Relationship between age and severity of cognitive impairment at diagnosis for early-onset and late-onset Alzheimer's disease: Comparison of LEADS and ADNI.

INTRODUCTION: Recent work has identified unique cognitive profiles for early-onset Alzheimer's disease (EOAD) relative to late-onset Alzheimer's disease (LOAD), however, examination has been limited in determining whether the association between age and cognitive severity at presentation also differs across conditions. METHODS: A series of linear spline regression models was conducted across baseline cognitive data from 325 EOAD and 314 LOAD participants, after accounting for education, sex, and apolipoprotein ε4 status. RESULTS: Significant differences existed in the relationship between ba

Traumatic Brain Injury and All-Cause and Dementia-Related Mortality in the Framingham Heart Study.

IMPORTANCE: Traumatic brain injury (TBI) has been associated with mortality and chronic conditions, including dementia. However, research is limited regarding the disease process by which TBI leads to long-term mortality. OBJECTIVE: To assess TBI incidence over decades and its association with long-term all-cause and dementia-related mortality among participants in the Framingham Heart Study (FHS), with the hypothesis that the association would be largely attributable to dementia-related mortality. DESIGN, SETTING, AND PARTICIPANTS: This cohort study used data from the original and offspring

Molecular signatures of resilience to Alzheimer's disease in neocortical layer 4 neurons.

Selective neuronal vulnerability is a hallmark of Alzheimer's disease (AD), yet the molecular basis of resilience remains poorly understood. Using single-nucleus and spatial transcriptomics to compare neocortical regions affected early (prefrontal cortex, precuneus) or late (primary visual cortex) in AD, we identified a resilient excitatory population in layer 4 of the primary visual cortex expressing RORB, CUX2, and EYA4. Layer 4 neurons in association neocortex shared molecular signatures of resilience. Early-stage resilient neurons upregulated genes associated with synapse maintenance, syna

Repression of RIPK1 kinase by INPP5D inhibits expression of diverse proinflammatory mediators and late-onset Alzheimer's disease risk factors.

Genome-wide association studies strongly implicate neuroinflammation in late-onset Alzheimer's disease (LOAD). Genetic risk loci for LOAD are enriched for genes expressed in microglia, but the relationship among microglial LOAD risk genes has been unclear. We found that the N-terminal SH2 domain of INPP5D, an important LOAD risk gene, directly interacted with the cell death regulator RIPK1 at p-Y383 to suppress RIPK1 kinase activation. Microglial INPP5D deficiency cell-autonomously promoted RIPK1-mediated transcriptional induction of diverse LOAD risk genes, proinflammatory cytokines, compleme

Human in vitro and rodent in vivo models highlight progressive mitochondrial dysfunction as a starting point of cerebral amyloidosis.

Mitochondrial dysfunction is a well-established hallmark of Alzheimer's disease (AD), particularly in the context of amyloid-beta (Aβ) accumulation. Here, we explored the progression of mitochondrial impairment associated with cerebral amyloidosis in human and rodent systems expressing AD-relevant APP mutations. We investigated mitochondrial function, dynamics, and degradation in human neural progenitor cells differentiated for two and six weeks, carrying the APP (Swedish/London) mutations. These analyses were complemented by studies in 3- and 9-month-old McGill-R-Thy1-APP transgenic (Tg) rats

ABCA7 rs115550680 risk allele carriers have lower medial temporal lobe dynamic network flexibility than APOE-ε4 allele carriers among older African Americans.

Alzheimer's disease (AD) pathology disrupts functional brain connectivity long before symptoms emerge. African Americans face elevated AD risk, yet underlying mechanisms remain unclear. Genetic risk differs by ancestry: APOE-ε4 strongly predicts late-onset AD in European ancestry, whereas ABCA7 rs115550680 confers substantial risk in African ancestry. Yet, how these variants influence neural function in African Americans is unclear. The medial temporal lobe (MTL) is an early target of AD pathology and resting-state functional Magnetic Resonance Imaging (rs-fMRI) measures of dynamic network con

Reference gene variability across age and sex in 5XFAD mice highlights normalization challenges in Alzheimer's models.

