was low, which then gradually increased to 7.3 mg/dL at age 12. At the age of 31, the bilirubin level began to rise rapidly and we submitted

Optimizing Inpatient Phototherapy in Adult Crigler-Najjar Syndrome Type 1: A Practical Framework for Acute Hyperbilirubinemia Management.

Crigler-Najjar syndrome type 1 (CN1) causes complete UGT1A1 deficiency, lifelong severe unconjugated hyperbilirubinemia, and persistent risk of bilirubin-induced neurologic dysfunction. We present a 21-year-old woman with genetically confirmed CN1 who developed acute-on-chronic hyperbilirubinemia to 30 mg/dL during hospitalization for Shigella enteritis when her home high-intensity phototherapy system was unavailable. Systematic optimization of inpatient phototherapy using multiple high-irradiance units with simultaneous dorsal and ventral coverage produced a 7.5 mg/dL bilirubin reduction with

Case Report: Therapeutic plasma exchange for severe unconjugated hyperbilirubinemia with bilirubin-induced neurologic dysfunction in an adolescent with Crigler-Najjar syndrome type II.

BACKGROUND: Crigler-Najjar syndrome type II (CNS-II) is a rare inherited disorder of bilirubin conjugation that typically responds to phenobarbital and is less frequently complicated by bilirubin neurotoxicity than CNS-I. During physiologic stress, unconjugated bilirubin may acutely rise, potentially increasing unbound bilirubin and neurologic risk. CASE PRESENTATION: A 15-year-old male with a known history of CNS-II presented with two weeks of progressive abnormal movements and worsening jaundice, followed by decreased consciousness and incoherent speech. Initial testing demonstrated severe

Hepatic Expression of Fibroblast Growth Factor 19 Significantly Correlates With Serum Bile Acids in Neonatal Cholestasis.

BACKGROUND: Bile acids in the ileum act as a feedback regulator of their own synthesis by inducing the release of ileal fibroblast growth factor 19 (FGF19), which inhibits the cholesterol-7-alpha hydroxylase enzyme. In cholestasis, this feedback mechanism is dysregulated. FGF19 is not expressed in the healthy liver. We aimed to assess the hepatic expression of FGF19 in neonatal cholestasis (NC) and its relation to serum bile acids. METHODS: The study included 41 patients with NC. FGF19 immunohistochemical staining in liver tissue (hepatocytes, endothelial cells, bile ducts, and bile canalicul

Crigler-Najjar syndrome type 2 complicating cholecystitis in a patient with UGT1A1 gene double homozygous mutations.

Crigler-Najjar syndrome (CNS) and Gilbert syndrome (GS; OMIM: 143500) are rare autosomal recessive diseases that cause unconjugated hyperbilirubinemia due to decreased UGT1A1 enzyme activity. Crigler-Najjar syndrome type 2 (CNS2; OMIM: 606785) increases the risk of gallbladder stone formation and cholecystitis, while GS seldom causes health issues. We found a 28-year-old male patient with recurring right upper abdomen pain who experienced persistent jaundice from birth. CNS2 with gallbladder stones and cholecystitis was diagnosed after genetic testing revealed rare double homozygous mutations
