Effects of linear vestibular stimulation on body-rocking behavior in adults with profound mental retardation.

The effects of linear vestibular stimulation on body-rocking behaviors in adults with profound mental retardation were studied. A single-system multiple base-line design across 3 subjects was used. The design included three phases spread over a period of 9 weeks. An interval time sampling procedure was used to collect data. Data from the 3 subjects were graphed and interpreted through visual inspection. Trend lines were computed with the celeration line approach to supplement the visual inspection of the data. The results demonstrated a decrease in body-rocking behaviors during the treatment a

Severe behaviour problems associated with rapid cycling bipolar disorder in two adults with profound mental retardation.

The authors report the clinical histories of two adults with profound mental retardation, features of rapid cycling bipolar disorder, and periodic maladaptive behaviour. In each case, primary features of mania and depression were identified, operationally defined and measured with an ongoing data system, which was used to track SIB and aggression. In the first case, data analysis across days showed that 1-week episodes of depressive features alternated with 2-week episodes of manic features and that SIB was only associated with the depressive features. In the second case, episodes of manic and

Causal heterogeneity in isolated lissencephaly.

We report clinical, cytogenetic, and molecular studies in 65 patients with isolated lissencephaly sequence (ILS). All had type I lissencephaly of varying severity and a grossly normal cerebellum. Some had additional brain abnormalities. Facial appearance was essentially normal. All had severe to profound mental retardation, seizures, hypotonia that evolved into spasticity, and feeding difficulties. Clinical and laboratory studies demonstrated etiologic heterogeneity. Molecular studies detected microdeletions in chromosome band 17p13.3 in six of 44 patients tested, confirming that deletion of a

Teaching adults with severe and profound retardation to exit their homes upon hearing the fire alarm.

Prompting, modeling, and differential reinforcement with client-chosen rewards were used to teach 3 nonverbal people with severe to profound mental retardation to exit their group homes at the sound of the house fire alarm, using a multiple baseline design. All 3 participants learned to exit independently in less than 2 min in all experimenter-initiated surprise fire drills and in the majority of staff-initiated surprise fire drills. Each participant was also able to exit from five areas of the house from which teaching was not done.

Occupational conditions of ward staff and quality of residential care for individuals with mental retardation.

Effects of occupational conditions of ward staff on the quality of residential care for individuals with mental retardation was assessed. Three questions were asked: (a) Does type of contract under which staff members are employed differentially affect their distribution of activities? (b) Does length of duty have an effect on their distribution of activities? (c) Does the number of staff members present on the living group influence quality of care? Over a 40-week period, data were collected on 30 ward staff members who were responsible for 39 residents with severe and profound mental retarda

Mortality in a large southeastern facility for persons with mental retardation.

Mortality rates for institutionalized persons with mental retardation were presented. Rates were provided for two time intervals, 1974 through 1979 and 1980 through 1985, and by age, race, and gender. Consistent differences between black and white residents or by gender were not indicated. However, significant improvement in mortality did occur between the two time periods. Persons with profound retardation were found to have higher mortality than those whose retardation was mild to severe. Respiratory disease was the most prevalent cause of death among the individuals with profound mental ret

Therapeutic effect of fluoxetine on naltrexone-resistant self-injurious behavior in an adolescent with mental retardation.

ABSTRACT A significant and sustained reduction in self-injurious behavior (SIB) was observed in a 17-year-old male with severe to profound mental retardation who was treated in an open clinical trial with fluoxetine. The 45-55% reduction in SIB was accompanied by improvement in affective stability, motor capacities, and social activity. No adverse effects or diminishing therapeutic effects were observed during 2 years of treatment at 40 mg daily. Twelve weeks before the start of the fluoxetine trial, this adolescent had been found to be unresponsive to the opiate receptor blocker naltrexone in

Mental retardation, acromegalic face, and megalotestes in two half-brothers: a specific form of X-linked mental retardation without fra(X) (q)?

We describe a family with two half-brothers affected with severe mental retardation. The phenotype in the affected individuals is characterized by apparent acromegaly, profound mental retardation, and hyperactivity. The mother has analogous but less severe facial anomalies and mild mental impairment. Screening for fra(X) (q) was negative in peripheral lymphocytes using methotrexate for fra(X) enhancement. The clinical findings in our patients are similar to those described by Fryns et al. [1986] in two patients with acquired lesions of the central nervous system. CT investigations in one of ou

Comparative effects of gentle teaching and visual screening on self-injurious behaviour.

Gentle teaching and visual screening procedures have been used to control severe behaviour problems in persons with mental retardation. An alternating treatments design was used to compare gentle teaching, visual screening and a task-training condition in the reduction of high levels of self-injury of an adult with profound mental retardation. Following baseline, a task-training condition using standard behavioural techniques was implemented to establish the effects of training the subject on age-appropriate tasks. Results showed a modest reduction in self-injury. This was followed by an alter

Communication profiles of two types of gesture using nonverbal persons with severe to profound mental retardation.

A structured communication sampling procedure was used to measure the form and function characteristics of intentional communication acts produced by nonverbal adults with severe mental retardation. Four "contact" subjects (who communicated only with contact gestures) and 4 "distal" subjects (who used distal as well as contact gestures) participated in this study. All subjects produced communication acts that were coded as initiations, and all subjects produced protoimperative-type communication acts. However, contact subjects produced no protodeclarative-type communication acts, whereas all d

Progressive encephalopathy with edema, hypsarrhythmia, and optic atrophy (PEHO syndrome).