Gene expression profiling in neurodegenerative diseases such as Alzheimer’s Disease (AD) is frequently performed using real-time quantitative polymerase chain reaction (RT-qPCR). The accuracy of this technique relies heavily on selecting suitable reference genes (RGs) for normalization and internal control. Ideally, RGs should maintain consistent transcription levels, unaffected by cellular or pathological changes. However, identifying stable RGs is challenging, particularly in diseases like AD, where gene expression fluctuates across disease stages. This study aimed to determine the most stab

Interplay between Peripheral and Central Nervous System Myeloid Cells during Aging: Impact for Lade-Life Depression and Alzheimer's Disease.

BACKGROUND: Aging is associated with enumerative and functional changes of peripheral innate immune cells, notably myeloid cells (e.g., monocytes/macrophages and neutrophils). Peripheral myeloid cells routinely infiltrate the brain, particularly at the brain borders, influencing cognition, mood, and stress responses. SUMMARY: Here, we review how the dysfunctional crosstalk between circulating myeloid cells and brain cells may contribute to the development of late-life depression and Alzheimer's disease during aging. The aged cerebral microglia (i.e., resident macrophages) exhibit dystrophic m

Evidence for reduced somatic T-cell receptor sequence diversity profiles among Midwestern Amish in an aging cohort study.

Late-onset Alzheimer disease (LOAD), the most common form of dementia among older adults, is a neurodegenerative disease characterized by brain amyloid-β (Aβ) plaque deposition and neurofibrillary tangles. The causes of LOAD are not known but several recent lines of evidence implicate the adaptive immune system. Here, we sought to characterize somatic T-cell receptor (TCR) sequence diversity profiles and class I and II human leukocyte antigen (HLA) alleles from DNA extracted from peripheral tissues from Midwestern Amish participating in longitudinal studies of aging. We immunosequenced the TCR

Distinct Tau PET Dynamics in Early vs. Late Age-of-Onset Alzheimer's disease.

Early-onset Alzheimer's disease (EOAD) and Late-onset AD (LOAD) differ in clinical presentations and rates of progression. We aimed to compare baseline and longitudinal tau PET burden, and their relationship with clinical variables in amyloid-PET positive, cognitively impaired participants from the Longitudinal Early-Onset Alzheimer's Disease Study (EOAD; n=390) and Alzheimer's Disease Neuroimaging Initiative (LOAD; n=211). Patients with EOAD showed higher baseline tau PET retention, broader neuroanatomical involvement and faster accumulation rates over time compared to LOAD, after adjusting f

Informing depression-specific dementia risk models: An evidence-based analysis of moderators of the depression-dementia association.

Depression is a major modifiable risk factor for dementia, yet most prediction models treat it as a homogeneous exposure, despite evidence that risk varies among people with depression. This study aimed to identify key modifiers of the depression-dementia association to inform the development of tailored prediction models. A narrative synthesis was conducted, incorporating (1) an umbrella review of nine meta-analyses examining the depression-dementia association; (2) a systematic review of depression-related medication use on dementia risk; and (3) findings from three Lancet Commission reports

Loss of CAMKK2 and iron-transport proteins-transferrin and its receptor-in the Alzheimer's disease hippocampus: link to tau pathology.

INTRODUCTION: Calcium and iron are essential bioelements regulating neuronal function and survival. Dysregulation of calcium signaling and iron homeostasis is implicated in Alzheimer's disease (AD), contributing to oxidative stress, synaptic dysfunction, and neurodegeneration. Previously, using in vitro cell-based models and transgenic mice, we demonstrated that CAMKK2, a calcium/calmodulin-dependent protein kinase, regulates iron transport via transferrin (TF) and transferrin receptor (TFRC). While excessive iron deposition is a hallmark of AD brains, the mechanisms underlying its dysregulati

Multicenter development and validation of a probability-based model to diagnose Lewy body disease using ¹²³I-meta-iodobenzylguanidine.

