The role of the renin-angiotensin-aldosterone system in the relationships between stress exposure, stress-related psychopathology, and cardiovascular disease: a rapid scoping review.

BACKGROUND: The renin-angiotensin-aldosterone system (RAAS) is widely recognized for its role in maintaining fluid and electrolyte balance, as well as its involvement in the pathogenesis of certain forms of cardiovascular disease (CVD). Recent research suggests that, in addition, the RAAS may play a role in the body's response to stress, making activity of the RAAS a potential mechanism linking stress exposure or psychopathology to cardiovascular disease risk. PURPOSE: This rapid scoping review synthesizes the relevant literature that has been published on this topic over the past 16 years.

Chronic Dehydration in Older Adults: A Silent Threat to Healthy Aging.

In contrast to acute dehydration, which is usually recognized through identifiable risk factors, clinical signs, and laboratory abnormalities, chronic dehydration develops insidiously and frequently goes unrecognized. This is particularly true in older adults, among whom many of its nonspecific manifestations-such as weakness, constipation, headache, dizziness, gait instability, and confusion-are often mistakenly attributed to normal aging. This population is especially vulnerable to chronic dehydration because of age-related normal physiological changes, compounded by physical, cognitive, psy

The intermediate conductance calcium-dependent K+ channel does not contribute to the slow after hyperpolarization in oxytocin and vasopressin hypothalamic magnocellular neurons.

Vasopressin (VP) magnocellular neurosecretory neurons of the hypothalamic supraoptic nucleus (SON) are critical regulators of renal water retention and vascular tone. VP neurons undergo detrimental plastic changes in cardiovascular diseases such as heart failure (HF), resulting in hyperexcitability and thus altered fluid/electrolyte balance. A major intrinsic mechanism that regulates the firing activity of VP neurons is the slow afterhyperpolarization (sAHP), a phenomenon underlain by a calcium-dependent K+ current (IsAHP). The sAHP is activated by Ca2+ and results in an efflux of K+ from the

Bidirectional Interaction Between the Gastrointestinal System and the Kidney: Pathophysiological and Clinical Perspective.

The gastrointestinal (GI) system and the kidneys, though anatomically separate, are functionally interconnected through shared responsibilities in maintaining fluid-electrolyte balance, acid-base homeostasis, immune regulation, and hormonal signaling. Disruptions in one system often lead to secondary complications in the other, highlighting the need for a comprehensive understanding of their bidirectional interactions. Kidney involvement in GI diseases commonly results from mechanisms such as fluid loss, malabsorption, systemic inflammation, and exposure to toxins, as seen in conditions like i

Water and Energy Turnover in Chinese Young Adults: A Doubly Labeled Water Study of Metabolic Coupling.

BACKGROUND: Accurate estimation of water and energy requirements is fundamental for establishing dietary reference values in young adults. However, evidence integrating objectively measured energy expenditure with detailed water turnover components remains limited in Chinese populations. OBJECTIVES: To quantify water intake, water loss, and energy expenditure in healthy young college students, and to examine how energy metabolism is associated with specific components of water turnover under free-living conditions. METHODS: Twenty-one healthy adults aged 18-25 years participated in a 14-day

Disrupted fluid homeostasis in patients with post-Covid-19 syndrome - a case series.

The post-covid-syndrome (PCS) is characterized by severe persisting fatigue and other long haul symptoms following COVID-19 infection. Disease-driving mechanisms are elusive, biomarkers are missing, treatment options few, and yet millions of people worldwide are affected. We hypothesized that the common complaint of polydipsia/polyuria and its attendant effect on osmolality can be quantified and identified as an unrecognized clinical feature of the PCS. We conducted a clinical case-series of 10 consecutive patients with PCS (5 females, 5 males; mean age 44 ± 14 y) who completed questionnaires

Managing a Critically Ill Patient With Pulmonary Arterial Hypertension.

Management of critically ill patients with pulmonary hypertension presents significant clinical challenges. Regardless of the underlying etiology or chronicity of pulmonary hypertension, superimposed acute illness can precipitate decompensated right ventricular failure (RVF) and death. Hospitalization, particularly in the ICU, is associated with high mortality, with RVF serving as the direct or indirect cause in most cases. In this article, we will focus on the management of acute decompensated RVF in patients with known pulmonary arterial hypertension (PAH). ICU management of the patient with

Acute Kidney Injury for the Interventional Radiologist: A Review.

AKI is commonly seen among hospitalized patients, many of which the VIR is consulted to assess for a potential intervention. Rates are increased for critically ill patients and those with pre-existing CKD, advanced age, or significant comorbidities. Developing an AKI is associated with increased morbidity, mortality, and risk of progression to chronic kidney disease. Physiologically, renal function depends on the coordinated activity of nephrons, which regulate fluid balance, electrolyte homeostasis, and metabolic waste filtration. Disruption of renal perfusion, injury to the microscopic and m

Water and weight: the role of hydration in weight management.

Water is a mega nutrient, required in larger quantities than other macro-and micro-nutrients. It also constitutes the bulk of the human body. Yet, hydration is usually missing from normal discourse in medical nutrition therapy and metabolism. This review describes various evidence-based facts related to water and weight, or hydration and baro-homoeostasis. It argues that hydration should be used as a tool to prevent and fight obesity.

Astrocytes orchestrate neuroprotection and pathogenesis via bidirectional neuronal interactions in Parkinson's disease.