We describe 14 patients, from 11 families, who have a progressive encephalopathy with early onset. The clinical signs of the disease are severe hypotonia, convulsions with hypsarrhythmia, profound mental retardation, hyperreflexia, transient or persistent edema, and optic atrophy. These findings and the characteristic dysmorphic features allow recognition of these patients, although no basic metabolic defect has been found. Microcephaly and atrophy of the brain develop, especially in the cerebellar and brain stem areas. An autosomal recessive mode of inheritance is likely.

Factor structure of the Motivation Assessment Scale for persons with mental retardation.

The rating scale used to assess the motivators of maladaptive behaviors in persons with mental retardation was the Motivation Assessment Scale. In the current study, we validated the factor structure of the scale on a sample of 118 subjects with predominately severe or profound mental retardation. They exhibited deviant behaviors such as self-injurious and tantrum behavior, aggression, and passivity. The results of the factor analysis with varimax rotation validated the assumptions of the developers of the scale that the motivators could be grouped into sensory, escape, attention, and tangible

Diagnostic dilemmas. Results of screening tests for congenital hypothyroidism.

The natural history of congenital hypothyroidism has changed drastically since the institution of newborn screening in North America, Europe, and Japan. Before screening was available, diagnosis of this condition was often delayed. The IQ of 65% of all patients with congenital hypothyroidism was below 85, and 19% were profoundly retarded (IQ less than 15). In two large screening programs in which long-term IQ data are available, no child has tested below the IQ of 74. Furthermore, a comparison of affected children treated within the first month of life with matched controls demonstrated no sig

Generalized cortical dysplasia. Clinical and pathologic aspects.

Three children with profound mental retardation and intractable seizures died at ages 10 months, 3 years, and 7 years, respectively. Complete examination of their brains showed generalized cortical dysplasia, without any major malformation of the external gyral pattern. The neuropathologic features of cortical dysplasia include abnormally thickened cortex with indistinct demarcation of the gray-white matter junction. In many areas, the cortex contained increased numbers of large neurons with disordered cortical lamination. Heterotopic neurons were scattered throughout the white matter with dec

Descriptive analysis of interactions between co-workers and supported employees.

Varying types of interactions between workers without handicaps and supported employees were described. Results indicated that co-workers interacted extensively with persons who had mental retardation. Surprising findings included high levels of advocacy between co-workers and persons with severe or profound mental retardation and a relatively high percentage of co-workers assuming evaluation and training roles. Results were discussed in relation to the work behavior literature, which indicates that, in general, co-workers provide considerable natural support for employees across work environm

Cross-validation of the factor structure of the Aberrant Behavior Checklist for persons with mental retardation.

The original factor structure of the Aberrant Behavior Checklist was cross-validated with an American sample of 470 persons with moderate to profound mental retardation, including nonambulatory individuals. The results of the factor analysis with varimax rotation essentially replicated previous findings, suggesting that the original five factors (Irritability, Lethargy, Stereotypic Behavior, Hyperactivity, and Inappropriate Speech) could be cross-validated by factor loadings of individual items. The original five scales continue to show high internal consistency. These factors are easily inter

Analog language teaching versus natural language teaching: generalization and retention of language learning for adults with autism and mental retardation.

Examined the effects of two instructional methods on language generalization and long-term retention in 23 adults with autism and severe to profound mental retardation. Analog language teaching employed discrete trials in a controlled setting concentrating on discrimination and identification of materials. Natural language teaching emphasized instruction through interactions that occurred incidentally to training students in the use of materials to perform functional tasks. Assessments were conducted under conditions favoring analog teaching to assure against partiality toward natural language

Multicentric study on genetic causes of mental retardation in India. ICMR Collaborating Centres & Central Co-ordinating Unit.

A study on 1314 children with mental retardation (MR) without an obvious environmental cause was carried out at Bangalore, Bombay, Delhi and Lucknow to determine the extent and pattern of genetic causes of mental retardation in different parts of India. In all, 42.3 per cent patients had mild, 25.3 per cent moderate, 19.2 per cent severe and 13.1 per cent profound mental retardation. Among 1314 patients, the chromosomal anomalies were found in 23.7 per cent, metabolic defects in 5.0 per cent and an identificable genetic syndrome in 11.6 per cent of the patients. In the remaining 59.7 per cent

An apparently new mental retardation syndrome in three elderly sisters.

Three elderly sisters with profound mental retardation in association with the clinical features of microcephaly, short stature, brachydactyly type D, flattened occiput, down-slanting palpebral fissures, low-set large ears, broad prominent nose and kyphoscoliosis have been investigated. Each was more than 60 years of age and their clinical features were strikingly similar. The disorder has several manifestations in common with Rubenstein-Taybi syndrome and appears to be inherited as an autosomal recessive in this family.

Supported employment for persons with severe and profound mental retardation: a critical analysis.

An analysis of the employment outcomes of 109 persons with severe profound mental retardation was undertaken. The mean age of the group was 28 years and mean intelligence quotient (Stanford-Binet) was 30. The data were drawn from over 90 local community programmes in the United States. Results indicated that all persons were in supervised residential situations and that 93% of the total were competitively employed with the mean wage being $3.63 per hour. Mean hours of weekly employment were 22 hours. After 12 months of placement, 81.5% of the clients were still employed. Supported employment h