PURPOSE: We previously proposed that a probability-based sympathetic 123I-meta-iodobenzylguanidine (mIBG) index (SMILe) could distinguish the presence or absence of Lewy body disease (LBD) based on findings at a single center. However, whether the model would be useful in the real world remained uncertain. Therefore, we updated and evaluated its performance at five Japanese and three European institutions. METHODS: We compared data from 967 patients with suspected LBD with 62 controls from a normal database (NDB). Of 815 patients with guideline-based diagnoses, 483 had LBD (Parkinson disease

Distinct regional patterns of synaptic vulnerability across hippocampal and parahippocampal subregions in Alzheimer's disease.

Synaptic loss in the (para)hippocampus is a major contributor to cognitive decline in Alzheimer's disease (AD), yet its regional specificity and pathological correlates remain poorly understood. Here, we quantified synaptophysin-positive puncta across hippocampal subregions (CA4, CA2, CA1, and subiculum) and parahippocampal cortex (entorhinal cortex, parahippocampal and fusiform gyrus) in postmortem tissue from 28 AD and 17 controls, and assessed relationships to neuropathological severity and cognitive decline. Amyloid-β, phosphorylated tau (p-tau) load and neurofilament light (NfL) immunorea

Mid-life cardiorespiratory fitness and risk of late-onset dementia incidence.

INTRODUCTION: Cardiorespiratory fitness (CRF) is linked to dementia risk, but moderating factors remain unclear. This study examined how and when CRF in adulthood is associated with late-onset dementia (> 65 years) and whether sex, civil status, or education moderate this association. METHODS: In a cohort of 370,980 dementia-free individuals followed for a mean of 11.9 (standard deviation 6.0) years, CRF was estimated via a submaximal cycle test, with dementia incidence obtained from Swedish National Healthcare Registries. RESULTS: Results showed that high CRF was associated with lower demen

APOE4-driven T cell dysregulation in Alzheimer's disease: single-cell genomics and Mendelian randomization reveal novel therapeutic targets.

The ɛ4 allele of the apolipoprotein E gene (APOE4) has been identified as a significant risk factor for late-onset AD. However, the specific genetic differences in T lymphocytes between APOE4 and APOE3 carriers are poorly understood. This study aimed to identify these important genetic distinctions and investigate the causal relationship between these differences and the risk of Alzheimer’s disease (AD), offering insights that could lead to novel treatment approaches. We sequenced single peripheral blood cells from three APOE3 and three APOE4 patients using the BD Rhapsody sequencing technolog

PICALM Genetic Variant Alters mRNA Expression Without Affecting Protein Levels or Tau Spreading in Alzheimer's Disease.

Phosphatidylinositol-binding clathrin assembly protein (PICALM) is a clathrin adaptor essential for clathrin-mediated endocytosis. Genome-wide association studies (GWAS) have consistently identified PICALM as one of the most significant genetic susceptibility loci for late-onset sporadic Alzheimer's disease (AD). However, the functional impact of the most validated AD-associated variant, rs3851179, remains unclear. Here, we examined PICALM mRNA and protein expression in post-mortem AD brains with reference to rs3851179 genotype. We found that PICALM mRNA levels were significantly increased in

Cognitive Impairment, Dementia and Depression in Older Adults.

This narrative review integrates longitudinal cohort studies, neuroimaging and biomarker research, and major clinical trials to examine how depression and cognitive decline interact across the dementia continuum. Depression and cognitive impairment frequently co-occur in late life and exhibit substantial clinical and biological overlap. Meta-analytic and large population-based cohort studies consistently show that late-life depression increases the risk of mild cognitive impairment and dementia, with stronger associations observed for vascular dementia than for Alzheimer's disease. Characterizing TDP-43 involvement in vascular dementia.

INTRODUCTION: Vascular dementia (VaD) is a major therapeutic challenge. Tar DNA-binding protein 43 (TDP-43), known for its role in neurodegeneration, may contribute to VaD pathogenesis under chronic cerebral hypoperfusion (CCH). This study investigates TDP-43 dysregulation in VaD. METHODS: TDP-43 and phosphorylated TDP-43 (pTDP-43) expression and localization were assessed in a VaD animal model, neuronal cells exposed to oxygen-glucose deprivation (OGD), and post mortem human brain tissues. RESULTS: Bilateral Common Carotid Artery Stenosis (BCAS)-induced CCH led to increased pTDP-43 and aber