Parkinson's disease (PD), a prevalent neurodegenerative disorder, is characterized by the degeneration of dopaminergic neurons in the substantia nigra and striatum of the midbrain, manifesting as distinct motor impairments. While conventional theories attribute PD's development to neuronal damage, astrocytes have garnered significant attention for their potential protective role. As the most abundant glial cells in the central nervous system (CNS), astrocytes may exert neuroprotective effects through mechanisms including secretion of neurotrophic factors, regulation of the blood-brain barrier

Acid-Base and Electrolyte Disorders: Potassium Disorders.

A normal serum potassium level of 3.5 to 5.0 mEq/L is maintained via potassium ingestion, excretion, and distribution between intra- and extracellular fluid. Potassium balance is essential for maintenance of normal resting cell membrane potential in excitatory tissues. Abnormalities in serum potassium, whether low or high, can cause life-threatening complications due to cardiac, respiratory, or neurologic compromise. Hypokalemia results from renal or gastrointestinal losses, or transcellular shifts. In the absence of an identified cause, evaluation of urinary potassium excretion and acid-base

Acute Kidney Injury-to-Chronic Kidney Disease Transition-Associated Macrophage Subtypes: Biological Functions and Intercellular Crosstalk.

As a core organ responsible for metabolism and homeostatic regulation, the kidney performs physiological functions that encompass material excretion, fluid balance, electrolyte regulation, and endocrine control; and serves as a critical hub for maintaining the coordinated functioning of multiple organ systems. Kidney injury not only leads to disturbances in these core physiological functions but also generates systemic complications such as cardiovascular disease, hypertension, and diabetes mellitus through an "injury-inflammation-metabolic disorder" cascade. Severe Hypovolemic Hyponatremia Following Percutaneous External Biliary Drainage in a Patient With Metastatic Lung Adenocarcinoma: A Case Report.

Hyponatremia is a frequent electrolyte disorder that results from an imbalance between body water and sodium. While hypovolemic hyponatremia has been largely attributed to the loss of sodium either from the gut or the kidneys, sodium depletion caused by external biliary diversion remains a largely uncommon but underacknowledged condition. We report a case of a 78-year-old man with metastatic adenocarcinoma of the lung who developed severe symptomatic hyponatremia after a percutaneous transhepatic biliary drainage procedure for malignant obstructive jaundice due to metastases to the liver. He c

The Effects of Malnutrition and Refeeding on Perioperative Wound Complications in Surgical Patients With Eating Disorders: A Narrative Review.

Anorexia nervosa (AN) and bulimia nervosa (BN) are eating disorders (EDs) with the highest mortality rate of psychiatric illnesses and medical complications, often due to protein-energy malnutrition and micronutrient deficiencies, leading to poor long-term outcomes. Patients with EDs undergo changes in nutritional status that manifest as decreased skin tensile strength, impaired immune function, reduced collagen production, and disrupted electrolyte and fluid balance, thereby increasing the risk of surgical site infections (SSIs), pressure ulcers (PUs), refeeding syndrome (RS), and edema/refee

Phenotypic comorbidity network analysis of nasopharyngeal carcinoma: a multicenter study based on hospital discharge records.

BACKGROUND: Nasopharyngeal carcinoma (NPC) patients frequently have multiple comorbidities that affect treatment efficacy and survival. Prior studies focused only on individual common comorbidities, lacking a comprehensive analysis of the full spectrum of chronic diseases in NPC patients from endemic areas. This study adopted phenotypic comorbidity networks (PCNs) based on large-scale hospital discharge data to systematically characterize the overall comorbidity patterns of endemic NPC patients. METHODS: This retrospective cohort study was conducted using data from NPC patients identified in

Natural Flavonoids: Fortifying Renal Defence Mechanism.

BACKGROUND: The kidneys, intricate organs responsible for maintaining fluid and electrolyte balance, are susceptible to damage from diverse nephrotoxic insults, including drugs, toxins, and metabolic disorders. In recent years, flavonoids, bioactive compounds abundant in fruits, vegetables, and herbal extracts, have emerged as promising candidates for renal protection due to their potent antioxidant and anti-inflammatory properties. METHODS: We have collected the data that supported this idea to conduct a comprehensive review by using scientific databases, such as Pub Med ®, ScienceDirect ®,

Volume Assessment in Patients Undergoing Long-Term Dialysis.

Accurate assessment of fluid status is a priority for patients with kidney failure undergoing long-term dialysis. There is wide variation in current volume-related practices between dialysis units and an urgent need to develop better evidence to guide practice. Clinical decisions relating to volume management are implicitly based on assessment of volume status, and there are numerous different but imperfect methods of assessment. Isotope-based dilutions are impractical for clinical use and may not be a gold standard for patients with kidney failure. Individual trends in body weight and BP have

Change in plasma electrolyte concentrations during hemodialysis following a controlled step-up in dialysate bicarbonate concentration.

BACKGROUND: The concentration of bicarbonate in dialysis fluid (Dbic) that regulates acid-base balance in dialyzed patients is individually adjusted and may be modified during a hemodialysis session. We evaluated the impact of modifications of Dbic on plasma electrolytes. METHODS: Two midweek hemodialysis sessions were monitored in 25 anuric, prevalent hemodialysis patients (67.9 ± 9.3 years old, 32% females). During the first session (treatment A) Dbic was constant at 33.6 ± 1.7 mmol/L, while for the second week (treatment B) it was on average 30.8 ± 2.3 mmol/L for the initial 2 h and 34.0 ±
